The Functional Assessment of Cancer Therapy-Head & Neck Scale (FACT H&N, 4th version).

Abstract

The Functional Assessment of Cancer Therapy Core Instrument (FACT-G) added head and neck related symptoms sub-scales to create the FACT H&N questionnaire.The FACT H&N (version 4) is a subtype of FACT questionnaires family. The FACT H&N instrument specifically designed to measure health related quality of life among head and neck cancer patients. It consists of 27 core items - FACT-G - assessing patient function in four domains: Physical well-being (PWB); social and family well-being (SFWB); emotional well-being (EWB); and functional well-being (FWB). Eleven items were added to comprise the Additional Concern subscale that measures head and neck related symptoms. Each item was rated on 5 point Lickert Scale (0= Not at all; 4= Very much). Different subscale scores and indices could be calculated, however, in general the higher scores represent better quality of life
