    Assessing Patient Perceptions of Pre-Visit Methods to Reduce Wait Time and Provider Burden

    Due to multiple factors, patients at ambulatory settings tend to experience long appointment wait times, negatively impacting patient satisfaction and overall well-being. One proposed solution is to use pre-visit data collection to simplify check-in and screening processes at the office. This project surveyed patients at Brookfield Primary Care & Pediatrics regarding quantitative wait time data and qualitative perceptions on the proposed solution.     ZSM-5/SBA-15 versus Al-SBA-15 as supports for the hydrocracking/hydroisomerization of alkanes

    [EN] Al-SBA-15 and ZSM-5/SBA-15 (ZSC) composite were synthesized following hydrothermal procedures in the presence of triblock copolymer Poly(ethylene glycol)- poly(propylene glycol)-poly(ethylene glycol) (Pluronic P123) and a mixture of tetrapropylammonium bromide (TPABr)/Pluronic P123 templates, respectively. Pt (0.5 wt.%)/Al-SBA-15 and ZSC bifunctional catalysts were then prepared by a wet impregnation methodology and investigated in the hydrocracking/hydroisomerization of n-decane. Pt/ZSC, containing a trimodal porous texture and high Bronsted acidity, exhibited hydrocracking activity while Pt/Al-SBA-15, showing merely Lewis acidity, was active for hydroisomerization. At 300 degrees C the conversion of n-decane on Pt/ZSC reached 98% and cracking selectivity was about 99%.     Assessing Dietary Branched-Chain Amino Acids to Achieve Linear Programming Goals through Model Extrapolation and Empirical Research

    Renewed interest, especially in the United States, has sparked in assessing branched-chain amino acid interactions in practical diets for broilers. Indeed, as L-valine enters formulation bird nitrogen excesses are reduced as diet protein falls to the new first limiting amino acid (e.g., isoleucine, arginine, or tryptophan). For a United States based example, the result is less oilseeds and more gains, which typically result in increased inclusions in corn or corn by-products, coupled with a concomitant increase in dietary leucine. The proceedings outline the foundations of the branched-chain amino acid early research, antagonism studies, and a meta-analysis conducted on publications with Cobb and Ross birds from 2000 to present. Results indicate that branched-chain amino acid interactions can occur in broilers fed on practical diets, and that responses vary by strain

    Direct costs of hypertensive patients admitted to hospital in Vietnam:a bottom-up micro-costing analysis

    Background: There is an economic burden associated with hypertension both worldwide and in Vietnam. In Vietnam, patients with uncontrolled high blood pressure are hospitalized for further diagnosis and initiation of treatment. Because there is no evidence on costs of inpatient care for hypertensive patients available yet to inform policy makers, health insurance and hospitals, this study aims to quantify direct costs of inpatient care for these patients in Vietnam. Methods: A retrospective study was conducted in a hospital in Vietnam. Direct costs were analyzed from the health-care provider's perspective. Hospital-based costing was performed using both bottom-up and micro-costing methods. Patients with sole essential or primary hypertension (ICD-code I10) and those comorbid with sphingolipid metabolism or other lipid storage disorders (ICD-code E75) were selected. Costs were quantified based on financial and other records of the hospital. Total cost per patient resulted from an aggregation of laboratory test costs, drug costs, inpatient-days' costs and other remaining costs, including appropriate allocation of overheads. Both mean and medians, as well as interquartile ranges (IQRs) were calculated. In addition to a base-case analysis, specific scenarios were analyzed. Results: 230 patients were included in the study (147 cases with I10 code only and 83 cases with I10 combined with E75). Median length of hospital stay was 6 days. Median total direct costs per patient were US65(IQR:3795).TotalcostsperpatientwerehigherinthecombinedhypertensiveandlipidpopulationthaninthesolehypertensivepopulationatUS65 (IQR: 37 -95). Total costs per patient were higher in the combined hypertensive and lipid population than in the sole hypertensive population at US78 and US$53, respectively. In all scenarios, hospital inpatient days' costs were identified as the major cost driver in the total costs. Conclusions: Costs of hospitalization of hypertensive patients is relatively high compared to annual medication treatment at a community health station for hypertension as well as to the total health expenditure per capita in Vietnam. Given that untreated/undetected hypertension likely leads to more expensive treatments of complications, these findings may justify investments by the Vietnamese health-care sector to control high blood pressure in order to save downstream health care budgets

