Thyroid Isthmus Length and Iodine Turnover as Predictors of Successful Radioactive Iodine Therapy in Patients with Graves’ Disease
Radioactive iodine (RAI) therapy is an effective treatment option for Graves’ disease. However, predicting treatment failures after RAI therapy remains controversial. The objective of this study was to investigate the factors associated with the success rate of RAI therapy for treatment of Graves’ hyperthyroidism. Thyroid functional outcome, pre-RAI ultrasonographic features, and clinical parameters were evaluated retrospectively in 98 patients followed up for at least 12 months after RAI (mean RAI dose was 11.7 ± 1.8 mCi). Hypothyroidism was achieved in 59 patients (60.2%), and euthyroidism in 16 patients (16.3%), while 23 patients (23.5%) remained hyperthyroid. Age, sex, body mass index, pre-RAI thyroid function, or thyroid-stimulating immunoglobulin levels were not associated with treatment outcome. Length of thyroid isthmus (p=0.028) and 2- to 24-hour iodine uptake ratios (p=0.002) were significantly associated with treatment failure, which was defined as a persistent hyperthyroid status after RAI therapy. Patients with a longer isthmus had a higher risk of remaining hyperthyroid, with a threshold for isthmus length of 5.2 mm, with a sensitivity of 69.6% and specificity of 70.3% for treatment success. Measuring the length of the thyroid isthmus can be a simple and useful way to predict RAI treatment outcome
EFFECTS OF SPECIFIC MUSCLE IMBALANCE IMPROVEMENT TRAINING ON THE BALANCE ABILITY IN ELITE FENCERS
The purpose of this study was to investigate the effects of specific muscle imbalance improvement training (SMIIT) on the balance ability. Subjects were 9 male national team fencers with 28.2±2.2 yrs, 182.3±4.0 cm, and 76.5±8.2 kg. The SMIIT included flexibility training, Pilates, muscle balance training and was conducted for 12 weeks with 4 times per week. As a result, there was no significant difference in COM dispersion among static balance maintaining abilities, but reduction in the COP dispersion was shown. Body extract of tail amputated zebrafish promotes culturing of primary fin cells from glass catfish
The most spectacular regenerative events in vertebrates are epimorphic regeneration. In this study, interestingly, a whole-body extract 24 h after tail amputation enhanced primary cell growth and viability compared to that of a non-tail amputated body. Additionally, these effects of extract treatment in vitro were dose-dependent occurring at concentrations of 0.02, 0.05 and 0.1 mg/ml. This is the first in vitro study on the interaction between primary fin cells from glass catfish and tail amputated body extracts of zebrafish. Relationships among Physical Activity Level, Health-promoting Behavior, and Physiological Variables in Korean University Students
Purpose: Many Korean college students suffer from physical inactivity and mental health problems. However, it has not been sufficiently reported how this lack of exercise and health-related behavior affect their health. The present study was performed to identify the relationships among physical activity level, health-promoting behavior, and physiological variables in Korean undergraduate and graduate students. Methods: Participants were 115 undergraduate and graduate students from one university in Seoul. The Pearsons correlation analysis was performed using SPSS for Windows. Results: Physical activity level had significant positive correlations with health-promoting behavior (r=.32, p=.001) and exercise self-efficacy (r=.25, p=.008), and health-promoting behavior had a significant correlation with depression (r=-.33, p<.001) and exercise self-efficacy (r=.44, p<.001). Additionally, physical activity level had significant correlations with triglyceride (r=-.20, p=.034) and vitamin D (r=.20, p=.029) levels. The high density cholesterol level had significant negative correlations with systolic blood pressure (r=-.33, p<.001), diastolic blood pressure (r=-.29, p=.002), and vitamin D (r=-.20, p=.035) levels. Conclusion: Physical activity level or health-promoting behavior had significant relationships with the health status of college students. Strategies need to be developed to improve health-promoting behaviors among college students
Anti-allergic and anti-inflammatory effects of butanol extract from Arctium Lappa L
Background: Atopic dermatitis is a chronic, allergic inflammatory skin disease that is accompanied by markedly increased levels of inflammatory cells, including eosinophils, mast cells, and T cells. Arctium lappa L. is a traditional medicine in Asia. This study examined whether a butanol extract of A. lappa (ALBE) had previously unreported anti-allergic or anti-inflammatory effects.Methods: This study examined the effect of ALBE on the release of ??-hexosaminidase in antigen-stimulated-RBL-2H3 cells. We also evaluated the ConA-induced expression of IL-4, IL-5, mitogen-activated protein kinases (MAPKs), and nuclear factor (NF)-??B using RT-PCR, Western blotting, and ELISA in mouse splenocytes after ALBE treatment.Results: We observed significant inhibition of ??-hexosaminidase release in RBL-2H3 cells and suppressed mRNA expression and protein secretion of IL-4 and IL-5 induced by ConA-treated primary murine splenocytes after ALBE treatment. Additionally, ALBE (100 ??g/mL) suppressed not only the transcriptional activation of NF-??B, but also the phosphorylation of MAPKs in ConA-treated primary splenocytes.Conclusions: These results suggest that ALBE inhibits the expression of IL-4 and IL-5 by downregulating MAPKs and NF-??B activation in ConA-treated splenocytes and supports the hypothesis that ALBE may have beneficial effects in the treatment of allergic diseases, including atopic dermatitis. ?? 2011 Sohn et al; licensee BioMed Central Ltd
