This examination showed, in four and five apicalchamber views, a poorly visualizable right atrium (RA),plausibly compressed by an echorefractive echographicalstructure such as the liver (Fig. 1). Subcostal andparasternal projections, short axis and long axis, poorlyvisualized the compressed area. To characterize theunidentified mass, total body computed tomographywas suggested; it recognized a bilateral polycystic kidney[1] and, because of the presence of an enormous cyst atthe apical area of the right kidney, a thoracic movementof the liver was determined, with consequent com-pression by the liver on the right lung and heart, andRA in particular. The cyst had a homogeneous structure,its longitudinal diameter was 7.5 cm and the transversewas 5 cm. Cyst resection was advised.Reference1 Fain O. Renal polycystosis.
