Safety and risk in road traffic: selected problems
In the field of road safety, the concept of "risk" is used to define a measurable level of road safety dependent on numerical accident exposure value. This is an approach different (even contradictory) to calculate the level of security, measured by the number of accidents or injuries. So, whether otherwise - risk assessment is needed to improve the safety of road transport and define priorities in the field of public health. Cooperative breeding (CB) can be defined as a reproductive system in which individuals other than the mother (allomothers) deliver routine care. Among primates, the strongest reliance on allomaternal care is found in callitrichids and humans. The cooperative breeding system requires the support of special motivational and cognitive processes, including increased social tolerance or attentional bias toward monitoring motivations and behaviors of others. Allomaternal care can explain the spectacular increase in brain size in the hominin lineage (the expensive brain hypothesis). This phenomenon can also account for both the emergence of menopause long before women’s death and the longevity gender gap. Moreover, the theory in question can account for impressive cognitive abilities of our species. Some issues in the field of developmental psychology have been discussed from the perspective of the CB theory

KRATTA, a triple telescope array for charged reaction products
KRATTA, a new, low threshold, broad energy range triple telescope array has been built to measure the energy, emission angles and isotopic composition of light charged reaction products. It has been equipped with fully digital chains of electronics. The array performed very well during the ASY-EOS experiment, conducted in May 2011 at GSI. The structure and performance of the array are presented using the first experimental results
Poisoning deaths in Poland : types and frequencies reported in Łódź, Kraków, Sosnowiec, Gdańsk, Wrocław and Poznań during 2009-2013
Objectives: The aim of this study has been to assess the characteristics of acute poisoning deaths in Poland over a period of time 2009–2013. Material and Methods: The analysis was based on the data obtained from the patient records stored in toxicology departments in 6 cities – Łódź, Kraków, Sosnowiec, Gdańsk, Wrocław and Poznań. Toxicological analyses were routinely performed in blood and/or urine. Major toxic substances were classified to one of the following categories: pharmaceuticals, alcohol group poisonings (ethanol and other alcohols), gases, solvents, drugs of abuse, pesticides, metals, mushrooms, others. Cases were analyzed according to the following criteria: year, age and gender of analyzed patients, toxic substance category and type of poisoning. The recorded fatal poisonings were classified according to the International Classification of Diseases. Results: The record of 261 deaths were retrospectively reviewed. There were 187 males (71.64%) and 74 females (28.36%) and the male to female ratio was 2.52. Alcohol group poisonings were more frequently responsible for deaths in men compared to all poisonings, 91.1% vs. 71.6%, respectively (p < 0.05), and pharmaceutical agents were more frequently responsible for deaths in women, 47.4% vs. 28.4%, (p < 0.05). Methanol was the most common agent in the alcohol group poisonings, accounting for 43.75% (N = 49), followed by ethylene glycol, 39.29% (N = 44), and ethanol, 16.96% (N = 19). Conclusions: Epidemiological profile data from investigation of poisoning deaths in Poland may be very useful for the development of preventive programs. Ankle-brachial index and other risk factors of diastolic dysfunction of the left ventricle
Wstęp Dysfunkcja rozkurczowa lewej komory jest
czynnikiem ryzyka rozwoju powikłań sercowo-naczyniowych.
Celem opisywanego badania było określenie
występowania dysfunkcji rozkurczowej lewej
komory przy użyciu metod ultrasonograficznych
w badanej populacji wiejskiej w zależności od współistniejących
czynników ryzyka.
Materiał i metody Badanie przeprowadzono u 178
mieszkańców wsi Boguszyce (średni wiek 53,9 ±
13,5 r.). Funkcję rozkurczową lewej komory oceniano
za pomocą parametrów echokardiograficznych,
takich jak prędkość fali E i fali A, stosunek E/A, czas
deceleracji fali E (DcT E) i czas rozkurczu izowolumetrycznego
(IVRT). W badanej populacji oceniano
również wskaźnik masy ciała (BMI), skurczowe
i rozkurczowe ciśnienie tętnicze, parametry gospodarki
lipidowej oraz wskaźnik kostka-ramię.
