Acute radial head replacement with bipolar prostheses: midterm results
In irreparable radial head fractures, especially if primary stabilizers of the elbow are damaged, the prosthetic replacement prevents instability and stiffness. Concerns have arisen over the use of bipolar press-fit prostheses due to the frequent finding of osteolysis and the risk of instability if compared to monopolar implants. Our aim was to assess midterm clinical and radiological outcomes of bipolar implants and the influence of osteolysis on proximal pain. Seventeen patients with irreparable fractures of the radial head, treated in acute with the same prosthetic model (rHEAD recon SBI/Stryker) between January 2015 and December 2018, were enrolled. Clinical assessment was performed using MEPS and DASH scores; a radiographic study was done to identify heterotopic ossifications and periprosthetic osteolysis. Outcomes at the last follow-up, according to MEPS, were excellent in 10 cases, good in 5 and fair in 2; none of the patients had severe pain or instability. In 3 cases, it was necessary to remove the implant, mainly because of early loosening. Radiological findings of osteolysis were detected in 9 cases, but no statistical correlation was found with MEPS and proximal pain. The use of bipolar implants is reliable if an accurate repair of ligament tears is performed and provides a good stability. Nevertheless, the risk of early aseptic loosening in uncemented implants is not negligible, and the follow-up of the patient must be strict. Late osteolysis does not seem to have clinical relevance, but further prospective studies are necessary to clarify this topic

Terminology is a crucial problem in science, since quite often the terms used to denote theoretical concepts are directly taken from the vocabulary of common words, which usually have multiple meanings. To solve this problem, scientists must operationalize the theoretical concepts they use but, in psychology, sometimes, this can be a difficult task, since the meaning of the terms used to give the instructions to observers can interfere with the spontaneous meaning that observers give to those terms. Starting from some empirical observations, we show that colour appearance is affected not only by the perceptual but also by the linguistic context
Nodular histiocytic/mesothelial hyperplasia as consequence of chronic mesothelium irritation by sub-phrenic abscess.
Nodular histiocytic/mesothelial hyperplasia (NHMH) is a benign localized alteration, first
described in 1975 by Rosai in the hernia sac [1]. Few pulmonary cases have been reported in literature
[2â6]. Sometimes it has been reported in the pericardium [7,8] or presenting as an inguinal
mass [9]. The âmesothelial/monocytic incidental cardiac excrescenceâ, first described by Weinot et al.
in 1994 [10] is now considered a similar lesion to NHMH [11].
It consists of a reactive proliferation of histiocytes and mesothelium secondary to chronic irritation
and it has been observed in pleura-damaging processes, such as pneumothorax [5], or as consequence
of cardiac catheterization, inflammation, mechanical or tumor stimulation [11].
The rarity of NHMH and the moderate cytological atypia often present, make this lesion difficult
to diagnose. It can be easily confused with primary mesothelial lesions and neoplasms such
as adenocarcinomas, granulosa cell tumors or Langerhansâ histiocytosis.
We report a case of pleural NHMH in a patient with a subphrenic abscess, in which no pulmonary
pathogenic noxa was evident. We hypothesize a transdiaphragmatic chronic irritation as a
pathogenetic mechanism underlying NHMH