Triglyceride glucose index predicts coronary artery calcification better than other indices of insulin resistance in Korean adults: the Kangbuk Samsung Health Study
Purpose Insulin resistance is one of the most important mechanisms in the development of diabetes, and it is closely related to the presence and severity of coronary heart disease. Triglyceride glucose (TyG) index is a useful marker of insulin resistance; however, few studies have investigated the relationship between TyG and subclinical atherosclerosis. Therefore, we evaluated the association of TyG and subclinical coronary atherosclerosis as measured by coronary artery calcium score (CACS). Methods Our study included 30,776 participants (mean age of 41 years, 80.4% male) enrolled in a health screening program, in whom CACS were measured. Homeostasis model assessment of insulin resistance (HOMA-IR), TyG index, TyG-body mass index (BMI), and TyG-waist circumference (WC) were subsequently analyzed. Indices were calculated using the following formulae: HOMA-IR=fasting insulin (μU/mL)×fasting plasma glucose (FPG; mmol/L)/22.5; TyG index=Ln [TG (mg/dL)×FPG (mg/dL)/2]; TyG-BMI=TyG index×BMI; and TyG-WC=TyG index×WC. CACS was measured using multidetector computed tomography, and the presence of coronary artery calcification (CAC) was defined by CACS>0. Results The prevalence of CAC was 14.4% in the study population. Multivariate logistic regression analysis showed that participants with TyG-BMI in the highest tertile were 1.638 times more likely to have CAC after adjustment for other metabolic parameters compared with participants with TyG-BMI in the lowest tertile (odds ratio, 1.612; 95% confidence interval, 1.465 to 1.774). The receiver operating characteristics curve for prediction of CAC showed that TyG-WC index had a higher area under the curve (AUC=0.626) than other indices (AUCTyG=0.617, AUCTyG-BMI=0.616, AUCHOMA-IR=0.562). Conclusion TyG index predicted CAC better than other markers of insulin resistance, and could be a useful marker for predicting subclinical atherosclerosis
Initial serum sodium concentration determines the decrease in sodium level after terlipressin administration in patients with liver cirrhosis
BACKGROUND: Terlipressin, as a prodrug of vasopressin, has agonistic effects on the V1 receptor and partial agonistic effects on renal vasopressin V2 receptors. However, its effects on serum sodium concentration are controversial. METHODS: This study retrospectively investigated 127 patients with liver cirrhosis to examine the incidence and risk factors for the decrease in serum sodium level following terlipressin administration. RESULTS: Terlipressin was prescribed for bleeding control (99) and management of hepatorenal syndrome (28). Serum sodium level decreased from 134.0 ± 6.5 mmol/L to 130.4 ± 6.2 mmol/L during or after terlipressin treatment (P < 0.001) in all patients. In 45 patients (35.4%), the serum sodium concentration decreased by > 5 mmol/L, in 29 patients (22.8%); by 5–10 mmol/L; and in 16 patients (12.6%), by > 10 mmol/L. Five patients in the latter group showed neurological manifestations. In the univariate analysis, several factors including age, purpose of use, serum creatinine, and Model for End-Stage Liver Disease score, representing liver function, were significantly associated with the decrease in serum sodium after terlipressin administration. However, a multivariate analysis revealed that only initial sodium level was the most powerful predictor of terlipressin-induced reduction in serum sodium. CONCLUSION: An acute reduction in serum sodium concentration was not uncommon during terlipressin treatment, and the baseline serum sodium level was closely related to the reduction in serum sodium concentration
Analysis of the predictive role and new proposal for surgical strategies based on the modified Tomita and Tokuhashi scoring systems for spinal metastasis
BACKGROUND: We sought to identify preoperative factors significantly correlated with survival. We also aimed to evaluate the validity of the prognostic scores in the Tomita and Tokuhashi systems and discuss several aspects to improve the predictive accuracy of these systems. Moreover, we suggest modified criteria for selecting treatment strategies. METHODS: In total, the outcomes of 112 patients with spinal metastasis who underwent surgery between January 2006 and June 2011 were retrospectively reviewed. The validity of the prognostic scores was assessed on the basis of their correlation with survival. For various primary malignancies, new scoring criteria were applied in each system according to the survival results obtained in this study. Each revised scoring system was adjusted with a similar principle of scoring as described previously. Patient survival according to each preoperative factor was analyzed by the Kaplan-Meier method. The predictive value of each scoring system was evaluated by the log-rank test and Cox regression analysis. RESULTS: The interval from the diagnosis of the primary malignancy to that of spinal metastasis (p = 0.023) and the interval from the diagnosis of spinal metastasis to surgery (p = 0.039) were significantly correlated with survival. Regarding Tokuhashi scores, the correlation coefficient was 0.790 before adjustment (p = 0.001) and 0.853 after adjustment (p < 0.001). For Tomita scores, the correlation coefficient was -0.994 (p < 0.001) both before and after adjustment. CONCLUSIONS: Tomita scores more accurately predicted survival than Tokuhashi scores. It is helpful to evaluate both scoring systems with adjustment for primary malignancy depending on the clinical setting. Patients with Tomita scores less than or equal to 8 and Tokuhashi scores greater than or equal to 6 are recommended to undergo surgical management
