    Cultural distance, gender and praise in peer review

    Background: Understanding review comments holds significant importance within the realm of scientific discourse. This study aims to conduct an empirical analysis of factors associated with praise in peer review.Methods: The study involved manual labeling of "praise" in 952 review comments drawn from 301 articles published in the British Medical Journal, followed by regression analysis.Results: The study reveals that authors tend to receive longer praise when they share a cultural proximity with the reviewers. Additionally, it is observed that female reviewers are more inclined to provide praiseConclusions: In summary, these discoveries contribute valuable insights for the development of a constructive peer review process and the establishment of a more inclusive research culture

    Suicidal Ideation and Suicide Attempts After Direct or Indirect Psychotherapy A Systematic Review and Meta-Analysis

    Importance Suicidal ideation and suicide attempts are debilitating mental health problems that are often treated with indirect psychotherapy (ie, psychotherapy that focuses on other mental health problems, such as depression or personality disorders). The effects of direct and indirect psychotherapy on suicidal ideation have not yet been examined in a meta-analysis, and several trials have been published since a previous meta-analysis examined the effect size of direct and indirect psychotherapy on suicide attempts. Objective To investigate the effect sizes of direct and indirect psychotherapy on suicidal ideation and the incidence of suicide attempts. Data Sources PubMed, Embase, PsycInfo, Web of Science, Scopus, and the Cochrane Central Register of Controlled Trials were searched for articles published up until April 1, 2023. Study Selection Randomized clinical trials of psychotherapy for any mental health problem, delivered in any setting, compared with any control group, and reporting suicidal ideation or suicide attempts were included. Studies measuring suicidal ideation with 1 item were excluded. Data Extraction and Synthesis PRISMA guidelines were followed. Summary data were extracted by 2 independent researchers and pooled using 3-level meta-analyses. Main Outcomes and Measures Hedges g was pooled for suicidal ideation and relative risk (RR) was pooled for suicide attempts. Results Of 15 006 studies identified, 147 comprising 193 comparisons and 11 001 participants were included. Direct and indirect psychotherapy conditions were associated with reduced suicidal ideation (direct: g, -0.39; 95% CI, -0.53 to -0.24; I-2, 83.2; indirect: g, -0.30; 95% CI, -0.42 to -0.18; I-2, 52.2). Direct and indirect psychotherapy conditions were also associated with reduced suicide attempts (direct: RR, 0.72; 95% CI, 0.62 to 0.84; I-2, 40.5; indirect: RR, 0.68; 95% CI, 0.48 to 0.95; I-2, 0). Sensitivity analyses largely confirmed these results. Conclusions and Relevance Direct and indirect interventions had similar effect sizes for reducing suicidal ideation and suicide attempts. Suicide prevention strategies could make greater use of indirect treatments to provide effective interventions for people who would not likely seek treatment for suicidal ideation or self-harm

    Preconception carrier screening in couples seeking IVF:exploring the patient perspective

    RESEARCH QUESTION: What is the level of understanding, and what are the attitudes and considerations regarding preconception carrier screening (PCS) among couples seeking IVF or intracytoplasmic sperm injection (ICSI)? DESIGN: A mixed-methods design was used. Nine interviews were conducted with couples or individual partners (n = 16) who had an initial consultation for IVF/ICSI in the 2 years preceding this study. A questionnaire was completed by 115 participants. No actual PCS was offered. RESULTS: All interviewed couples expressed a positive attitude towards PCS, and over half of the respondents stated that they would pursue or seriously consider pursuing PCS if possible. Some couples falsely believed that PCS could identify a cause for their fertility problems and increase their chance of conceiving. The desire to make an informed reproductive decision was the most important argument in favour of PCS. The primary argument against PCS was the apprehension of being confronted with reproductive dilemmas. The longer the delay to IVF/ICSI treatment required to perform PCS, the more couples would be inclined to decline screening. Participants indicated that they would prefer to receive information about PCS from a medical specialist at an early stage in their IVF/ICSI treatment. CONCLUSION: Although attitudes towards PCS were generally positive, some concerns were raised about treatment delays and potential reproductive dilemmas, and some couples had misconceptions about the purpose of screening within the context of their IVF/ICSI treatment. These findings highlight the importance of tailoring information and counselling to the specific needs of couples seeking IVF/ICSI

