    Maxillary nerve block: A comparison between the greater palatine canal and high tuberosity approaches.

    Aim: Analgesia and anxiolysis during dental procedures are important for dental care and patient compliance. This study aims to compare two classical maxillary nerve block (MNB) techniques: the greater palatine canal (GPC) and the high tuberosity (HT) approaches, seldom used in routine dental practice. Methods: The study was conducted on 30 patients, scheduled for sinus lift surgery, who were randomly divided into 2 groups: the GPC approach to the MNB was used in 15 and the HT one in the other 15 patients. Anxiolysis was also used, depending on the results of the pre- preoperative assessment. Patients\u2019 sensations/pain during the procedure, details about anesthesia, and the dentist\u2019s considerations were all recorded. Data are expressed as mean \ub1SD. Statistical tests including ANOVA, \u3c72 following Yates correction and linear regression analysis were carried out. A < 0.05 p value was considered significant. Results: Study results showed that the anesthesia was effective and constant in the molar and premolar area. Additional infiltrations of local anesthetics were necessary for vestibular and palatal areas in the anterior oral cavity, respectively, in the GPC and HT groups. The two techniques were equally difficult to carry out in the dentist\u2019s opinion. There were no differences in pain or unpleasant sensations between the two groups, nor were any anesthesia-related complications reported. Conclusion: The GPC approach ensures effective anesthesia in the posterior maxillary region as far as both the dental pulp and the palatal/vestibular mucous membranes are concerned; the HT approach did not guarantee adequate anesthesia of the pterygopalatine branch of the maxillary nerve. These regional anesthesia techniques were characterized by a low incidence of intra and postoperative pain, no noteworthy complications, and high patient satisfaction

    Severe hypertension in childhood due to prolonged skin application of a mineralocorticoid ointment

    We report the case of a 9-year-old boy suffering from exzematous dermatitis who was treated for 6 years with a daily dose of 100 mg of a dermatological ointment containing 9 alpha-fluoroprednisolone-21-acetate. At examination the patient's blood pressure was persistently 230/160 mm Hg and was considered essential in origin after secondary forms of arterial hypertension had been excluded. Treatment with nifedipine and labetalol lowered the blood pressure to 150/100 mm Hg. When we became aware of the dermatological treatment, we advised its discontinuance. In the subsequent 7 days, the blood pressure fell to hypotensive levels (75/40 mm Hg) and then became normal a few days after discontinuance of the antihypertensive therapy. This case suggests that prolonged use of topical steroids, commonly prescribed for skin diseases, may cause hypertension, especially in childhood

    El  siguiente  recorrido se  enmarca dentro  del  Proyecto  de Investigación  PI  V112 “Destinos de(s)ubjetivación en infancia/s y adolescencia/s, intersecciones y comunidad”, dirigido por la Dra. Patricia V. Weigandt y Co-dirigido por la Lic. y Prof. Marina  La  Vecchia  (CURZA-UNCo).  Se  inicia  con  un recorrido  dentro  del  espacio  de investigación  de  la  autora  del  texto,  y  es  seguido  por  una reflexión  acerca  de  las condiciones  de  admisibilidad  descriptas  en  el  “Protocolo  de  Servicio  de Guardias Pasivas  para  Personas  en  Situación  de  Violencia  en  el  Ámbito  de  las  Relaciones Familiares e Interpersonales”  del  año  2019,  de  una  Institución  de  la  provincia  de  Río Negro.  Se  retoman aspectos  conceptuales  provenientes  del  campo  de  los  estudios  de género,  como  así  también  de la teoría  psicoanalítica.  Asimismo,  se  vale  del  marco normativo  vigente  en  Argentina  para  el  abordaje de  las  situaciones  de  situaciones  por razones de género, como así también la normativa nacional vigente para los padecimientos  de  salud  mental  y  las  adicciones.  En  este  sentido,  se  intenta reflexionar acerca de los modos institucionales de acoger, alojar o (des)alojar a mujeres atravesadas por diversos  tipos  de  padecimientos,  que  se  encuentran  a  su  vez,  en  situación  de violencia  por razones  de  género  y  los  efectos  de(s)ubjetivación  que  estas  prácticas pueden llegar a producir en ellas

    Análise palinológica de sedimentos cretáceos da região de Marabá, estado do Pará

    The work here presented deals specifically with the palynological analysis of greyish sandstones considered as belonging to the "Barreiras Group", that occur near Marabá, State of Pará. The study of these sediments revealed the presence of an abundant and Lower Cretaceous microflora. The species identified permit the precise establishment of an Aptian age for the sandstones. The depositional environment, inferred from the fossil content, is interpreted as continental, possibly fluvio-lacustrine.O trabalho apresentado trata especificamente da análise polínica de arenitos cinzentos, atribuídos ao Grupo Barreiras, que ocorrem nas proximidades de Marabá, Estado do Pará. O estudo dos sedimentos revelou a presença de uma microflora abundante e diversificada, característica do Cretáceo Inferior. As espécies identificadas permitem com precisão estabelecer uma idade correspondente ao Aptiano para os arenitos. O ambiente de deposição, deduzido a partir do conteúdo fóssil, é interpretado como continental, de natureza possivelmente flúvio-lacustre
