Methods of in-service Education to Address the Difficulties Experienced by Novice Public Health Nurses with Nursing Experience Working in Local Governments
目的:看護師経験を有し自治体で働く新任保健師が感じた困難を明らかにし,現任教育への示唆を得る.
方法:看護師経験を有し自治体で働く新任保健師 11 名を対象に半構造化面接を行い,質的帰納的に分類した.
結果:保健師活動の困難は 66 コード,25 サブカテゴリ,13 カテゴリが生成され,【看護師との業務内容の違いから感じる困難】,【保健師の役割や専門性に対して感じる困難】,【行政事務に不慣れな事に関する困難】,【看護師との教育体制の違いに対する困難】の 4 つの特徴に整理された.
結論:看護師経験を有し自治体で働く新任保健師の困難に対する現任教育は,マネジメント能力の向上により看護師業務とは異なる保健師業務への効果的な適応,個人と集団を同時に対象とする保健師の役割や専門性の認識,事業運営のための事務や行政文書の作成,行政事務への適応,看護師の教育を受けた経験を考慮した早期の自立支援を視野に入れる必要があると示唆された.Objectives: To identify the difficulties faced by novice public health nurses with nursing experience working in municipalities and generate suggestions for in-service education.
Methods: Semi-constructive interviews were conducted with 11 individuals with nursing experience who had been employed as public health nurses in local municipalities for up to 3 years concerning the difficulties they experienced in their public health nursing activities. The results were then classified both qualitatively and inductively.
Results: The difficulties perceived in the activities of public health nurses were grouped into 66 codes, 25 subcategories, and 13 categories. The 13 categories were organized into the following four characteristics: [Difficulties felt due to differences in work content from that of nurses], [Difficulties felt regarding the role and expertise of public health nurses], [Difficulties related to unfamiliarity with administrative work], and [Difficulties related to differences in the education system from that of nurses].
Conclusions: In-service education for novice public health nurses who have nursing experience and work in local governments should include effective adaptation to public health nursing work, which is different from nursing work, through improving management skills, recognition of the role and expertise of public health nurses who target individuals and groups at the same time, and adaptation to administrative work, such as that for business management and the preparation of administrative documents. These findings suggest that early support for self-support is necessary, taking into account the experience of nurses who have been educated as nurses.原著articl
Proposal for the Selection of Picture Books for Spiritual Care: Focusing on Three Phases of Bibliotherapy
目的:スピリチュアルケアにおける有益な絵本活用に向けて,スピリチュアルケアの理論と読書療法の類似性に着目し選定した絵本を一提案として示す.
方法:入手可能な国内外の死をテーマとした対象絵本 69 作品を精読した後,ケア対象者である読者が【同一視】できる登場人物と共に,【感情の浄化】【洞察】という読書療法における各プロセスを疑似体験できると判断した表現を抽出し,それらすべてを包含した絵本を選定する.
結果:ケア対象者が【同一視】する登場人物と【感情の浄化】【洞察】という読書療法のプロセスを辿りうると考えられる絵本を 15 作品選定することができた.
結論:物語を通じて,ケア対象者が【同一視】できる登場人物と共に,疑似的に苦悩や悲しみの感情を表出【感情の浄化】し,死に対する受容や自己変容【洞察】へと促す絵本は,危機的状況を克服し新たな人生の意味を見出し,アイデンティティを確保するための一助になり得ることから,スピリチュアルケアにおける絵本選定の一提案を示していると考える.Objective: This paper aims to propose the effective selection of picture books for spiritual care, with a focus on the similarities between the theories of spiritual care and bibliotherapy.
Methods: After carefully reading 69 available picture books on the theme of death in Japan and abroad, we selected those that included all of the following characteristics: characters with whom the readers, the subjects of care, could “identify”and expressions through which they could vicariously experience each process of bibliotherapy: “purification of feelings (catharsis)” and “insight.”
Results: We were able to select 15 picture books that the subjects of care could trace through the bibliotherapy process of “purification of feelings (catharsis)” and “insight” with characters with whom they could “identify”.
