Functional neuroimaging of conversion disorder: The role of ancillary activation
AbstractBackgroundPrevious functional neuroimaging studies investigating the neuroanatomy of conversion disorder have yielded inconsistent results that may be attributed to small sample sizes and disparate methodologies. The objective of this study was to better define the functional neuroanatomical correlates of conversion disorder.MethodsTen subjects meeting clinical criteria for unilateral sensory conversion disorder underwent fMRI during which a vibrotactile stimulus was applied to anesthetic and sensate areas. A block design was used with 4 s of stimulation followed by 26 s of rest, the pattern repeated 10 times. Event-related group averages of the BOLD response were compared between conditions.ResultsAll subjects were right-handed females, with a mean age of 41. Group analyses revealed 10 areas that had significantly greater activation (p < .05) when stimulation was applied to the anesthetic body part compared to the contralateral sensate mirror region. They included right paralimbic cortices (anterior cingulate cortex and insula), right temporoparietal junction (angular gyrus and inferior parietal lobule), bilateral dorsolateral prefrontal cortex (middle frontal gyri), right orbital frontal cortex (superior frontal gyrus), right caudate, right ventral-anterior thalamus and left angular gyrus. There was a trend for activation of the somatosensory cortex contralateral to the anesthetic region to be decreased relative to the sensate side.ConclusionsSensory conversion symptoms are associated with a pattern of abnormal cerebral activation comprising neural networks implicated in emotional processing and sensory integration. Further study of the roles and potential interplay of these networks may provide a basis for an underlying psychobiological mechanism of conversion disorder

Consisting of two cross-sectional studies, the present study investigated whether psychological flexibility mediates the relations between self-concealment and negative psychological outcomes. Study 1 examined whether psychological flexibility mediates the relations between self-concealment and emotional distress in stressful interpersonal situations. In addition to replicating results of Study 1, Study 2 investigated whether psychological flexibility mediates the relationship between self-concealment and general psychological ill-health. Psychological flexibility was found to mediate the relation between self-concealment and emotional distress in stressful interpersonal settings and to partially mediate the relationship between self-concealment and general psychological ill-health
Single-Cell Profiling Reveals Inflammatory Polarization of Human Carotid Versus Femoral Plaque Leukocytes
Femoral atherosclerotic plaques are less inflammatory than carotid plaques histologically, but limited cell-level data exist regarding comparative immune landscapes and polarization at these sites. We investigated intraplaque leukocyte phenotypes and transcriptional polarization in 49 patients undergoing femoral (n = 23) or carotid (n = 26) endarterectomy using single-cell RNA-Seq (scRNA-Seq; n = 13), flow cytometry (n = 24), and IHC (n = 12). Comparative scRNA-Seq of CD45+-selected leukocytes from femoral (n = 9; 35,265 cells) and carotid (n = 4; 30,655 cells) plaque revealed distinct transcriptional profiles. Inflammatory foam cell-like macrophages and monocytes comprised higher proportions of myeloid cells in carotid plaques, whereas noninflammatory foam cell-like macrophages and LYVE1-overexpressing macrophages comprised higher proportions of myeloid cells in femoral plaque (P \u3c 0.001 for all). A significant comparative excess of CCR2+ macrophages in carotid versus plaque was observed by flow cytometry in a separate validation cohort. B cells were more prevalent and exhibited a comparatively antiinflammatory profile in femoral plaque, whereas cytotoxic CD8+ T cells were more prevalent in carotid plaque. In conclusion, human femoral plaques exhibit distinct macrophage phenotypic and transcriptional profiles as well as diminished CD8+ T cell populations compared with human carotid plaques
An Introduction to the Chandra Carina Complex Project
The Great Nebula in Carina provides an exceptional view into the violent
massive star formation and feedback that typifies giant HII regions and
starburst galaxies. We have mapped the Carina star-forming complex in X-rays,
using archival Chandra data and a mosaic of 20 new 60ks pointings using the
Chandra X-ray Observatory's Advanced CCD Imaging Spectrometer, as a testbed for
understanding recent and ongoing star formation and to probe Carina's regions
of bright diffuse X-ray emission. This study has yielded a catalog of
properties of >14,000 X-ray point sources; >9800 of them have multiwavelength
counterparts. Using Chandra's unsurpassed X-ray spatial resolution, we have
separated these point sources from the extensive, spatially-complex diffuse
emission that pervades the region; X-ray properties of this diffuse emission
suggest that it traces feedback from Carina's massive stars. In this
introductory paper, we motivate the survey design, describe the Chandra
observations, and present some simple results, providing a foundation for the
15 papers that follow in this Special Issue and that present detailed catalogs,

