(PHONE METROPOLITAN 0149) We, the undersigned, American citizens desiring to become members of the American Feder-ation of the Physically Handicapped, Inc., hereby apply for a Charter as a duly authorized Lodge. If our request is granted by the Federation, We agree to abide by the rules and by-laws of the American Federation of the Physically Handicapped, Inc. We agree to advocate, in every legal way, advancement of the interests of the Physically Handicapped citizens of the United States. We agree to forbid any activity in our lodge detrimental to the Federation or to the Physi-cally Handicapped in general, and will eliminate any person, or group, from our membership, who, in any manner, reflect discredit upon ourselves, or the movement. We further agree to expand our membership as rapidly as possible, and to promote a bet-ter understanding of the problems of the Physically Handicapped, and suggest remedies therefor, by education, and enlistment of public interest through such activities as may be engaged in, by the Federation. As a Lodge, we agree to pay our financial obligations to the American Federation of the Physically Handicapped, Inc., promptly, when due, covering-1. Charter Fee (to accompany this form), $10.00. 2. Initiation Fee, $1.00, on each PARTICIPATING MEMBER. 3. As per capita tax, we will remit to the National Office, in advance 1. PARTICIPATING MEMBERSHIP, open only to Physically Handicapped. Initiation Fee, $1.00. Dues, $3.00 per year, if paid in advance, or, 35c per month. (Following classes are open to either Handicapped or Non-Handicapped.) 2. ASSOCIATE MEMBERSHIP (a) YEARLY HONORARY MEMBERSHIP, $10. (b) YEARLY CONTRIBUTING MEMBERSHIP, $5. (c) HONOR ROLL ANNUAL CONTRIBUTION, $1. 4. Purchase of literature and supplies by Lodges from the National Office, as per price-lists furnished by Federation. 5. Insignia and Emblems of the Federation may be used only by permission of the National Office, by Lodges and members in good standing. 6. ALL REMITTANCES by Lodges should be sent to National Office so as to arrive by the 20th day of each month. 7. The American Federation of the Physically Handicapped, Inc., specifically reserves the right to reject any application for Charter, or for individual membership.
