    [Excerpt] Supported employment has been a service option for a long time and is responsible for many individuals working who were previously considered unemployable. It is characterized by competitive employment in community businesses with training and support provided by a skilled job coach for as long as the individual is employed. Supported employment is for those individuals who need help finding a job, intensive support to learn the job, and on-going follow-along support in order to keep their job. It is based on the premise that individuals do no have to “get ready” for work but rather receive the necessary supports that bridge the gap between their skills and the job requirements once they are employed. Job coach assistance is provided more intensely at first followed by intermittent on-going support once that individual is able to complete the job. Examples of some of the supports that may help someone perform their job are: advocacy, natural supports, assistive technology, job modifications, job carving, rehabilitation engineering, compensatory strategies, and behavioral training techniques

    Background Gestational diabetes mellitus is associated with increased incidence of adverse perinatal outcomes including newborns large for gestational age, macrosomia, preeclampsia, polyhydramnios, stillbirth, and neonatal morbidity. Thus, fetal growth should be monitored by ultrasound to assess for fetal overnutrition, and thereby, its clinical consequence, macrosomia. However, it is not clear which reference curve to use to define the limits of normality. Our aim is to determine which method, INTERGROWTH21st or customized curves, better identifies the nutritional status of newborns of diabetic mothers. Methods This retrospective cohort study compared the risk of malnutrition in SGA newborns and the risk of overnutrition in LGA newborns using INTERGROWTH21st and customized birth weight references in gestational diabetes. The nutritional status of newborns was assessed using the ponderal index. Additionally, to determine the ability of both methods in the identification of neonatal malnutrition and overnutrition, we calculate sensitivity, specificity, positive predictive value, negative predictive value and likelihood ratios. Results Two hundred thirty-one pregnant women with GDM were included in the study. The rate of SGA indentified by INTERGROWTH21st was 4.7% vs 10.7% identified by the customized curves. The rate of LGA identified by INTERGROWTH21st was 25.6% vs 13.2% identified by the customized method. Newborns identified as SGA by the customized method showed a higher risk of malnutrition than those identified as SGA by INTERGROWTH21st. (RR 4.24 vs 2.5). LGA newborns according to the customized method also showed a higher risk of overnutrition than those classified as LGA according to INTERGROWTH21st. (RR 5.26 vs 3.57). In addition, the positive predictive value of the customized method was superior to that of INTERGROWTH21st in the identification of malnutrition (32% vs 27.27%), severe malnutrition (22.73% vs 20%), overnutrition (51.61% vs 32.20%) and severe overnutrition (28.57% vs 14.89%). Conclusions In pregnant women with DMG, the ability of customized fetal growth curves to identify newborns with alterations in nutritional status appears to exceed that of INTERGROWTH21s

    The problem of estimating a signal that is corrupted by additive noise has been of interest to many researchers for practical as well as theoretical reasons. Many of the traditional denoising methods have been using linear methods such as the Wiener filtering. Recently, nonlinear methods, especially those based on wavelets have become increasingly popular, due to a number of advantages over the linear methods. It has been shown that wavelet-thresholding has near-optimal properties in the minimax sense, and guarantees better rate of convergence, despite its simplicity. Even though much work has been done in the field of wavelet-thresholding, most of it was focused on statistical modeling of the wavelet coefficients and the optimal choice of the thresholds. In this paper, we propose a custom thresholding function which can improve the denoised results significantly. Simulation results are given to demonstrate the advantage of the new thresholding function

    In response to a longterm research program for a didactical ontology, this report intends to present the results and methods for representing didactical models from the ontology we developed. The question is: How can computer technology be used to support the communication of knowledge in an educational context? This question cannot be answered by psychological experiments that ignore the core of educational behaviour: the transmission of meaning (Hönigswald 1927). Therefore this article focuses on the didactical tradition. Computer technology as a medium requires a special form of knowledge organisation, which allows learners to go individually and in a reflective way through the content (Customized Learning Sequences), thus requiring teachers to produce individually navigable hypertexts. Individualization does not mean offering "pureâ€? self-directed learning, as learning presupposes instruction by others. We have to aid teachers in reorganizing knowledge to hypertexts that allows individual navigation. Supporting learners in finding their individual path is also a crucial factor.How to aid teachers and how to set up meaningful navigation aids will be discussed in four steps:\ud 1.) Theoretical considerations; 2.) First step of Web-Didactics: Decontextualisation; 3.) Second step of Web-\ud Didactics: Recontextualisation; 4.) Research.
