Literature review on the mental health of mothers who choose to have a painless delivery.

目的　本研究の目的は，無痛分娩を選択する女性のメンタルヘルスについて述べられている文献から，妊娠分娩産褥各期の母親のメンタルヘルスの特徴について明らかにし，助産師の支援の方向性について検討する． 方法　キーワード「無痛分娩，母親」，「無痛娩娩，心理」，「無痛分娩，メンタルヘルス」を検索，また，ハンドリサーチにて 2 件を追加した40件を抽出し，無痛分娩を選択する母親のメンタルヘルスについて記載のある，11件を分析対象とした． 結果　無痛分娩を選択した母親の妊娠期の研究内容は，無痛分娩を希望した理由・プロセスについての内容であった．分娩期の研究内容は，無痛分娩の価値や満足度，痛みに対するメンタルヘルスについての内容であった．産褥期の研究内容は，出産後の感情や育児へのメンタルヘルスについての内容であった． 考察　妊娠期の母親は自身の心身の変化や痛みに目が向くことは正常であるが，胎児にも関心を向けることで母子の相互作用が形成されると考える．また，無痛分娩による痛みの軽減と肯定的な分娩体験の振り返りが，分娩後の母親の安心した生活や自己効力感の向上に繋がると考える．さらに，助産師が妊娠期から産後にかけて切れ目のない支援を行うことで，母親が安心感を持ち，子どもと積極的に向き合える環境を整える必要性が示された．departmental bulletin pape

Literature review on data utilization by public health nurses

目的　本研究は，保健師のデータ活用について述べられている文献から，保健師のデータ活用の課題と工夫を明らかにする． 方法　医学中央雑誌 Web 版により，検索期間は2014年～2024年までに発表された原著論文を対象とし，検索キーワードは「保健師」and「データ」and「活用」とした．15件を分析対象とし，文献レビュー・マトリックスを作成し整理した． 結果　保健師のデータ活用上の課題は，データ活用のための時間の確保が困難な状況，関係者との連携であった．保健師のデータ活用上の工夫は，担当部署の連携を意識した体制づくりや共通の電子システム導入であった． 考察　保健師のデータ活用のためには，保健師と関係者がそれぞれの長所を補い合いながら，協働へと発展させるための連携が重要である．そしてデータを正しく活用するための理解とデータ活用スキルを身につける必要がある.departmental bulletin pape

A Qualitative Study on Clinical Nurses with a Difficult Experience ：Creation of a Difficulty-Overcoming Model

目的　臨床看護師の語りから困難克服過程や成長過程の様相を探究する． 方法　GTA に準拠して看護師17名に半構造化面接法を実施し，面接では実際に体験した困難とその克服過程を重視した． 結果　困難克服過程において【困難体験ステージ】と【困難克服ファクター】が構成された．困難体験は【困難体験ステージ】上の直面期・自省期・自覚期・向上期に位置づけられる．看護師は，【困難克服ファクター】である知識・実践の統合・専門職者の責任・他者からの支え・看護師成長欲求に基づき，自己成長していく．最終モデルは困難克服の様相を説明できる『困難克服モデル』と命名した． 考察　一般的に困難は，年齢や職業とは関係なく誰もが体験する日常的な現象であり，その体験者は成長促進と成長抑制の分岐点に立たされる．『困難克服モデル』は，看護師の日々に適用することが好ましく，困難を体験した際に成長促進へと向かわせる事が明らかとなった．Objective　The narratives of clinical nurses will be used to explore aspects of the process of overcoming difficulties and the process of growth. Methods　Semi-structured interviews were conducted with 17 nurses according to the GTA, and the interviews focused on the actual difficulties experienced and the process of overcoming them. Results　In the process of overcoming difficulties, the ［Difficulty Experience Stage］ and the ［Difficulty Overcome Factor］ were constructed. Difficult experiences are positioned in the ［Confrontation Stage］, ［Self-reflection Stage］, ［Awareness Stage］, and ［Improvement Stage］ on the ［Difficult Experience Stage］. Nurses develop themselves based on the ［Overcoming Difficulties Factor］: ［Integration of Knowledge and Practice］, ［Professional Responsibility］, ［Support from Others］, and ［Desire to Grow as a Nurse］. The final model was named the “Difficulties-Overcoming Model” which can explain the aspects of overcoming difficulties. Discussion　Generally speaking, “difficulties” are a daily phenomenon experienced by everyone, regardless of age or occupation, and those who experience them are at a crossroads between “growth promotion” and “growth inhibition.