    Visible light-induced switching of soft matter materials properties based on thioindigo photoswitches

    Thioindigos are visible light responsive photoswitches with excellent spatial control over the conformational change between their trans- and cis- isomers. However, they possess limited solubility in all conventional organic solvents and polymers, hindering their application in soft matter materials. Herein, we introduce a strategy for the covalent insertion of thioindigo units into polymer main chains, enabling thioindigos to function within crosslinked polymeric hydrogels. We overcome their solubility issue by developing a thioindigo bismethacrylate linker able to undergo radical initiated thiol-ene reaction for step-growth polymerization, generating indigo-containing polymers. The optimal wavelength for the reversible trans-/cis- isomerisation of thioindigo was elucidated by constructing a detailed photochemical action plot of their switching efficiencies at a wide range of monochromatic wavelengths. Critically, indigo-containing polymers display significant photoswitching of the materials’ optical and physical properties in organic solvents and water. Furthermore, the photoswitching of thioindigo within crosslinked structures enables visible light induced modulation of the hydrogel stiffness. Both the thioindigo-containing hydrogels and photoswitching processes are non-toxic to cells, thus offering opportunities for advanced applications in soft matter materials and biology-related research

    Outpatient antibiotic prescribing for acute respiratory infections in Vietnamese primary care settings by the WHO AWaRe (Access, Watch and Reserve) classification: An analysis using routinely collected electronic prescription data

    Background: This study aims to investigate patterns of antibiotic prescribing and to determine patient-specific factors associated with the choice of antibiotics by the World Health Organization's Access-Watch-Reserve (WHO AWaRe) class for acute respiratory infections (ARIs) in rural primary care settings in northern Vietnam. Methods: We retrospectively reviewed health records for outpatients who were registered with the Vietnamese Health Insurance Scheme, visited one of 112 commune health centres in 6 rural districts of Nam Dinh province, Vietnam during 2019, and were diagnosed with ARIs. Patient-level prescription data were collected from the electronic patient databases. We used descriptive statistics to investigate patterns of antibiotic prescribing, with the primary outcomes including total antibiotic prescriptions and prescriptions by WHO AWaRe group. We identified patient-specific factors associated with watch-group antibiotic prescribing through multivariable logistic regression analysis. Findings: Among 193,010 outpatient visits for ARIs observed in this study, 187,144 (97.0%) resulted in an antibiotic prescription, of which 172,976 (92.5%) were access-antibiotics, 10,765 (5.6%) were watch-antibiotics, 3366 (1.8%) were not-recommended antibiotics. No patients were treated with reserve-antibiotics. The proportion of watch-antibiotic prescription was highest amongst children under 5-years old (18.1%, compared to 9.5% for 5–17-years, 4.9% for 18–49-years, 4.3% for 50–64-years, and 3.7% for 65-and-above-years). In multivariable logistic regression, children, district, ARI-type, comobid chronic respiratory illness, and follow-up visit were associated with higher likelihood of prescribing watch-group antibiotics. Interpretation: The alarmingly high proportion of antibiotic prescriptions for ARIs in primary care, and the frequent use of watch-antibiotics for children, heighten concerns around antibiotic overuse at the community level. Antimicrobial stewardship interventions and policy attention are needed in primary care settings to tackle the growing threat of antibiotic resistance

    Magnetic CoFe2O4 nanoparticles were synthesised by one step synthetic method through thermal decomposition of Co and Fe precursors in triethylenetetramine solvent at high temperature. The advantage of this method is the ability to make monodisperse nanoparticles with high water-dispersibility and stability. The particle size can be tuned in the range of 7-11.3 nm by varying synthetic conditions. The obtained particles with small DLS size (less than 21 nm) are ready to disperse and stable in aqueous solution for weeks without any surface modification