Wyniki W modelu regresji logistycznej wykazano, że
niezależnymi czynnikami wystąpienia dysfunkcji rozkurczowej
lewej komory są wiek (OR: 1,11; p < 0,01),
wysoki BMI (OR: 1,02; p < 0,05), nadciśnienie tętnicze
(OR: 1,61; p < 0,05), niska wartość wskaźnika
kostka-ramię (OR: 0,52; p < 0,05) i niskie stężenia
cholesterolu frakcji HDL (OR: 0,98; p < 0,05).
Wnioski Według autorów pracy dysfunkcja rozkurczowa
lewej komory występuje częściej u osób
z wieloma czynnikami ryzyka chorób sercowo-naczyniowych.
Interesujący jest fakt ujemnej korelacji
wskaźnika kostka-ramię ze wskaźnikami dysfunkcji
rozkurczowej w badanej populacji, gdzie większość
badanych miała prawidłowe wartości tego
wskaźnika.Background Diastolic dysfunction of the left ventricle is
a risk factor of cardiovascular complications. The aim of the
study was to determine the diastolic dysfunction of the left
ventricle with the use of echocardiography in the rural
population according to coexisting risk factors.
Material and methods The study was performed in 178
people (mean age 53.9 ± 13.5), inhabitants of a village of
Boguszyce. The diastolic function of left ventricle was assessed
by echocardiography, by various parameters including
E and A waves, E/A ratio, deceleration time (DT) of
E (DTE) and isovolumetric relaxation time (IVRT). In
the studied population BMI, systolic and diastolic arterial
pressures, lipid parameters and ankle/arm index were
measured.
Results In a model of logistic regression analysis it was
revealed that independent risk factors of diastolic left ventricle
dysfunction incidence are: age (OR: 1.11; p < 0.01),
high BMI (OR: 1.02; p < 0.05), arterial hypertension (OR:
1.61; p < 0.05), low ankle–brachial index (OR: 0.52; p <
0.05) and low HDL cholesterol level (OR: 0.98; p < 0.05).
Conclusion We conclude that diastolic dysfunction of the
left ventricle is more common in patients with multiple
risk factors of cardiovascular diseases
Dyslipidemias in healthy individuals and patients with cardiovascular diseases in rural population
Wstęp Choroby serca i naczyń są w Polsce główną
przyczyną zgonów. Do głównych czynników ryzyka
chorób układu krążenia zalicza się: zaburzenia lipidowe,
palenie tytoniu, nadciśnienie tętnicze, cukrzycę,
otyłość oraz czynniki psychospołeczne. Celem
pracy była ocena częstości występowania zaburzeń
gospodarki lipidowej w populacji wiejskiej.
Materiał i metody Badaniami objęto 178 osób (118
kobiet i 60 mężczyzn) w wieku 19-84 lat (śr. wieku:
53,9 ± 13,5) ze wsi Boguszyce w powiecie oleśnickim
(koło Wrocławia) na Dolnym Śląsku. Wykonywane
badania biochemiczne obejmowały oznaczenia
cholesterolu całkowitego, cholesterolu frakcji
HDL oraz trójglicerydów w surowicy. Oznaczeń dokonywano
w laboratorium Katedry i Kliniki Chorób Wewnętrznych, Zawodowych i Nadciśnienia Tętniczego
Akademii Medycznej we Wrocławiu. Stężenie
cholesterolu frakcji LDL obliczano za pomocą wzoru
Friedewalda. Badania przeprowadzono w ramach
studenckiego obozu naukowego w dniach 3-14
września 2005 roku.
Wyniki Średnie stężenie cholesterolu całkowitego
w próbie wyniosło 193,97 ± 38,59 mg/dl. Stężenie cholesterolu
całkowitego korelowało dodatnio z wiekiem
(r = 0,53, p = 0,027). Częstość występowania podwyższonego
stężenia cholesterolu całkowitego
(≥ 190 mg/dl) w badanej populacji wyniosła 51,47%.
