    Evaluation of Three Rapid Tests for Diagnosis of P. Falciparum and P. Vivax Malaria in Colombia.

    The diagnostic capacity of three malaria rapid diagnostic tests (RDTs), NOW-Malaria-ICT, OptiMAL-IT, and Paracheck-Pf, was evaluated against expert microscopy in Colombia. We tested 896 patients, of whom microscopy confirmed 139 P. falciparum, 279 P. vivax, and 13 mixed P.f/P.v infections and 465 negatives. Paracheck-Pf and NOW-malaria-ICT were more accurate in detecting P. falciparum (sensitivities 90.8% and 90.1%, respectively) in comparison with Optimal-IT (83.6%). NOW showed an acceptable Pf detection rate at low densities (< 500/microL), but resulted in a higher proportion of false positives. For P. vivax diagnosis, Optimal-IT had a higher sensitivity than NOW (91.0% and 81.4%, respectively). The choice between the two Pf/Pv detecting RDTs balances P. falciparum and P. vivax detection rates. Considering some degree of P. falciparum overtreatment and failure to detect all P. vivax cases as more acceptable than missing some cases of P. falciparum, we recommend careful implementation of NOW-malaria-ICT in areas where microscopy is lacking.     El objetivo del estudio caracterizar las pautas de crianza que utilizan los padres de niños y niñas con trastorno del espectro autista de 2 a 5 años, bajo un enfoque cualitativo bajo un diseño y alcance fenomenológico, en donde la población con la que se realizara la presente investigación serán padres de familia de niños entre 2 a 5 años de edad que tienen un diagnostico con espectro autista de las instituciones educativas de Preescolar Mundo Infantil Pachi y el Hogar Infantil el Manantial, se aplicaran como instrumentos una entrevista semi estructurada a los padres, un grupo focal para recolectar experiencias relacionadas con el espectro autista y la observación no participante para recolectar información asociada con la experiencia de las docentes, para finalizar se espera el análisis y triangulación de la información recolectada por medio de los instrumentos para poder dar respuesta a los objetivos planteados para la propuesta de investigación

    Evaluación del potencial antioxidante en extracto de espinaca por voltamperometría cíclica

    The antioxidant capacity of plant extracts has been studied by voltammetric techniques thanks to its sensitivity, selectivity, low cost, simplicity and speed. In this research, Cyclic Voltamperometry was used to study the antioxidant potential and stability in ethanolic extracts of spinach leaves (Spinacia oleracea) over time. The extracts were obtained after ultrasound of macerating spinach leaves with industrial ethanol. The potentiometric sweeps of the extracts at 0, 7 and 15 days of preparation were carried out in a Sørensen buffer. Electrodes of Au (work), Ag/AgCl (reference) and Pt (auxiliary) were used from -1.2 to 1.2V. The results showed a high antioxidant capacity of the extracts (oxidation potentials below 600mV) and a decrease in the current while the extracts preservation (0, 7 and 15 days), which allows the conclusion that its Antioxidant potential decreases over time describing a first-order kinetic. Also, the results of the antioxidant capacity were correlated with the Folin Ciocalteu method.La capacidad antioxidante de extractos de plantas ha sido estudiada por técnicas voltamperométricas gracias a su sensibilidad, selectividad, bajo costo, simplicidad y rapidez. En esta investigación se utilizó la Voltamperometría Cíclica para estudiar el potencial antioxidante en la preservación de extractos etanólicos de hojas de espinaca (Spinacia oleracea). Los extractos se obtuvieron luego de macerar hojas de espinaca con etanol industrial y llevar a ultrasonido. Los barridos potenciométricos de los extractos a 0, 7 y 15 días de preparación se realizaron en una solución amortiguadora Sørensen. Se utilizaron electrodos de Au (trabajo), Ag/AgCl (referencia) y Pt (auxiliar) desde -1,2 hasta 1,2V. Los resultados mostraron una alta capacidad antioxidante de los extractos (potenciales de oxidación por debajo de los 600mV) y una disminución en la corriente durante el aumento de la preservación de los extractos (0, 7 y 15 días); lo cual permitió concluir que su potencial antioxidante disminuye a través del tiempo gobernada por una cinética de primer orden. Además, los resultados de la capacidad antioxidante fueron correlacionados con la cuantificación de polifenoles totales, mediante en método de Folin Ciocalteu.A capacidade antioxidante de extratos de plantas foi estudada por meio de técnicas voltamétricas atribuído à sua sensibilidade, seletividade, baixo custo, simplicidade e velocidade. Voltametria cíclica foi utilizada nesta pesquisa para estudar o potencial antioxidante e estabilidade no extrato etanolico de folhas de espinafre (Spinacia oleracea) ao longo do tempo. Esses extratos foram obtidos por maceração das folhas de espinafre com álcool industrial e sonificadas por 40min. Os barridos potenciometricos dos Extratos foram realizados numa solução tampão de Sorensen. Eléctrodos de trabalho (Au), de Ag/AgCl (referência) e Pt (eléctrodo auxiliar) a uma velocidade de varrimento de 100mV/s foram usadas -1,2 a -1,2mV. Os resultados mostraram uma elevada capacidade antioxidante dos extratos (potenciais de oxidação mV abaixo de 600) e uma diminuição na corrente com respeito à idade dos extratos (0, 7 e 15 dias), o que permitiu concluir que o potencial antioxidante diminui ao longo do tempo por uma cinética de primeira ordem. Além disso, os resultados da capacidade antioxidante foram correlacionados com a cuantificação de polifenoles totales, através do método de Folin Ciocalteu

