Trabekülektomi ve mitomisin-C uygulanan yüksek riskli glokomu olan 35 olgu, antifibroblastik ajanlarından mitomisin-C nin trabekülektomi operasyonuna etkilerini değerlendirmek amacıyla retrospektif ve prospektif olarak incelendi. 35 hastanın 42 gözü çalışma kapsamına alındı. Sekonder glokorrilu 21, juvenil glokomlu 4, neovasküler glokomlu 5, dar açılı glokomlu 5, konjenital glokomlu 7 göze trabekülektomi+mitomisin-C uygulandı. Olgulardan 18'i daha önceden 1 veya daha fazla trabekülektomi operasyonu geçirmişti. Ortalama takip süresi 21.85±19.81ay idi(2-42 ay). Operasyondan sonra 32 gözde göziçi basıncı tedavisiz 21 mmHg veya daha az olarak bulundu.Olgulardan 5'i.nde uveal efüzyon, 4'ünde hifema, 3'ünde hipotoni, 1'inde hipotoni makülopati, 1'inde geç dönemde endoftalmi görüldü. 6 gözde tedaviye rağmen göziçi basıncı yüksek bulundu. Sonuç olarak prognozu kötü olan glokomlu olgularda filtrasyon cerrahisinde mitomisin-C uygulamalarının etkin bir yöntem olduğu kanısına varıldı.We retrospectively and prospectively examined 35 cases with high risk glaucoma who underwent trabeculectomy and intraoperative MMC application in order to evaluate the effect of antifibroblastic agents of trabeculectomy procedure. 42 eyes of 35 patients were included in this study. We performed trabeculectomy with MMC to 21 eyes with secondary glaucoma, 4 eyes with juvenile glaucoma, 5 eyes with neovascular glaucoma, 5 eyes with closed angle glaucoma, and 7 eyes with congenital glaucoma. 18 of the cases had had 1 or more failed trabeculectomy operation before hand. Mean follow up period was 21.85±19.81 months(range 2 and 42 months). Following the operation the intraocular pressure was less than 21 mmHg in 32 eyes without medication. We observed uveal effusion in 5, hyphema in 4, hypotony in 3, hypotony maculopathy in 1 and late endophthalmitis in 1 of the cases. In 6 of the cases intraocular pressure was found to be high in the postoperative period despite the medication. We can suggest that the application of MMC is an effective procedure in cases with high risk glaucoma