Prospect theory and tax evasion: a reconsideration of the Yitzhaki puzzle
The standard expected utility (EUT) model of tax evasion predicts that evasion is decreasing in the marginal tax rate (the Yitzhaki puzzle). Recent literature shows cases in which incorporating prospect theory (PT) does and does not overturn the Puzzle. In a general environment that nests both PT and EUT preferences, we provide a detailed study of how the elements of PT affect the Puzzle. PT does not always reverse the Puzzle, hence we give and interpret conditions for when it does and does not. When allowing for stigma and/or variable audit probability, PT reverses the Puzzle in the same way and with the same limitations as does EUT, if equally augmented
Comorbidities in patients with gout prior to and following diagnosis: case-control study
OBJECTIVES: To determine the burden of comorbidities in patients with gout at diagnosis and the risk of developing new comorbidities post diagnosis.
METHODS: There were 39 111 patients with incident gout and 39 111 matched controls identified from the UK Clinical Practice Research Data-link. The risk of comorbidity before (ORs) and after the diagnosis of gout (HRs) were estimated, adjusted for age, sex, diagnosis year, body mass index, smoking and alcohol consumption.
RESULTS: Gout was associated with adjusted ORs (95% CIs) of 1.39 (1.34 to 1.45), 1.89 (1.76 to 2.03) and 2.51 (2.19 to 2.86) for the Charlson index of 1-2, 3-4 and >/=5, respectively. Cardiovascular and genitourinary diseases, in addition to hyperlipidaemia, hypothyroidism, anaemia, psoriasis, chronic pulmonary diseases, osteoarthritis and depression, were associated with a higher risk for gout. Gout was also associated with an adjusted HR (95% CI) of 1.41 (1.34 to 1.48) for having a Charlson index >/=1. Median time to first comorbidity was 43 months in cases and 111 months in controls. Risks for incident comorbidity were higher in cardiovascular, genitourinary, metabolic/endocrine and musculoskeletal diseases, in addition to liver diseases, hemiplegia, depression, anaemia and psoriasis in patients with gout. After additionally adjusting for all comorbidities at diagnosis, gout was associated with a HR (95% CI) for all-cause mortality of 1.13 (1.08 to 1.18; p<0.001).
CONCLUSIONS: The majority of patients with gout have worse pre-existing health status at diagnosis and the risk of incident comorbidity continues to rise following diagnosis. The range of associated comorbidities is broader than previously recognised and merits further evaluation

Early Release Science of the exoplanet WASP-39b with JWST NIRCam
Measuring the metallicity and carbon-to-oxygen (C/O) ratio in exoplanet
atmospheres is a fundamental step towards constraining the dominant chemical
processes at work and, if in equilibrium, revealing planet formation histories.
Transmission spectroscopy provides the necessary means by constraining the
abundances of oxygen- and carbon-bearing species; however, this requires broad
wavelength coverage, moderate spectral resolution, and high precision that,
together, are not achievable with previous observatories. Now that JWST has
commenced science operations, we are able to observe exoplanets at previously
uncharted wavelengths and spectral resolutions. Here we report time-series
observations of the transiting exoplanet WASP-39b using JWST's Near InfraRed
Camera (NIRCam). The long-wavelength spectroscopic and short-wavelength
photometric light curves span 2.0 - 4.0 m, exhibit minimal systematics,
and reveal well-defined molecular absorption features in the planet's spectrum.
Specifically, we detect gaseous HO in the atmosphere and place an upper
limit on the abundance of CH. The otherwise prominent CO feature at 2.8
m is largely masked by HO. The best-fit chemical equilibrium models
favour an atmospheric metallicity of 1-100 solar (i.e., an enrichment
of elements heavier than helium relative to the Sun) and a sub-stellar
carbon-to-oxygen (C/O) ratio. The inferred high metallicity and low C/O ratio
may indicate significant accretion of solid materials during planet formation

A broadband thermal emission spectrum of the ultra-hot Jupiter WASP-18b
Close-in giant exoplanets with temperatures greater than 2,000 K (''ultra-hot
Jupiters'') have been the subject of extensive efforts to determine their
atmospheric properties using thermal emission measurements from the Hubble and
Spitzer Space Telescopes. However, previous studies have yielded inconsistent
results because the small sizes of the spectral features and the limited
information content of the data resulted in high sensitivity to the varying
assumptions made in the treatment of instrument systematics and the atmospheric
retrieval analysis. Here we present a dayside thermal emission spectrum of the
ultra-hot Jupiter WASP-18b obtained with the NIRISS instrument on JWST. The
data span 0.85 to 2.85 m in wavelength at an average resolving power of
400 and exhibit minimal systematics. The spectrum shows three water emission
features (at 6 confidence) and evidence for optical opacity,
possibly due to H, TiO, and VO (combined significance of 3.8).
Models that fit the data require a thermal inversion, molecular dissociation as
predicted by chemical equilibrium, a solar heavy element abundance
(''metallicity'', M/H = 1.03 solar), and a
carbon-to-oxygen (C/O) ratio less than unity. The data also yield a dayside
brightness temperature map, which shows a peak in temperature near the
sub-stellar point that decreases steeply and symmetrically with longitude
