    Evaluating Lichen Sclerosus in Phimosis: Insights From a Multidisciplinary Retrospective Study

    Objectives: This study aimed to evaluate the prevalence of lichen sclerosus (LS) in male patients undergoing circumcision for phimosis, emphasising the significance of a multidisciplinary approach in the early diagnosis and management of this condition. Methods: A retrospective analysis was performed on 841 male patients who underwent circumcision at a high-volume medical centre between 2001 and 2023. Data were collected on clinical diagnoses made by both dermatologists and urologists, along with the corresponding histological findings. Logistic regression models were utilised to assess diagnostic accuracy and the relationship between clinical suspicion and histological confirmation. Results: Histological confirmation of LS was identified in 30.6% of the cases. Dermatologists demonstrated a diagnostic accuracy of 81.8%, whereas urologists showed an accuracy of 46.1%. The evaluations conducted by dermatologists exhibited a significant correlation with histological confirmation of LS (odds ratio 4.81; 95% confidence interval: 1.96–11.80; p = 0.001). Conclusions: This study elucidates the differences in diagnostic accuracy between dermatologists and urologists in identifying LS in patients undergoing circumcision for phimosis, highlighting the necessity of a multidisciplinary approach to improve the diagnostic process. Furthermore, the epidemiological data obtained from this research offer valuable insights into the prevalence of LS and the associated diagnostic challenges within a substantial patient cohort

In its concluding section, the article evaluates potential remedies, including proposals for constitutional reforms as well as measures aimed at facilitating the appointment process within the framework of the current constitutional system

    Training Load, Official Match Locomotor Demand, and Their Association in Top-Class Soccer Players During a Full Competitive Season

Twenty-five top-class soccer players competing in UEFA international competitions were included. The season was divided into 2 different categories: 2 matches (M2) or 3 matches (M3) in 8 days. Starters and nonstarters were classified. Total distance (TD), high-speed running (HSR, 15-20 km·h-1), very high-speed running (VHSR, 20.1-24 km·h-1), sprint (SPR, >24.1 km·h-1), and accelerations/decelerations (Acc + Dec, >3 m·s2) were recorded. Trivial to moderate differences (p 0.05) in M2 and M3 with ∼5 to ∼29% match-to-match variability depending on metrics. Total load (i.e., training plus match loads) was higher (p < 0.05, ES: 0.75/1.61) in starters than nonstarters, because of a higher match load and no difference in the training load. Very high-speed running and SPR accumulated during training sessions were largely to very largely (r = 0.60 to 0.72) associated with TD, HSR, VHSR, and Acc + Dec covered during official match; VHSR and TD during training were largely to very largely (r = 0.57 and 0.71) associated with SPR and Acc + Dec during official match. In conclusion, (a) congested periods seemed to not affect official match locomotor performance; (b) practitioners may consider high week-by-week workload variability for individualizing training prescriptions, especially for nonstarters; and (c) the VHSR and SPR accumulated during training were associated with the official match locomotor demands, and it may be considered for maximizing performance

    Analysis of Flavescence dorèe leaf symptoms using hyperspectral imaging and machine learning

    This work presents a novel approach for the detection and classification of Flavescence dorèe disease based on hyperspectral imaging. Three machine learning model are trained to classify leaf samples of Pinot Noir into healthy, asymptomatic, and diseased classes according to a combination of vegetation indexes calculated on the per-pixel spectrum of the sample. The dataset used includes 201 hypercubes collected from the same field in 2023 and 2024, equally divided into the three classes. Results highlight good performances (92% accuracy) for the model trained on the population of features extracted from the vegetation indexes for each leaf and normalized using z-score

    Analysis of clinical and oncological outcomes after simple and intentional lung segmentectomies: the deceptive simplicity of the S6 segment

    Background: Currently, few studies analyze postoperative outcomes for different types of segmental resections. Therefore, our aim was to compare lower lobe apex with other simple and intentional segmentectomies, in terms of clinical and oncological outcomes. Methods: A retrospective and multicenter study was conducted on patients who underwent simple and intentional segmentectomies from 2015 to 2020. Inclusion criteria were stage cT1a/b N0 M0 disease, peripheral localization and predicted postoperative forced expiratory volume in one second (ppoFEV1) >60%. Pre, intra and post operative parameters were collected and we performed statistical analysis on overall survival and recurrence. Results: The inclusion criteria were met by 252 patients, with 125 having an anatomical S6 resection ("S6" group) and 127 having another anatomical simple segmentectomy ("others" group). There were no significant differences in baseline data. We noted a worse 5-year disease-free survival (DFS) for the "S6" group (P=0.053). Twenty-eight of 46 patients with relapsed disease had loco-regional recurrence. Of these, 80% belonged to the "S6" group (P<0.001). Conclusions: Oncological outcomes were not the same for all simple and intentional segmentectomies. Particularly, S6 exhibited worse oncological outcomes, mainly in terms of disease-free survival. This disparity could be explained by the unpredictable lymphatic drainage or by the smaller parenchymal volume removed during dissection