    Citrus pectins impact the function of chicken macrophages

    The restrictions on excessive use of antimicrobials in the poultry industry have led to the search for alternative strategies including nutritional interventions to enhance gut health with the ultimate aim to prevent gut infections. Pectins as prebiotics have shown beneficial effects on gut health in humans and mice by improving the gut barrier function, altering the gut microbiota, and by modulating the gut immune response. However, little is known about immunomodulatory properties of pectins in chickens. The present in vitro study assessed the effect of three pectins (SPE6, SPE7, SPE8) differing in methyl esterification, on responsiveness of the chicken macrophage cell line HD11 cells and primary monocyte derived macrophage from the blood, through interaction with chicken TLRs. All three pectins increased gene expression of iNOS and IL10 in chicken macrophages. Differences in immunomodulatory activity between the three pectins were observed in other assays. The low methoxyl pectin (SPE8) interacted with TLR4 leading to the production of NO, but also to increased phagocytosis of E. coli, while high methoxyl pectins SPE6 and SPE7 did not activate TLR4. All three pectins were able to attenuate PAM3CSK4 induced activation of chicken macrophages as measured by decreased NO production and phagocytosis. Additional studies using ITC and flow cytometry suggest that the inhibiting properties of pectins (SPE6, SPE7) on macrophages are due to pectins occupying TLR2 and blocking PAM3CSK4 to activate chicken macrophages, whereas SPE8 actually binds to the TLR2 ligand and that way attenuates the PAM3CSK4 induced activation. Based on these immunomodulatory properties observed in this study, these pectins may in the future be suitable as feed additive for the treatment and prevention of inflammatory disorders in poultry

    Retinal vessel analysis to assess microvascular function in the healthy eye:A systematic review on the response to acute physiological and pathological stressors

    The retina allows noninvasive in vivo assessment of the microcirculation. Autoregulation of the retinal microvasculature meets the changing requirements of local metabolic demand and maintains adequate blood flow. Analysis of the retinal vascular reactivity contributes to the understanding of regulatory physiology and its relationship to the systemic microcirculation. We conducted a literature review on the effect of different acute stimuli onto the retinal vasculature was conducted in accordance with the PRISMA guidelines. A literature search between 1-1-2005 and 17-10-2022 was performed in Medline, Embase, Web of Science and the Cochrane Library. We report the retinal vascular behavior of healthy individuals in response to both physiological and pathological stressors in 106 included articles. Tables of methodological characteristics for each stressor are provided. Hypoxia, hypercapnia, high altitude, flicker light stimulation, rise of core temperature, blood pressure lowering, and the condition immediately after endurance exercise associate with larger retinal vessels. Hyperoxia, hypocapnia, blood pressure rise (Bayliss effect), and the condition during isometric exercise associate with smaller retinal vessels. The retinal vasculature is highly reactive to physiological and pathological stressors. This autoregulatory capacity is hypothesized to be a source of biomarkers for vascular health. Dynamic and static retinal vessel analysis are non-invasive methods to assess this (micro)vascular function. Exploring its diagnostic potential and application into clinical practice requires the development of standardized assessment methods, for which some recommendations are made

    Diffusion-derived intravoxel-incoherent motion anisotropy relates to CSF and blood flow

    This study investigates the feasibility of multi-b-value, multi-directional diffusion MRI for assessing the anisotropy of the cerebral pseudo-diffusion (D*)-tensor. We examine D*-tensor's potential to (1) reflect CSF and blood flow, and (2) detect microvascular architectural alterations in cerebral small vessel disease (cSVD) and aging. Methods: Multi-b-value diffusion MRI was acquired in 32 gradient directions for 11 healthy volunteers, and in six directions for 29 patients with cSVD and 14 controls at 3 T. A physics-informed neural network was used to estimate intravoxel incoherent motion (IVIM)–DTI model parameters, including the parenchymal slow diffusion (D-)tensor and the pseudo-diffusion (D*)-tensor, from which the fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) were derived. Comparisons of D*-tensor metrics were made between lateral, third, and fourth ventricles and between the middle cerebral arteries and superior sagittal sinus. Group differences in D*-tensor metrics in normal-appearing white matter were analyzed using multivariable linear regression, correcting for age and sex. Results: D*-anisotropy aligned well with CSF flow and arterial blood flow. FA(D*), MD(D*), AD(D*), and RD(D*) were highest in the third, moderate in the fourth, and lowest in the lateral ventricles. The arteries showed higher MD(D*), AD(D*), and RD(D*) than the sagittal sinus. Higher FA(D*) in the normal-appearing white matter was related to cSVD diagnosis and older age, suggesting microvascular architecture alterations. Conclusion: Multi-b-value, multi-directional diffusion analysis using the IVIM–DTI model enables assessment of the cerebral microstructure, fluid flow, and microvascular architecture, providing information on neurodegeneration, glymphatic waste clearance, and the vasculature in one measurement.</p

