Equine-Assisted Interventions and Physical Health in Older Adults: A Meta-Analysis
International audienceObjective :While equine-assisted interventions (EAI) have proved their efficacy in a young population, they represent a new therapeutic field to promote the physical health of older adults. Through a meta-analysis, this study aimed to highlight the effects of EAI on the physical health of an older population. Methods : An exhaustive search of the PubMed, Embase, PsycINFO and Cochrane Library databases was carried out. Studies focusing on EAI and the physical domain in older adults over 60 years of age were included. Study quality was assessed using RoB2 and ROBINS-I. Measures were calculated with 95% CIs; heterogeneity was also explored. Results : Six articles involving 150 (women n = 107) healthy older adults were included. Sixty-four of them benefited from a program of multiple EAI sessions (20.67 ± 12.18). The meta-analysis indicates that performing an EAI program significantly enhances gait (SMD = 0.93, 95% CI [0.41, 1.44], p = .0004) and muscular activity (SMD = 0.90, 95% CI [0.11, 1.70], p = .03). Regarding balance, a slight improvement was observed (SMD = 0.44, 95% CI [−0.02, 0.90], p = .06). Conclusion : This meta-analysis reveals promising results regarding the benefits of EAI on the physical health of older adults. However, additional research is needed to strengthen these conclusions and promote these interventions among this population
Financements intermédiés et fragilité en Afrique
Nonobstant le grand intérêt sur la question du financement des Etats fragiles, très peu d'études se sont penchées jusqu'ici sur l'impact des ressources externes sur la fragilité dans les pays. Cette étude vient compléter la littérature et se penche sur l'impact spécifique des financements intermédiés sur la fragilité des pays africains. L'étude utilise la méthode de l'équilibrage entropique et la méthode de l'appariement par score de propension appliquée à un panel de 50 pays africains sur la période 2009-2019, pour estimer l'effet des financements intermédiés sur la fragilité. Les résultats sont significatifs et montrent que le financement intermédié réduit la fragilité des pays concernés. Le test de robustesse confirme cet impact positif et significatif indépendamment de la source de financement. Ces résultats devraient intéresser les décideurs politiques, les institutions financières internationales et les banques centrales, de même que les chercheurs dans la formulation des politiques et stratégies d'inclusion financière et de renforcement de la résilience dans les Etats fragiles.</div

Impact of excess weight on clinical features of psoriasis and efficacy of biologic therapies in children with severe psoriasis: Analysis of data from the BiPe cohorts
International audienceBACKGROUND: Being overweight or obese is associated with both the occurrence of childhood psoriasis and increased severity thereof. We evaluated the impact of being overweight or obese on the clinical features of severe psoriasis in children treated with etanercept, adalimumab, or ustekinumab, and on the therapeutic effectiveness of these treatments using data from the BiPe cohorts. PATIENTS AND METHODS: Children from the BiPe cohorts with available baseline body mass index data were included. Data from two groups, "normal weight" and "overweight-to-obese" (OTO), and from two OTO subgroups, overweight and obese, were analyzed. RESULTS: Of the 134 children included, 98 (73.1%) were of normal weight, and 36 (26.9%) were OTO, including 21 (15.7%) overweight and 15 (11.2%) obese children. There were no between-group differences in age at psoriasis onset, family history, clinical features, or severity. Compared to the normal weight group, psoriatic arthritis was more frequent in the OTO group (5.1% versus 16.7% respectively; P = 0.03) and in the overweight and obese subgroups (5.1% versus 14.3% and 20.0%, respectively; P = 0.08). Analysis of PASI scores after 3 months of treatment suggested that biologic therapy efficacy was higher for the OTO group than the normal weight group. Drug continuation rates tended to be lower for the OTO group, particularly for adalimumab (P = 0.0002). DISCUSSION: OTO status was associated with a higher frequency of psoriatic arthritis in children with psoriasis. Except for lower rates of adalimumab continuation, overall biologic therapy efficacy was similar in the normal and OTO groups. Studies involving larger cohorts are needed to confirm these findings
A Trans and Queer Discursive Approach to Gender Diversity and Misgendering in the Transgender and Gender Diverse Population: Queering a Study for ICD-11
International audienceProducing knowledge about transgender and gender-diverse (TGD) individuals is a core public health strategy challenge. Yet several systemic limitations arise, notably the exclusion or exploitation of TGD individuals by research systems reproducing systemic discrimination by embedding social norms as self-evident facts of nature. This is particularly worrying in biomedical research, and contributes to the invisibilization of participants’ gender diversity. This trans research illustrates methodological challenges through queering an earlier study by focusing on misgendering as a discursive element. We based our work on discursive materials reported by TGD participants in an ICD-11 study on gender incongruence. We used network analyses to illustrate potential differences between declared gender identity and discourse practices. Our results highlight a gap between declared gender identity and discourse practices, bringing the number of non-binary participants in the sample from 15 (20.8%) to 36 (50.0%). Moreover, misgendering and the use of derogatory terms are more common toward gender-diverse individuals. Sexual orientation shows a similar trend. This study reveals the reproduction of social norms within research processes and medical knowledge, as well as how, from an individual perspective, their non-compliance seems to be a key factor in TGD individuals’ experience. By providing this simple methodological example, we hope to promote better integration of gender and its various dimensions into biomedical and public health research

Inflammatory status of periprostatic adipose tissue from men with prostate cancer: Ethno-geographic variations and association with lipid composition
International audienceBackground: Since prostate cancer (PCa) risk is associated with ethnicity, it is crucial to investigate ethno-geographic variations in PCa studies. Periprostatic adipose tissue (PPAT) has been involved in cancer aggressiveness through the release of lipids and inflammatory mediators, and we previously reported a different lipid composition of PPAT according to the ethno-geographic origin. We aimed to analyze the expression of a panel of adipokines in PPAT from African-Caribbean and Caucasian patients, in correlation with features of PCa aggressiveness and lipid composition.Methods: Adipokines expression was analyzed by RTqPCR in PPAT from 110 Caucasians and 50 African-Caribbeans patients, in parallel with the characterization of fatty acids and cholesterol content.Results: The most expressed cytokines were IL10, leptin and IL8. The expression of most adipokines was higher in PPAT from Caucasians compared to African-Caribbean patients. IL6 was associated with features of PCa aggressiveness in Caucasians, with a difference close to significance. Most cytokines were associated with the lipid composition of PPAT, mainly with arachidonic acid and free cholesterol content.Conclusions: The differential cytokines expression in PPAT from African-Caribbean patients compared to Caucasians probably reflects a different inflammatory status. The close relationship between adipokines expression and lipid composition highlights the importance of diet and lipid metabolism in adipose tissue inflammation
