Why use ultrasound in rheumatology?A valuable tool for diagnosis and researchOver the past decade, musculoskeletal US (MSUS) hasplayed an increasingly important role in optimizing clinicalassessment and monitoring of patients with rheumato-logical diseases [1, 2]. This technique has allowed us tominutely evaluate periarticular and intra-articular struc-tures involved in musculoskeletal diseases [2, 3]. Wecould also say that MSUS is now at the cutting edge ofresearch in rheumatology [4, 5]. High-resolution MSUSwith Doppler technique provides an accurate and sensi-tive assessment of joint and enthesis inflammatory activityand structural damage in inflammatory arthritis and OA[47]. MSUS allows a detailed diagnosis of soft tissue syn-dromes [8, 9]. It is now also increasingly being used in thediagnosis of paediatric musculoskeletal diseases and thisapplication in itself is an evolving field [10]. However, thecapabilities and advantages of MSUS in adults can beeven greater in children with inflammatory and infectiousdiseases [11].MSUS is a valuable bedside tool for guiding accurateand safe musculoskeletal diagnostic fluid aspiration andperi- or intra-lesional therapeutic injections [13]. It pro-vides confirmation of the clinical diagnosis and the indi-cation for injection. Real-time US enables us to correctlyplace the needle, accurately deliver medication and visu-alize the steroid suspension during and after the proced-ure. These advantages are very useful in the managementof rheumatic disease. Evidence of the role and validity ofUS in evaluating the above abnormalities is increasing[12]. New frontiers of MSUS that are worthy of attentioninclude US assessment of the skin and vessels in rheum-atological diseases.In conclusion, MSUS is a relevant part of current andfuture rheumatology practice and research because itsubstantially improves our diagnostic and therapeuticcapabilities.