    Health promotion in the school context:a scientific mapping of the literature

    Purpose: This study presents an overview of research literature on health promotion in schools, utilizing metadata extracted from 4,328 publications indexed in the Scopus database over the past 35 years. Design/methodology/approach: A bibliometric approach was used to analyze the development and current state of using publication and citation data. A structured keyword search was conducted in the Scopus database to retrieve relevant publications in the field. Frequency counts, rank-ordered tables and time series charts were used to illustrate the dynamic growth of publication and citation data, the core journals, the leading countries and the most frequently used keywords in research on health promotion in school contexts. A series of social network analyses was conducted to explore and visualize the social, intellectual and conceptual structure of the field. Findings: Findings demonstrate that health promotion in the school context is a growing research field that has gained significant momentum in recent years. The research in this field is widely distributed internationally, but the research output is dominated by the US and other English-speaking countries. The study reveals a trend toward increased collaboration among research groups. The level of international collaboration varies. The research field is highly interdisciplinary, and the main research themes addressed in the literature include mental health, well-being and quality of life; health behaviors; oral health education; sexual and reproductive education and general health promotion and health education in schools. Originality/value: This is the first study to map the development of a research field with growing recognition. It provides a comprehensive overview of the emerging field of health promotion in the school context and its progress over time, contributing to the organization of the research domain. The study demonstrates the need for a new framework for health promotion research that supports the sustainability of health promotion research in schools

    Cerebrovascular Function in Sporadic and Genetic Cerebral Small Vessel Disease

    Objective: Cerebral small vessel diseases (SVDs) are associated with cerebrovascular dysfunction, such as increased blood–brain barrier leakage (permeability surface area product), vascular pulsatility, and decreased cerebrovascular reactivity (CVR). No studies assessed all 3 functions concurrently. We assessed 3 key vascular functions in sporadic and genetic SVD to determine associations with SVD severity, subtype, and interrelations. Methods: In this prospective, cross-sectional, multicenter INVESTIGATE-SVDs study, we acquired brain magnetic resonance imaging in patients with sporadic SVD/cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL), including structural, quantitative microstructural, permeability surface area product, blood plasma volume fraction, vascular pulsatility, and CVR (in response to CO 2) scans. We determined vascular function and white matter hyperintensity (WMH) associations, using covariate-adjusted linear regression; normal-appearing white matter and WMH differences, interrelationships between vascular functions, using linear mixed models; and major sources of variance using principal component analyses. Results: We recruited 77 patients (45 sporadic/32 CADASIL) at 3 sites. In adjusted analyses, patients with worse WMH had lower CVR (B = −1.78, 95% CI −3.30, −0.27) and blood plasma volume fraction (B = −0.594, 95% CI −0.987, −0.202). CVR was worse in WMH than normal-appearing white matter (eg, CVR: B = −0.048, 95% CI −0.079, −0.017). Adjusting for WMH severity, SVD subtype had minimal influence on vascular function (eg, CVR in CADASIL vs sporadic: B = 0.0169, 95% CI −0.0247, 0.0584). Different vascular function mechanisms were not generally interrelated (eg, permeability surface area product~CVR: B = −0.85, 95% CI −4.72, 3.02). Principal component analyses identified WMH volume/quantitative microstructural metrics explained most variance in CADASIL and arterial pulsatility in sporadic SVD, but similar main variance sources. Interpretation: Vascular function was worse with higher WMH, and in WMH than normal-appearing white matter. Sporadic SVD-CADASIL differences largely reflect disease severity. Limited vascular function interrelations may suggest disease stage-specific differences. ANN NEUROL 2025;97:483–498.</p

    Reproductive options and genetic testing for patients with an inherited cardiac disease

    In the past decade, genetic testing for cardiac disease has become part of routine clinical care. A genetic diagnosis provides the possibility to clarify risk for relatives. For family planning, a genetic diagnosis provides reproductive options, including prenatal diagnosis and preimplantation genetic testing, that can prevent an affected parent from having a child with the genetic predisposition. Owing to the complex genetic architecture of cardiac diseases, characterized by incomplete disease penetrance and the interplay between monogenic and polygenic variants, the risk reduction that can be achieved using reproductive genetic testing varies among individuals. Globally, disparities, including regulatory and financial barriers, in access to reproductive genetic tests exist. Although reproductive options are gaining a prominent position in the management of patients with inherited cardiac diseases, specific policies and guidance are lacking. Guidelines recommend that prenatal diagnosis and preimplantation genetic testing are options that should be discussed with families. Health-care professionals should, therefore, be aware of the possibilities and feel confident to discuss the benefits and challenges. In this Review, we provide an overview of the reproductive options in the context of inherited cardiac diseases, covering the genetic, technical, psychosocial and equity considerations, to prepare health-care professionals for discussions with their patients
