A  decrease in high-density lipoprotein (HDL) happens in type 2 diabetes mellitus. The cytokines TNFa and IL-6 are proinflammatory cytokines that are elevated in the blood in many patients with DM. Turmeric is one of the natural ingredients with antioxidant, anti-cholesterol, and anti-inflammatory properties through its active substance, curcumin. Objective: To prove the effectiveness of the combination of metformin and turmeric extract on HDL levels, TNFa levels, and IL-6 levels in Streptozotocin-Nicotinamide-induced Wistar rats. Methods: The study used a post-test-only controlled group design. The subjects were 24 male Wistar rats which were randomly divided into four groups, namely K1, K2, P1, and P2. The whole group was a type 2 DM model induced by streptozotocin-nicotinamide. Group K2 was given metformin 45mg/KgBW/day, group P1 was given turmeric extract at a dose of 200 mg/KgBW/day, and group P2 was given a combination of metformin and turmeric extract. On day 15, blood was drawn for HDL, IL-6, and TNFa examination. The data were analyzed using the one-way Anova test. Results: The mean HDL level in the P2 group (72,4±33,31) was the highest compared to the other three groups. The lowest mean TNFa level among the four groups was the P2 group (7,56±0,44). The lowest mean levels of IL-6 were obtained in the P2 group (58,82±2,15). One-way Anova test on HDL, IL-6, and TNFa levels showed a significant difference between groups with p-value = 0.000. Conclusion: The combination of metformin and turmeric extract can increase HDL levels and reduce IL-6 levels and TNFa levels better than giving metformin or turmeric extract alone. Keywords:  turmeric extract, HDL, IL-6, TNF

Rheumatic Heart Disease (RHD) has high morbidity and mortality. Cardiac valve disorders after rheumatic fever are the leading cause of heart failure in children and young adults. A comprehensive understanding of RHD can help early diagnosis and provide optimal management in the future. It has been reported that a 55-year-old man came to the heart clinic of Bhayangkara Hospital Denpasar for routine control of heart disease. The patient has been diagnosed with rheumatic heart disease since 2018. The patient complains of shortness of breath when walking long distances and doing strenuous activities. The patient was diagnosed with CHF FC II ec RHD + MS moderate-severe (Wilkins score 8) + AF NVR (CHA2DS2VASc 3) and treated with heart failure and antiplatelet medication. Mitral stenosis due to RHD produces the typical pathological features seen in Transthoracic echocardiography (TTE), including fibrous thickening and calcification of the valve leaflets, fusion of the commissures, and thickening and shortening of chordae tendineae. Chronic stress overload on the LA due to MS can cause LA dilatation and trigger AF. RHD patients with AF are recommended for anticoagulation for stroke prevention. Patients with acute rheumatic fever clinical features should be evaluated further and given long-term therapy to prevent more severe heart problems. Keywords:  atrial fibrillation, mitral stenosis, rheumatic  heart  diseas

The use of ventilators during the COVID-19 pandemic has increased the incidence of Ventilator-Associated Pneumonia (VAP). This is accompanied by a trend of antibiotic resistance resulting from irrational use. The emergence of multi-drug resistant (MDR) bacteria during the pandemic presents a challenge due to limited antibiotic options. Objective: To determined the pathogenic bacterial profile and antibiotic susceptibility in Ventilator Associated Pneumonia (VAP) patients with COVID-19 infection. Methods: This is a descriptive cross-sectional study with a retrospective approach using secondary data from the medical records of UKRIDA Hospital patients from January 2021 to August 2021. Results: Most of the study sample had characteristics of being male (71.9%), aged 25-64 years (78.1%), overweight (40.6%), and hypertension (40.6%) as the comorbid. Gram-negative bacteria is the pre-dominated bacteria in this study, with Acinetobacter baumannii, Klebsiella pneumoniae, and Stenotrophomonas maltophilia being the most isolated bacteria. Antibiotics such as colistin still showed good susceptibility to Acinetobacter baumannii and Klebsiella pneumoniae, with 75% and 90% sensitivities, respectively. Antibiotics such as linezolid, tigecycline, and gentamicin-synergy still showed (100%) sensitivity to all Gram-positive bacteria. Conclusion: The discovery of MDR bacteria in patients with VAP and the limited availability of susceptible antibiotics highlights the importance of conducting antibiotic resistance surveillance and infection control, especially in the era of COVID-19.Keywords:  antibiotic susceptibility, bacteria profile, COVID-19, VA

Mesenchymal Stem Cells Wharton's Jelly (MSC-WJ) has become a new hope for Alzheimer's disease due to its capability to suppress Amyloid-beta (Aβ) plaques formation. It is found that people with Alzheimer's disease have a lower expression of Sirtuin-1 because of the oxidative stress that was produced by Aβ plaques. Objective: To observe the Sirtuin-1 gene in Alzheimer's disease rat models that AlCl3 was inducing after being given MSC-WJ. Methods: This was an experimental study with a post-test-only control group design, including three groups with a total sample of 18 samples of Alzheimer RNA rat models. Group-1without AlCl3 and MSC-WJ; Group-2 with AlCl3; Group-3 AlCl3 and MSC-WJ. Mean value of the Sirtuin-1 gene was obtained by the Image-J application using a semiquantitative method which was then tested using the One Way ANOVA test. The study was significant if the p-value < 0,05. Results: This study found that the mean ± standard error value of the Sirtuin-1 gene in three groups were 0.18±0.04; 0.16±0.03; and 0.36±0.06 respectively. The statistical test showed a significant difference between the three groups (p = 0.014). Conclusion: There was an increased Sirtuin 1 gene expression in Alzheimer’s rat Model after being given Mesenchymal Stem Cells Wharton’s Jelly. Keywords:  Alzheimer, MSC-WJ, Sirtuin-
