    Implantation of a leadless pacemaker as a promising solution for patients with arythmia ÔÇô current state of knowledge

    Introduction: In cardiac electrotherapy, new solutions are constantly sought to minimise the invasiveness of methods used so far and reduce the risk of complications. Implantation of traditional cardiac pacemakers carries risks of high probability that may cause both early and late adverse effects which are associated with presence of subcutaneous lodge and intravenous wires. A significant reduction in complications may be achieved by using leadless pacemakers the main representative of which is MicraTM (MicraTM transcatheter pacing system: TPS, Medtronic plc, Mounds View, MN, USA). Micra is 90% smaller than conventional pacemaker. The generator and electrode system is contained in one capsule-like device that is implanted directly into the right ventricle of the heart. The transcatheter pacing system is less invasive because it is placed in the heart with the use of a catheter via a femoral vein. thus no chest incision and scar. Purpose of the work: The aim is to present the subject of leadless cardiac pacing with the use of Micra device based on a review of current scientific publications. Methods and materials: The available medical literature related to cardiac pacing was analysed taking leadless technology into account. The analysed material was obtained from PubMed and Google Scholar. Conclusion: Cardiac electrotherapy with the use of leadless Micra pacemaker is a great example of substantial technological progress. Due to its structure and form, it is a beneficial alternative to a traditional pacemaker. It also offers promising solutions for the future. This is, however, a relatively new method of stimulation requiring further observation and analysis

    Introduction and purpose of the study: The problem of pain occurs in many patients, and its perception is a subjective aspect. This is a big problem because it negatively affects not only the patient's health but also many aspects of his life: physical activity, social relations, mental and emotional state. Physicians of many specialties face the problem of treatment both in doctor's offices and in hospital rooms. The choice of the appropriate form of therapy can be facilitated by scales and pain assessment questionnaires. This paper presents ways to determine the severity of pain that can be used in contact with both children and the elderly. Proper determination of the degree of pain intensity enables the selection of the appropriate threshold of the analgesic ladder. Materials and methods: The review was based on the analysis of materials collected in the "PubMed" database and WHO guidelines. The work uses the latest scientific works, review articles. The search was carried out using the keywords: "pain management", "analgesic ladder", "pain management scales", "pain management questionnaires", "neuropathic pain", "opioids". Summary: Treatment is a big challenge because each patient feels pain differently, which means that each patient must be approached individually. In order to facilitate and systematize the treatment to some extent and, above all, to choose the best therapeutic method for the patient, pain scales and questionnaires should be used to assess pain intensity and observe the results of the treatment. The variety of methods of imaging the pain intensity scale allows us to choose the most appropriate one for a given patient, depending on his cognitive abilities and age. The choice of treatment method should be based on the analgesic ladder. It is necessary to take into account both the patient's condition, his diseases, treatment and side effects of the introduced treatment

One of the most modern electrotherapy treatments that uses high frequency currents is Indiba therapy. It is a procedure based on the use of an electric flow of radiofrequency current with a frequency of 448 kHz. Radio frequency currents have become one of the basic methods of treating pain and musculoskeletal injuries. Clinical studies show that therapy with high-frequency currents reduces pain, accelerates blood flow, oxygenates the body and accelerates cell metabolism.     In this doctoral dissertation I compared the effectiveness of my method of teaching the voice emission ÔÇô which I refer to as holistic ÔÇô with the effectiveness of 3 other selected methods of teaching the acting students. Cognitive basis for this work is: the general didactics, multidimensionality of teaching by W. Oko┼ä, holistic pedagogy by A. Szyszko-Bohusz, and positive disintegration concept by K. D─ůbrowski. I presented a review of various methods of working with the voice. I described at length my own method based on multidimensional and multisensorial pedagogical impact, which also embraces the studentÔÇÖs individual learning style / preferences. Main hypothesis was: in shaping 5 voice qualities ÔÇô color, intonation, capacity, range, and rhythm ÔÇô my holistic method is more effective than other investigated methods. To test my hypothesis, I used pedagogical experiment which lasted 2 years. I invited to it the Theatrical Academy students who were taught with the use of 4 methods: holistic, psychological, based on physical exercises, and based on vocal exercises. I investigated changes of the examined voice qualities and compared the outcomes of holistic teaching (experimental group) with the results of applying 3 other selected methods (encompassed altogether in control group). The results showed that the holistic method is more effective for 3 voice qualities ÔÇô intonation, capacity and range. Such qualities as color and rhythm are not affected or are affected marginally regardless the applied method. Also, it is worth mentioning that the most significant progress occurred after the 1st year of pedagogical work

    Thermal imaging becoming more popular in medicine and also in sports. The technique may be helpful as an additional method in standard training protocol because of its non-invasive and safe properties. The aim of study was to perform the thermal analysis of endurance sportsman during training cycle and sports clothes temperature parameters evaluation. An attempt was made to evaluate the metabolic rate and thermoregulation mechanisms of athleteÔÇÖs organisms based on obtained thermal parameters. The data was used also for temperature changes analyse in case of body efficiency rating and for the thermal comfort evaluation in the sports thermo-active clothing. Some dependences between training level and the body temperature stability were showed, which seems to confirm the thesis of adaptation processes in athleteÔÇÖs body. Moreover, the obtained correlations between standard bodyÔÇÖs efficiency and thermal parameters show high convergence and statistical significance, which may indicate the possibility of using thermal imaging as a training plan support method. The thermoactive clothes thermal analysis seems to be helpful for selection and evaluation the training wear, which will be adapted to training conditions and support the body thermoregulation natural mechanisms and overheating protection

    Cardiovascular diseases are the main cause of death among patients with chronic kidney disease. Classic and disease-specific cardiovascular risk factors are observed in this group of patients. Recently, arterial stiffness has been paid more attention to as a cardiovascular risk factor. High-resolution echo-tracking is a direct and non-invasive ultrasound method of local arterial stiffness evaluation. New publications show that indices of local arterial stiffness, such as stiffness parameter beta, are predictors of cardiovascular mortality. Evaluation of local arterial stiffness can take a prominent place in cardiovascular risk assessment due to limitations of standard diagnostic methods present in chronic kidney disease.