Znacznie podwyższone stężenie tego czynnika
(≥ 320 mg/dl) stwierdzono u 5,12% badanych. Średnie
stężenie cholesterolu frakcji LDL w próbie wyniosło
112,13 ± 34,50 mg/dl. U 44,85% badanych stwierdzono
podwyższone stężenie cholesterolu frakcji LDL
(≥ 115 mg/dl). Średnie stężenie cholesterolu frakcji
HDL w próbie wyniosło 55,42 ± 14,35 mg/dl. Obniżone
stężenie cholesterolu frakcji HDL (< 46 mg/dl
u kobiet oraz < 40 mg/dl u mężczyzn) zanotowano
u 25,45% badanych. Średnie stężenie trójglicerydów
w próbie wyniosło 132,08 ± 82,08 mg/dl. Wszystkie
dyslipidemie występowały istotnie częściej u chorych
z rozpoznanymi chorobami układu krążenia.
Wnioski W badanej populacji wiejskiej częstość zaburzeń
lipidowych jest większa niż w całej populacji
Polski. Wynik ten może świadczyć o nieprawidłowym
odżywianiu się i niewłaściwym stylu życia.
Większa częstość zaburzeń lipidowych u chorych
z rozpoznanymi chorobami układu krążenia wskazuje
na niedostateczne leczenie tych schorzeń i może
świadczyć o gorszym dostępie do lekarzy pierwszego

of death in European Union countries. Many determinants
of global cardiovascular risk have been identified. Modifiable
risk factors include: arterial hypertension,
dyslipidemia, obesity, diabetes mellitus and tobacco smoking.
A group of non-modifiable factors includes: age, gender,
family history of the cardiovascular disease and formerly
diagnosed heart and/or vascular diseases. The aim
of the study was to estimate the frequency of lipid disorders
in rural population.
Material and methods The study group consisted of 178
inhabitants of the village of Boguszyce (118 women and 60
men, aged 19-84; mean age 53.9 ± 13.5) in Olesnica district
(in the neighbourhood of the city of Wroclaw) in
Lower Silesia. Studies were carried out by trained students
of medical faculty during summer scientific meetings. Serum
levels of total cholesterol, HDL cholesterol and
triglycerides were assessed.Results Mean total cholesterol concentration was 193.97
± 38.59 mg/dl. Mean cholesterol level correlated positively
with age (r = 0.53, p = 0.027). The frequency of increased
total cholesterol level (≥ 190 mg/dl) in the study population
was 51.47%. The excessive increase of cholesterol level
(≥ 320 mg/dl) was detected in 5.12% of studied individuals.
Mean LDL cholesterol level was 112.13 ± 34.50 mg/dl. In
44.85% of the study group the increased concentration of
LDL cholesterol was noted (exceeding ≥ 115 mg/dl).
Mean HDL cholesterol concentration was 55.42 ± 14.35
mg/dl. The decreased HDL concentration (< 46 mg/dl for
women and < 40 mg/dl for men) was present in 25.45% of
subjects. Mean triglycerides concentration was 132.08 ±
82.08 mg/dl. All dyslipidemias were more common in patients
with known cardiovascular disroders.
Conclusions In the studied rural population lipid disturbances
are more common than in general population of
Poland. It may suggest that village dwellers are more prone
to unhealthy lifestyle with improper diet. Higher prevalence
of dyslipidemias in subjects with known cardiovascular
diseases points out inadequate treatment and possibly

Long-term bio-functional performance of a novel, self-positioning balloon expandable transcatheter biological aortic valve system in the ovine aortic banding model
Background: The aim of the study was to evaluate bio-functionality of a novel, proprietary balloon-expandable biological transcatheter aortic valve implantation (TAVI) system (InFlow, CardValve Consortium, Poland) in an ovine model of aortic banding.
Methods: Surgical ascending aorta banding was created in 21 sheep. Two weeks later, 18 biological valves were implanted within the model using 15–16 F InFlow TAVI systems and carotid cut-down approach. Follow-up transthoracic echocardiography was performed at 30, 90, and 180-day. At designated time, animals were euthanized and valves harvested for analysis.