    This paper addresses the aggregation of wind, photovoltaic and thermal units with the aim to improve bidding in an electricity market. Market prices, wind and photovoltaic powers are assumed as data given by a set of scenarios. Thermal unit modeling includes start-up costs, variables costs and bounds due to constraints of technical operation, such as: ramp up/down limits and minimum up/down time limits. The modeling is carried out in order to develop a mathematical programming problem based in a stochastic programming approach formulated as a mixed integer linear programming problem. A case study comparison between disaggregated and aggregated bids for the electricity market of the Iberian Peninsula is presented to reveal the advantage of the aggregation

    Severe Illness Anxiety Treated by Integrating Inpatient Psychotherapy With Medical Care and Minimizing Reassurance

    Illness anxiety disorder (IAD, formerly hypochondriasis) is characterized by preoccupation with fear of serious illness despite medical reassurance. IAD is common, debilitating, challenging to treat, and results in high healthcare utilization. Outpatient management of IAD is challenging because patients can compulsively seek reassurance from numerous providers, which interferes with learning more productive coping skills. We present the case of a woman with severe IAD who presented to the emergency room with increasing frequency over several months, despite regular outpatient medical visits and escalating psychiatric care. We made the unusual decision to hospitalize her for IAD for 1 month, in the absence of typical hospitalization criteria. This hospitalization allowed us to consolidate all medical and psychiatric care into a single provider team and train all staff and family to communicate with her in a consistent manner. We successfully treated her by integrating a general cognitive-behavioral therapy (CBT) protocol into medical care and decision-making. In response to her numerous health concerns, we minimized medical work-up, reassurance, and reactive medication changes, and instead used the concerns as opportunities to reinforce the psychotherapy. This approach allowed us to simplify her medication regimen and manage her co-morbid hypertension and vitamin deficiencies. Though inpatient hospitalization is likely infeasible in most cases of IAD, outpatient providers may create similar treatment plans based on the example of our case report, without needing highly specialized expertise. Such a plan would require a straightforward understanding of IAD psychology, which we review here, combined with readily accessible tools including a universal CBT protocol, online CBT courses, and clinical symptom scales. We discuss our approach for responding to health concerns, maintaining therapeutic alliance, integrating CBT principles into patient interactions, and managing medications. Since patients with IAD share health concerns with all providers, staff, and family, we also include our own IAD communication guide, appropriate for a general audience, that demonstrates how to respond in these conversations

    Food groups consumption and sociodemographic characteristics in Mexican population

    Objective. To estimate the recommendable and non-recommendable food groups for usual consumption by sociodemographic characteristics in Mexican population. Materials and methods. Through a 7-day, semi-quan­titative food frequency questionnaire used in 2016 National Health and Nutrition Survey, we estimated the proportions of population (preschool and school children, adolescents and adults) who consumed food groups that are relevant for public health by area, region and socioeconomic status (SES). Results. Less than 50% of population consumed vegetables daily; almost 80% of the population consumed plain water daily and sweetened beverages (3 d/week). Center and Mexico City regions had the highest percent­age of fruits and vegetables consumers (p<0.012). High SES had the highest consumer´s percentage of recommendable and non-recommendable food groups. Conclusions. A high percentage of the population do not consume fruits, vegetables and plain water daily

    A Systematic Review of Reproductive Counseling in Cases of Parental Constitutional Reciprocal Translocation (9;22) Mimicking BCR-ABL1

    We aim to determine the spectrum of cytogenetic abnormalities and outcomes in unbalanced offspring of asymptomatic constitutional balanced t(9;22) carriers through a systematic literature review. We also include a case of a constitutional balanced t(9;22) carrier from our institution. Among the 16 balanced t(9;22) carriers in our review, 13 were maternal and 3 were paternal. Of the 15 unbalanced translocation cases identified, 13 were live births, one was a missed abortion, and one resulted in pregnancy termination. The spectrum of established syndromes reported among the live births was the following: trisomy 9p syndrome (6/13), dual trisomy 9p and DiGeorge syndrome (3/13), dual 9q subtelomere deletion syndrome and DiGeorge syndrome (1/13), 9q subtelomere deletion syndrome (1/13), and DiGeorge syndrome (1/13). One unbalanced case did not have a reported syndrome. The phenotype of the unbalanced cases included cardiac abnormalities (5/13), neurological findings (7/13), intellectual disability (6/10), urogenital anomalies (3/13), respiratory or immune dysfunction (3/13), and facial or skeletal dysmorphias (13/13). Any constitutional balanced reciprocal t(9;22) carrier should be counseled regarding the increased risk of having a child with an unbalanced translocation, the spectrum of possible cytogenetic abnormalities, and predicted clinical phenotype for the unbalanced derivative
