    The Development of A Systematic Discharge Planning Process For the Care of Copd Patients In A Small Urban Community Hospital

    Background: Several attempts have been made to examine factors that influence 30-day readmissions in a hospital setting to ensure that inpatient care is accompanied by an effective post-discharge plan that can decrease 30-day readmissions to guide hospitals to use practices that increase hospitals ‘quality implications (Shah et al., 2015; Kripalani et al., 2007; Rinne et al., 2017, Jenks, Williams and Coleman, 2009, Shah, Press, Husingh-Scheetz & White, 2016; Sickler et al., 2015; Pruitt, 2018; Hansen et al., 2013; Simmering et al., 2016; Alper, O’Malley, & Greenwald, 2019). Purpose: To determine the role of post-discharge care in 30-day readmissions along with the typical clinical outcomes identified, we examined a small urban hospital population and the patient characteristics in each post-discharge care setting (HSC, HHC, LTAC, and SNF). Patients and Methods: A retrospective study was conducted in patients with COPD hospitalizations using the data from a small urban community hospital from 2014 to 2019, n = 1,008. Results: Home health care was identified as having the highest readmission rate in this small urban community hospital using a test of proportions. The weighted variables from a researcher-developed covariate scoring table were analyzed using a Chi-square analysis.     In-packet Bloom filters: Design and networking applications

    The Bloom filter (BF) is a well-known space-efficient data structure that answers set membership queries with some probability of false positives. In an attempt to solve many of the limitations of current inter-networking architectures, some recent proposals rely on including small BFs in packet headers for routing, security, accountability or other purposes that move application states into the packets themselves. In this paper, we consider the design of such in-packet Bloom filters (iBF). Our main contributions are exploring the design space and the evaluation of a series of extensions (1) to increase the practicality and performance of iBFs, (2) to enable false-negative-free element deletion, and (3) to provide security enhancements. In addition to the theoretical estimates, extensive simulations of the multiple design parameters and implementation alternatives validate the usefulness of the extensions, providing for enhanced and novel iBF networking applications.Comment: 15 pages, 11 figures, preprint submitted to Elsevier COMNET Journa

    Genetic variation and gastric cancer risk: a field synopsis and meta-analysis.

    BACKGROUND: Data on genetic susceptibility to sporadic gastric carcinoma have been published at a growing pace, but to date no comprehensive overview and quantitative summary has been available. METHODS: We conducted a systematic review and meta-analysis of the evidence on the association between DNA variation and risk of developing stomach cancer. To assess result credibility, summary evidence was graded according to the Venice criteria and false positive report probability (FPRP) was calculated to further validate result noteworthiness. Meta-analysis was also conducted for subgroups, which were defined by ethnicity (Asian vs Caucasian), tumour histology (intestinal vs diffuse), tumour site (cardia vs non-cardia) and Helicobacter pylori infection status (positive vs negative). RESULTS: Literature search identified 824 eligible studies comprising 2 530 706 subjects (cases: 261 386 (10.3%)) and investigating 2841 polymorphisms involving 952 distinct genes. Overall, we performed 456 primary and subgroup meta-analyses on 156 variants involving 101 genes. We identified 11 variants significantly associated with disease risk and assessed to have a high level of summary evidence: MUC1 rs2070803 at 1q22 (diffuse carcinoma subgroup), MTX1 rs2075570 at 1q22 (diffuse), PSCA rs2294008 at 8q24.2 (non-cardia), PRKAA1 rs13361707 5p13 (non-cardia), PLCE1 rs2274223 10q23 (cardia), TGFBR2 rs3087465 3p22 (Asian), PKLR rs3762272 1q22 (diffuse), PSCA rs2976392 (intestinal), GSTP1 rs1695 11q13 (Asian), CASP8 rs3834129 2q33 (mixed) and TNF rs1799724 6p21.3 (mixed), with the first nine variants characterised by a low FPRP. We also identified polymorphisms with lower quality significant associations (n=110). CONCLUSIONS: We have identified several high-quality biomarkers of gastric cancer susceptibility. These data will form the backbone of an annually updated online resource that will be integral to the study of gastric carcinoma genetics and may inform future screening programmes.This is the author accepted manuscript.     Ultra-Stable and Robust Response to X-Rays in 2D Layered Perovskite Micro-Crystalline Films Directly Deposited on Flexible Substrate

    2D layered hybrid perovskites have recently attracted an increasing interest as active layers in LEDs and UV–Vis photodetectors. 2D perovskites crystallize in a natural self-assembled quantum well-like structure and possess several interesting features among which low-temperature (<100 °C) synthesis and low defect density. Here are presented solid-state ionizing radiation direct detectors based on the 2D layered hybrid perovskite PEA2PbBr4 (PEA = C6H5C2H4NH3+) deposited from solution using scalable techniques and directly integrated onto a pre-patterned flexible substrate in the form of micro-crystalline films displaying crystal-like behavior, as evidenced by the ultra-fast (sub-microsecond) and good detection performances under UV light. The effective detection of X-rays (up to 150 kVp) is demonstrated with sensitivity values up to 806 µC Gy−1 cm−2 and Limit of Detection of 42 nGy s−1, thus combining the excellent performance for two relevant figures of merit for solid-state detectors. Additionally, the tested devices exhibit exceptionally stable response under constant irradiation and bias, assessing the material robustness and the intimate electrical contact with the electrodes.     A role for NRAGE in NF-κB activation through the non-canonical BMP pathway

    <p>Abstract</p> <p>Background</p> <p>Previous studies have linked neurotrophin receptor-interacting MAGE protein to the bone morphogenic protein signaling pathway and its effect on p38 mediated apoptosis of neural progenitor cells via the XIAP-Tak1-Tab1 complex. Its effect on NF-κB has yet to be explored.</p> <p>Results</p> <p>Herein we report that NRAGE, via the same XIAP-Tak1-Tab1 complex, is required for the phosphorylation of IKK -α/β and subsequent transcriptional activation of the p65 subunit of NF-κB. Ablation of endogenous NRAGE by siRNA inhibited NF-κB pathway activation, while ablation of Tak1 and Tab1 by morpholino inhibited overexpression of NRAGE from activating NF-κB. Finally, cytokine profiling of an NRAGE over-expressing stable line revealed the expression of macrophage migration inhibitory factor.</p> <p>Conclusion</p> <p>Modulation of NRAGE expression revealed novel roles in regulating NF-κB activity in the non-canonical bone morphogenic protein signaling pathway. The expression of macrophage migration inhibitory factor by bone morphogenic protein -4 reveals novel crosstalk between an immune cytokine and a developmental pathway.</p
