    Intracranial chordoma presenting as acute hemorrhage in a child: Case report and literature review

    BACKGROUND: Chordomas are rare, slow-growing malignant neoplasms derived from remnants of the embryological notochord. Pediatric cases comprise only 5% of all chordomas, but more than half of the reported pediatric chordomas are intracranial. For patients of all ages, intracranial chordomas typically present with symptoms such as headaches and progressive neurological deficits occurring over several weeks to many years as they compress or invade local structures. There are only reports of these tumors presenting acutely with intracranial hemorrhage in adult patients. CASE DESCRIPTION: A 10-year-old boy presented with acute onset of headache, emesis, and diplopia. Head computed tomography and magnetic resonance imaging of brain were suspicious for a hemorrhagic mass located in the left petroclival region, compressing the ventral pons. The mass was surgically resected and demonstrated acute intratumoral hemorrhage. Pathologic examination was consistent with chordoma. CONCLUSION: There are few previous reports of petroclival chordomas causing acute intracranial hemorrhage. To the authors' knowledge, this is the first case of a petroclival chordoma presenting as acute intracranial hemorrhage in a pediatric patient. Although uncommon, it is important to consider chordoma when evaluating a patient of any age presenting with a hemorrhagic lesion of the clivus

    Pneumonia is a major cause of morbidity and mortality especially in developing countries. The prevention of pneumonia and its later disabilities can be achieved by the reduction of risk factors and evaluating subjects of high risk. Risk factors evaluation was one of the three aims of this retrospective age and gender matched case control study. A total of 260 children five years and younger diagnosed with pneumonia according to ICD-10 were involved, 43% of them (n = 113) having pneumonia of viral origin and 57% (n = 147) having bacterial pneumonia. Pneumonia risk factors were determined using Chi-square and Binary Logistic Regression. Incomplete immunization found to be the most significant risk predictor among all the analyzed factors (OR = 3.71, p< 0.001)

    First Case Report of Primary Testicular Diffuse Large B-Cell Lymphoma from the Western Region of Saudi Arabia

    Primary testicular lymphoma (PTL) represents 1-2% of all types of non-Hodgkin lymphomas (NHLs) and 1-10% of testicular neoplasms. Up to the best of my knowledge, this is the first case of PTL of the diffuse large B-cell lymphoma (DLBCL) in a 60-year-old man presented with a painless mass in the left testis as revealed by physical examination in a tertiary care hospital in Al-Madinah Al-Munwarah in the western region of the Kingdom of Saudi Arabia (KSA). Radiological examination revealed a large well-defined heterogeneous predominantly hypo-echoic mass with increased vascularity in the upper portion of the testis. On the other hand, histopathological examination revealed a tumor involving the whole left testis, which was large (measuring 6 3.5 &nbsp;3.3 cm), solid and dark red with focal areas of hemorrhage and epididymal infiltration. Immunohistochemistry showed positivity of leucocyte common antigen (LCA), pan B-cell marker (CD20) and negativity of pan T-cell marker (CD3). Other immunohistochemical markers such as CD10, placental alkaline phosphatase (PLAP), cytokeratin, vimentin, desmin and S100 protein were also negative. However, there was a marked expression of Ki67 and Bcl2 markers. Accordingly, the diagnosis of DLBCL was established. The tumor was classified as stage I according to the Ann Arbor system. The case was treated by orchiectomy followed by prophylactic anthracycline-based chemotherapy and irradiation of the contralateral testis and central nervous system

    The high prevalence of psychopathological symptoms among renal patients, the reduced quality of life that adds to their ensuing actual losses, and the lack of information on their illness and treatment, indicates that health professionals should act on these specific points. By means of a critical analysis of interdisciplinarity vis-à-vis multidisciplinarity and taking advantage of the reach of group work, we propose an intervention, consisting of an opened rotating interdisciplinary group, offered to patients admitted to a Nephrology ward, in order to deal with their different demands. As an example, excerpts from one group session are presented, and the various aspects of the intervention are identified and commented on.A constatação da alta prevalência, em pacientes com doenças renais, de sintomas psicopatológicos, da perda da sua qualidade de vida, além das perdas reais, e da falta de informação sobre sua doença e tratamento, aponta para a necessidade da atuação dos profissionais da saúde sobre esses pontos. Através da análise crítica da interdisciplinaridade, em contraponto à multidisciplinaridade, e do alcance que o trabalho grupal apresenta, é feita uma proposta de intervenção junto aos pacientes internados em uma enfermaria de Nefrologia. A intervenção proposta é um grupo interdisciplinar rotativo aberto que trabalhe as diferentes demandas trazidas por esses pacientes. Como exemplo, é apresentado o recorte de um dos grupos realizados onde os diferentes aspectos dessa intervenção são identificados e comentados
