RENAL HISTOPATHOLOGY AFTER CHOLEDOCHOJEJUNOSTOMY. (EXPERIMENTAL STUDY IN RABBITS)

Abstract

THIS EXPERIMENTAL STUDY CONCERNS THE HISTOLOGICAL AND FUNCTIONAL RENAL CHANGESAFTER CHOLEDOCHOJEJUNOSTOMY. IT WAS PERFORMED ON HEALTHY RABBITS. INFLAMMATORY INFILTRATION ON THE PORTAL ZONES WAS FOUND ON THE FIRST POSTOPERATIVE DAY. ONE WEEK AFTER THE OPERATION IT WAS FOUND THAT THESE CHANGES WERE IN A HIGHER LEVEL. ALSO THERE WAS LIVER SCLEROSIS, INFLAMMATORY INFILTRATION ON THE PORTAL SPACE AND PROLIFERATIVE CHANGES ON THE BILE DUCT. IN THE KIDNEYS AN OEDEMA IN THE URINARY TUBULES WAS OBSERVED FROM THE FIRST POSTOPERATIVE DAY AND STATISTICALLY MORE SIGNIFICANT IN THE 15TH DAY. IT WAS ALSO FOUND THE GENERATION OF VACUOLES ON THE ONES. THERE WAS ALSO NO STATISTICALLY PROVED RELATION BETWEEN THE KIDNEY AND THE LIVER HISTOLOGICAL LESIONS. THE FOCAL HISTOLOGIC LESIONS ONTHE RENAL PARENCHYMA SEEMS TO BE RELATED TO THE CHOLEDOCHOJEJUNOSTOMY. IT ISBELIEVED THAT THESE HISTOLOGIC CHANGES ARE DIFFERENT FROM THE ONES OBSERVED ON THE HEPATORENAL SYNDROME ACCORDING TO THE BIBLIOGRAPHY. WE BELIEVE THAT THE CHOLEDOCHOPEPTIC ANASTOMOSIS IS RESPONSIBLE FOR THESE CHANGES ON THE RENAL PARENCHYMA DUE PROBABLY TO THE IMPAIRMENT OF THE BILE SECRETION AND FLOW INTO THEGUT.
