153  REZISTENŢA LA ANTIBIOTICE - ACTUALITĂŢI ÎN DIAGNOSTICUL DE LABORATOR  Oxana Iasîbaş (Cond. şt. - Olga Burduniuc, dr. șt. med., cercetător ştiinţific superior, Centrul Naţional de Sănătate Publică)  Introducere. În contextul actual de creştere rapidă a prevalenţei rezistenţei tulpinilor de Enterobacteriaceae, necesitatea utilizării metodelor de biologie moleculară devine din ce în ce mai stringentă, acestea fiind metode mult mai sensibile decât testarea fenotipică.   Scop. Detectarea unor marcheri de rezistenţă(betalactamaze) către preparatele antibacteriene a E. coli, responsabilă de producerea infectiilor tractului urinar (ITU). Material şi metode. Au fost cercetate 118 tulpini de E. coli izolate din uroculturile semnificativ pozitive, recoltate de la pacienţii cu ITU. Tulpinile au fost identificate pe baza proprietăţilor morfotinctoriale, culturale şi biochimice, utilizând teste API 20E (Biomerieux).  Rezultate. La tulpinile de E. coli studiate a fost semnalat fenomenul de rezistenţă la preparatele antibacteriene. Ponderea tulpinilor rezistente la fluorochinolone a constituit -89%, la betalactamine - 77%, aminoglicozide - 72% şi sulfamide - 72%.Enzimele betalactamaze cu spectru extins (BLSE) identificate la tulpinile de E. coli au fost în exclusivitate de tip CTX-M.  S-a constatat că filogenetic 58,5% tulpini de E. coli BLSE aparţin grupului B2, grupului A - 27,9%, grupului D – 12,7% şi altor grupe - 0,9%. Concluzii. Circulaţia tulpinilor E. coli tip CTX-M pe teritoriul Republicii Moldova determină un potenţial înalt de diseminare a acestora în colectivităţi şi creează dificultăţi în conduita terapeutică a pacienţilor cu ITU. Determinarea marcherilor moleculari de rezistenţă la antibiotice constituie o metodă rapidă şi eficientă de monitorizare a mecanismelor responsabile de fenomenele de rezistenţă la antibiotice a tulpinilor de E.coli, şi conduce la un management terapeutic mai eficient. In the present context of rapid increase in the prevalnce of resistant Enterobacteriaceae strains, the need to use molecular biology methods becomes increasingly stringent, these being much more sensitive than phenotypic testing.  Purpose. To detect the resistance markers (betalactamases) to antibacterial preparations of E. coli, responsible for production of urinary tract infections (UTI).  Material and methods. One hundred and eighteen E. coli strains isolated from importantly-positive urine cultures, harvested from patients with UTI have been investigated. Strains were identified on the basis of their  cultural and biochemical tests using API 20E (bioMérieux).   Results. In the studied E. coli strains the phenomenon of resistance to antibacterial preparations was recorded. The share of strains resistant to fluoroquinolones constituted 89%, to betalactamines - 77%, aminoglycosides - 72% and sulfonamides - 72%. Extended-Spectrum Beta-Lactamases enzimes identified in E. coli strains were of CTX-M type. It was found that 58.5% phylogenetic E. coli strains belong to group B2 ESBLs, 27,9% to group A, 12,7% to group D and to other groups constituted 0.9%. Conclusions. Circulation of E. coli strains of CTX-M type in Moldova determines their high potential for dissemination to the community and makes it difficult to conduct the management of therapy in patients with UTI. Determination of molecular markers of resistance to antibiotics is a fast and efficient monitoring method of the mechanisms responsible for the phenomena of resistance to antibiotics of strains of E. coli, and lead to a better therapeutic management.
