    An economic analysis of time-based Demand Response programs and facts devices implementation for congestion management

    This paper explores the utilization of combination of Alternating Current Transmission System (FACTS) devices, time-based Demand Response (DR) programs, and Generation Redispatch (GR) in short-term congestion management as well as minimization of generation costs in power systems. To achieve this, a multi-stage market clearing procedure is formulated. At the first stage, the market is cleared based on generation cost minimization, without considering network constraints. Market clearing formulation for the second stage is developed considering congestion, in which FACTS device (Thyristor Controlled Series Capacitor (TCSC)), and time-based DR programs (Time of Use (TOU)) are optimally coordinated with GR in the presence of network constraints, to manage congestion at minimum costs. In addition, to make conditions more realistic, operational conditions spanning for a day (24 hours) are considered in this study. Then capabilities of these approaches in different scenarios for congestion management as well as minimization of generation costs are examined on IEEE 14-bus system. Results show that although, applications of time-based DR programs are more effective as compared to other approaches in terms of generation cost reduction but they have limited capability for congestion management due to consumption of responsive loads which usually decreases in one period and increases in another

    A Case Report of Metastatic Castration-Resistant Prostate Cancer Harboring a \u3ci\u3ePTEN\u3c/i\u3e Loss

    The treatment landscape of metastatic castration-resistant prostate cancer (mCRPC) has dramatically improved over the last decade; however, patients with visceral metastases are still faced with poor outcomes. Phosphatase and tensin homolog (PTEN) loss is observed in 40%–60% of mCRPC patients and is also associated with a poor prognosis. Several PI3K/AKT/mTOR pathway inhibitors have been studied, with disappointing anti-tumor activity. Here, we present a case of a patient with heavily treated mCRPC who had a modest tumor response to concurrent carboplatin, abiraterone acetate/prednisone, and liver-directed radiation therapy. We discuss the potential rationale supporting the use of this combination therapy and its safety in mCRPC. While the underlying basic mechanism of our patient’s anti-tumor response remains uncertain, we suggest that further prospective studies are warranted to evaluate whether this combination therapy is effective in this population of patients with pre-treated mCRPC and PTEN loss

    Androgen Receptor Signaling in Prostate Cancer and Therapeutic Strategies

    Understanding of the molecular mechanisms of prostate cancer has led to development of therapeutic strategies targeting androgen receptor (AR). These androgen-receptor signaling inhibitors (ARSI) include androgen synthesis inhibitor-abiraterone and androgen receptor antagonists-enzalutamide, apalutamide, and darolutamide. Although these medications provide significant improvement in survival among men with prostate cancer, drug resistance develops in nearly all patients with time. This could be through androgen-dependent or androgen-independent mechanisms. Even weaker signals and non-canonical steroid ligands can activate AR in the presence of truncated AR-splice variants, AR overexpression, or activating mutations in AR. AR splice variant, AR-V7 is the most studied among these and is not targeted by available ARSIs. Non-androgen receptor dependent resistance mechanisms are mediated by activation of an alternative signaling pathway when AR is inhibited. DNA repair pathway, PI3K/AKT/mTOR pathway, BRAF-MAPK and Wnt signaling pathway and activation by glucocorticoid receptors can restore downstream signaling in prostate cancer by alternative proteins. Multiple clinical trials are underway exploring therapeutic strategies to overcome these resistance mechanisms

    Spontaneous Four-Wave Mixing of de Broglie Waves: Beyond Optics

    We investigate the atom-optical analog of degenerate four-wave mixing of photons by colliding two Bose-Einstein condensates (BECs) of metastable helium and measuring the resulting momentum distribution of the scattered atoms with a time and space resolved detector. For the case of photons, phase matching conditions completely define the final state of the system, and in the case of two colliding BECs, simple analogy implies a spherical momentum distribution of scattered atoms. We find, however, that the final momenta of the scattered atoms instead lie on an ellipsoid whose radii are smaller than the initial collision momentum. Numerical and analytical calculations agree well with the measurements, and reveal the interplay between many-body effects, mean-field interaction, and the anisotropy of the source condensate

    Decompression illness secondary to occupational diving : recommended management based current legistation and practice in Malaysia.

    Occupational divers are exposed to hazards which contribute to the risk of developing decompression illnesses (DCI). DCI consists of Type I decompression sickness (DCS), Type II DCS and arterial gas embolism (AGE), developed from formation of bubbles in the tissues or circulation as a result of inadequate elimination of inert gas (nitrogen) after a dive. In Malaysia, DCI is one of the significant contributions to mortality and permanent residual morbidity in diving accidents. This is a case of a diver who suffered from Type II DCS with neurological complications due to an occupational diving activity. This article mentions the clinical management of the case and makes several recommendations based on current legislations and practise implemented in Malaysia in order to educate medical and health practitioners on the current management of DCI from the occupational perspective. By following these recommendations, hopefully diving accidents mainly DCI and its sequalae among occupational divers can be minimized and prevented, while divers who become injured receive the proper compensation for their disabilities