Conclusion: Picture books that encourage care recipients (the subjects of care) to express feelings of suffering and sorrow with characters with whom they can identify through stories, to purify their feelings “purification of feelings (catharsis),” to gain acceptance and understanding of death and to discover a new self through self-transformation (“insight”) may help them find a new meaning of life from a crisis in spiritual care. Therefore, it is thought that the picture books focusing on the similarities between the theories of spiritual care and bibliotherapy selected in this study can be proposed as effective selections for spiritual care.その他articl
Research on the Effects of Three Kinds of Ultraviolet Rays on the Shielding Properties of Containers Using E. Coli and Medical 6% Liquid Sodium Hypochlorite
UVC の殺菌灯としての効果は良く知られているが,UVB,UVA の細菌やウイルス,真菌等の増殖に対する抑制効果はあまり確認されていない.我々は大腸菌と黄色ブドウ球菌を用いて UVC,UVB,UVAの効果を比較した.大腸菌の結果からは,UVB でも UVC の 70%程度の効果があること,黄色ブドウ球菌の結果からは,UVB は UVC と遜色ない効果があること,菌のコロニー数減少率と距離2 の間に直線的負の相関があることが分かった.また,希釈した次亜塩素酸ナトリウムの一時的保存容器にも注目し,塩素濃度で示される次亜塩素酸ナトリウムの劣化を指標にして,紫外線カットフィルム,飲料水用 PET ボトル,ポリスチレン素材の遮蔽性と,三種類の紫外線の関係性について調べた.結果からは,紫外線カットフィルムが非常に効果的であり,次亜塩素酸ナトリウムの劣化には UVC と UVB は関係が無いが,UVA が非常に関与していることが分かった.The effect of Ultraviolet C (UVC) as a germicidal lamp is well known, but the inhibitory effects of UVB and UVA on the growth of bacteria, viruses, fungi, etc., have not been confirmed. We compared the effects of UVC, UVB, and UVA on Escherichia coli and Staphylococcus aureus. From the results of E. coli, UVB was about 70% as effective as UVC, and from the results of S. aureus, UVB was as effective as UVC. A certain linear negative relationship was also found between the rate of decrease in the number of bacterial colonies and the square of the distance. Furthermore, we investigated the shielding properties of the storage containers and the effects of three types of UV rays using the degradation of sodium hypochlorite as indicated by chlorine concentration. The results showed that the UV-cut film was very effective, and that UVC and UVB were weakly related to the deterioration of sodium hypochlorite. UVA had the greatest effect, followed by light sources of 450─500 nm.報告articl
Assessing Student Readiness for Digital Transformation Education: Survey for the Development and Utilization of Virtual Reality Teaching Materials
目的:A 大学看護学部学生におけるバーチャルリアリティ(VR)に対するレディネスを明らかにし,VR教材作成と使用に向けた示唆を得る.
方法:X 年度に A 大学看護学部に在籍中の学生に,入学年度,VR の体験の有無と内容,VR 使用時の身体への影響の有無について Microsoft Forms® を用いて調査を行った.群馬県立県民健康科学大学倫理審査委員会の承認を得た.
結果:研究対象者 131 名中 130 名が VR を知っており,3 割の学生は VR を調査以前に体験していた.体験内容はゲーム(16 件)やアミューズメントパークでのアトラクション(12 件),体験に使用したデバイスはスマートフォン(14 件)やタブレット端末(11 件)であった.VR 体験で目がまわった(10 件)等体調不良を感じた者もいた.
結論:本研究では VR 未体験の者が 7 割存在し,体調不良を感じた学生も確認された.VR を教育に取り入れていく際には,教材の使い方や注意事項などを詳細に周知し,体調不良の発生に備える必要性がある.Objectives: This study clarifies the virtual reality (VR) readiness level among nursing students at University A to obtainsuggestions for developing and using VR materials.
Methods: A survey was conducted on students enrolled in the Faculty of Nursing at University A in 2022. Participating nursing students were asked about the year they had entered university, the presence or absence of prior VR experience and the VR method employed, and the presence or absence of effects on the body during VR use. Microsoft Forms® was used for data collection. The study was conducted after approval had been obtained from the ethics review committee of Gunma Prefectural University of Health Sciences.
Results: Among the study participants, 130 out of 131 knew VR, and 30% of the students had experienced VR previously. This experience had largely consisted of games (n=16) and attractions at amusement parks (n=14), and the devices used had been smartphones (n=14) and tablet terminals (n=11). Some students reported feeling dizzy and other symptoms of unwellness from their VR experiences (n=10).