Results: All sheep survived the banding procedure. There were 4 (22%) procedure related deaths within a 7-day period. During the observation an additional 2 sheep died. In one, the valve dislocated after the procedure — the animal was excluded. Two animals completed 30-day follow up, five 90-day follow-up and four terminal follow-up of 180 days. Valves examined via transesophageal echocardiography showed proper hemodynamic parameters without evidence of structural valve deterioration. The maximum and average flow gradients at 180 days were 31.4 (23.3–37.7) and 17.5 (13.1–20.2) mmHg, respectively. There was one case of moderate insufficiency and no case of perivalvular leaks. By histopathology, there were no inflammation, thrombosis, nor calcifications in any tested valves at long-term follow-up. Neointimal coverage of stent struts increased with time from basal part in “early” groups to nearly 3/4 of stent length in the 180-day group. The pannus tissue showed maturation that increased with time with no stenotic “collar” visible in orthotopically implanted valves.
Conclusions: The study showed good hemodynamic performance, durability and biocompatibility of the novel biological THV
Characteristics of the heart failure population in Poland: ZOPAN, a multicentre national programme
Background: It is difficult to define the optimal management of elderly heart failure (HF) patients with complex comorbidities.
Thus, comprehensive characterisation of HF patients constitutes a crucial pre-condition for the successful management
of this fragile population.
Aim: To analyse the ‘real life’ HF patients, including the evaluation of their health conditions, management and their use of
public health resources.
Methods and results: We examined 822 consecutive patients diagnosed with HF in NYHA classes II–IV in primary care
practices. The mean age was 68.5 years, and 56% were male. Only 23% of the patients who were of pre-retirement age
remained professionally active. Ischaemic or hypertension aetiology was found in 90% of participants. Nearly all patients had
multiple comorbidities. Most patients received converting enzyme inhibitors (88%) and beta-blockers (77%), 60% of them
both, although dosing was frequently inadequate. During the six months preceding the study, 31% had cardiovascular hospitalisation
and 66% required unscheduled surgery visits.
Conclusions: The real life HF population differs from trial populations. Most of the real life patients who had not yet reached
retirement age were professionally inactive, mainly due to a disability caused by cardiovascular conditions. Moreover, extremely
few participants were free from any comorbidity. Compared to 20th century Polish data, there has been an improvement
in the overall quality of HF-recommended pharmacotherapy. It must be stressed, however, that the percentage of
those on optimal dosage remains unsatisfactory.

wieloma chorobami współistniejącymi, jest zagadnieniem szczególnie trudnym. Dlatego też podstawą skutecznego postępowania
w tej grupie osób jest dokładna charakterystyka populacji.
Cel: Celem pracy była analiza charakterystyki chorych z HF na podstawie codziennej praktyki lekarskiej, z oceną ich stanu
zdrowia, jakości opieki oraz stopnia obciążenia dla systemu opieki zdrowotnej.
Metody i wyniki: Do badania włączono 822 kolejnych pacjentów ambulatoryjnych z HF w II-IV klasie NYHA z ośrodków
podstawowej opieki zdrowotnej. Średni wiek badanej grupy wynosił 68,5 roku, 56% stanowili mężczyźni, a 23% pacjentów
w okresie przedemerytalnym pozostawała aktywna zawodowo. Etiologia niedokrwienna lub nadciśnienie tętnicze było wskazywane
jako przyczyna HF w 90% przypadków. Choroby współistniejące stwierdzano u prawie wszystkich badanych osób.
Większość pacjentów otrzymywała inhibitory enzymu konwertującego angiotensynę (88%), a 77% - beta-adrenolityki,
z czego 60% chorych przyjmowało połączenie tych dwóch leków, przy czym ich dawkowanie było niższe od zalecanego.
W okresie 6 miesięcy poprzedzających badanie 31% osób było hospitalizowanych z przyczyn sercowo-naczyniowych, a 66%
wymagało dodatkowych, nieplanowanych wizyt lekarskich.
Wnioski: Rzeczywisty obraz populacji chorych z HF różni się od opisywanego w badaniach klinicznych. Większość pacjentów
w okresie przedemerytalnym była nieaktywna zawodowo, przede wszystkim z powodu chorób sercowo-naczyniowych.
Dodatkowo brak chorób współistniejących stwierdzano jedynie u niewielkiego odsetka osób. W porównaniu z poprzednimi
danymi w przypadku populacji polskiej poprawiła się jakość opieki nad chorymi z HF w odniesieniu do stosowanej farmakoterapii,
nadal jednak dawkowanie leków odbiega od standardów.