Conclusions: Seventy percent of study participants had never experienced VR, which caused physical distress in some. Therefore, we suggest that when incorporating VR into education, we need to prepare for feelings of sickness among some individuals and to disseminate detailed teaching materials and adopt precautions.報告articl
Support for Patients with Cancer who Receive bad news in the Outpatient Setting: Perceptions of Outpatient Nurses at Hospitals to which Oncology-Certified Nurse Specialists Belong and their Expectations for Oncology-Certified Nurse Specialists
目的:がん看護専門看護師(以下,OCNS)が所属する施設の外来看護師が認識している,外来で悪い知らせを伝えられるがん患者への支援と外来看護師が OCNS に期待することについて明らかにする.
方法:4 施設の外来看護師 14 名を対象にフォーカスグループ・インタビューを実施し,逐語録をもとにBerelson, B. を参考にして内容分析を行った.
結果:外来看護師が認識しているがん患者への支援は【OCNS との不十分な連携】【構築された同席システム】等の 5 カテゴリを,OCNS への期待は【OCNS の役割の発揮】等の 2 カテゴリを形成した.
結論:外来看護師と OCNS の連携の視点から,外来看護師が悪い知らせを伝えられるがん患者を支援できていないと認識しているカテゴリと支援できていると認識しているカテゴリに分けられた.OCNS は外来看護師の現状をアセスメントし,悪い知らせを伝えられるがん患者への支援が不足している部分を補えるように,活動環境を整備する必要性が示唆された.Objective: This study aimed to clarify the support for patients with cancer who receive bad news as perceived by outpatient nurses at facilities to which oncology-certified nurse specialists (OCNSs) belong and what the outpatient nurses expect from OCNSs.
Methods: Focus group interviews were conducted with 14 outpatient nurses from four hospitals. A content analysis was conducted based on the verbatim transcripts with reference to Berelson.
Results: The support for patients with cancer recognized by outpatient nurses was classified into five categories, including “insufficient coordination with OCNSs” and “established attendance system.” Expectations for OCNSs were classified into two categories, including “fulfilling the role of OCNSs.”
Conclusion: The support for patients with cancer who have received bad news as perceived by outpatient nurses can be divided into perceptions of being able and not being able to provide support. Outpatient nurses expected OCNSs to fulfill their role. OCNSs need to assess the current situation of outpatient nurses. In addition, an environment conducive to their activities to fulfill their expected roles needs to be created.報告articl

A Review of Japanese Literature on Learning through Shadowing Practice in Basic Nursing Education
Article目的:看護基礎教育におけるシャドーイング実習による学びについて整理し,本学の役割移行実習への示唆を得る.
方法:文献検索の結果,17 件の原著論文を分析対象とした.対象文献の研究目的や方法論などを検討し,シャドーイング実習による学びに関する結果のカテゴリーを抽出し,類似した内容ごとに再構成をしてテーマをつけた.
結果:シャドーイング実習の学びとして「患者の個別性をとらえた看護ケアの実際」「看護専門職としての姿勢や態度」などの 10 のテーマが明らかになり,看護過程の展開に必要な知識や技術の修得,看護職としての実務の理解,看護専門職者としてのあり様の理解の 3 つの効果が明らかになった.
結論:役割移行実習でシャドーイングの実習方法を用いることは適切であった.今後はロールモデルとなる看護師の選出,教員や指導者の意図的な関わりについて検討する必要性がある.Objective: The objectives of this study were to summarize learning through shadowing practice in basic nursing education and obtain suggestions for role transition practice at this university.
Methods: As a result of a literature search, 17 original articles were selected for analysis. The research objectives and methodologies of the articles were reviewed, and categories of results related to learning through shadowing practice were extracted and reorganized by similar content to form themes.
Results: Ten themes regarding learning from shadowing practice were identified, including “actual nursing care that captures the individuality of the patient” and “posture and attitude as a nursing professional”. In addition, three effects were identified: acquisition of knowledge and skills necessary for the development of the nursing process, understanding what is in-line with practical nursing work, and understanding what it means to be a nursing professional. The three effects of the study were: acquisition of the skills necessary to develop the nursing process, learning what is relevant to practice as a nurse, and understanding how to be a nursing professional.
Conclusion: The use of shadowing as a practice method in role transition training was appropriate. In the future, the selection of nurses to serve as role models and the intentional involvement of teachers and instructors need to be considered.総説群馬県立県民健康科学大学紀要18,1-12.(2023)departmental bulletin pape
