    Ancorado numa investiga√ß√£o mais alargada sobre os efeitos do trabalho por turnos, o presente trabalho prop√Ķe-se contribuir para a discuss√£o de possibilidades de interven√ß√£o nesta problem√°tica, considerando especialmente o papel do contexto organizacional. Tal discuss√£o √© feita a partir da refer√™ncia a tr√™s estudos realizados em empresas industriais, nos quais, foram avaliados alguns dos efeitos tipicamente associados ao trabalho por turnos na sa√ļde, vida familiar e social e avalia√ß√£o da satisfa√ß√£o com o pr√≥prio turno de trabalho. No conjunto das tr√™s empresas estiveram envolvidos 859 trabalhadores/as por turnos. Os dados foram recolhidos atrav√©s de question√°rio, integrando este escalas publicadas na literatura ou desenvolvidas pela equipa de investiga√ß√£o. As an√°lises de correla√ß√Ķes efetuadas indicaram uma associa√ß√£o estatisticamente significativa e no sentido esperado entre a perce√ß√£o do suporte do contexto organizacional e os efeitos avaliados. Especificamente, tal associa√ß√£o oscilou entre -.19 com os ‚Äúproblemas de sono‚ÄĚ e .47 com a interface com a ‚Äúvida fora da empresa‚ÄĚ. Al√©m da relev√Ęncia das pr√°ticas de gest√£o que podem ser usadas pelas organiza√ß√Ķes, s√£o apresentadas/discutidas outras possibilidades de interven√ß√£o. Em ambos os casos, o enfoque permanece no suporte que pode ser proporcionado pelo contexto organizacional na promo√ß√£o da adapta√ß√£o a esta modalidade hor√°ria de trabalho.Anchored on a wider research on the shiftwork effects, this study aims to contribute to the discussion of intervention possibilities in this issue considering, in particular, the role of the organizational context. This discussion is made with reference to three studies conducted in industrial companies which assessed some of the effects that are typically associated with shiftwork: effects on health, family and social life and satisfaction with the work shift. In the three companies 859 shift workers were involved. Data were gathered from a survey (the scales used in the survey included scales published in the literature and other developed by the research team). The correlation analysis indicated a statistically significant association between the perception of organizational support and the assessed effects. This association varied between -.19 with ‚Äúsleep problems‚ÄĚ and .47 with the interface with ‚Äúlife outside the company‚ÄĚ. Beyond the relevance of management practices that can be used by organizations, other possibilities of intervention are presented/discussed. In both cases, the focus remains on the support that may be provided by the organizational context by promoting the adaptation to this work schedule

    Estudo da estrutura muco-ciliar antes e após o tratamento termal

    Termalismo √© definido como o uso da √°gua mineral natural e de outros meios complementares para fins de preven√ß√£o, terap√™utica, reabilita√ß√£o ou bem-estar. As √°guas sulf√ļricas s√£o as mais adequadas no tratamento das vias respirat√≥rias superiores, tendo a√ß√Ķes anti-inflamat√≥rias, antioxidantes e dessensibilizantes. Atuam favoravelmente nas mucosas, aliviando a dor, a obstru√ß√£o nasal e a congest√£o nasal, diminuindo a intensidade e dura√ß√£o das agudiza√ß√Ķes e evitando a evolu√ß√£o para a cronicidade destas patologias otorrinolaringol√≥gicas. Em estudos anteriores √°reas hipercrom√°ticas foram encontradas no citoplasma das c√©lulas ciliadas nasais - estrias supranucleares hipercrom√°ticas - em maior abund√Ęncia em c√©lulas ciliadas nasais de pessoas saud√°veis e em menos abund√Ęncia em pessoas com patologia otorrinolaringol√≥gica subjacente. O presente estudo tem como principal objetivo estudar a altera√ß√£o da estrutura e depura√ß√£o mucociliar antes e ap√≥s o tratamento termal. Tem ainda como finalidade avaliar se estas altera√ß√Ķes alteram favoravelmente a sintomatologia nasal. Para tal, foram estudados 49 doentes das termas de Unhais da Serra. A todos foi feita por n√≥s uma hist√≥ria cl√≠nica orientada para a √°rea otorrinolaringol√≥gica, efetuada colheita de exsudado nasal atrav√©s de citologia nasal e aplicado o teste de tr√Ęnsito de sacarina, antes e ap√≥s todo o tratamento termal. Os doentes, com m√©dia de 12,4 dias de tratamento termal, tiveram um aumento na mediana das c√©lulas ciliadas nasais com estrias supranucleares hipercrom√°ticas, de 2 para 3 (p=0.130). Houve melhoria na depura√ß√£o mucociliar, traduzindo-se por uma diminui√ß√£o do teste de tr√Ęnsito de sacarina de, em m√©dia, 6.81 minutos ap√≥s o tratamento termal (intervalo de confian√ßa 95%: 4.88 a 8.74 min) (p<0.05). Houve melhoria sintom√°tica, que √© referida por 100% dos indiv√≠duos da nossa amostra. Com este estudo, conclu√≠mos que a √°gua termal ajuda na limpeza do muco, diminui os mediadores inflamat√≥rios, aliviando a dor e a sensa√ß√£o de obstru√ß√£o nasal. Assim, h√° uma depura√ß√£o mucociliar mais eficaz, no que se traduz num tempo de transporte mucociliar mais curto, n√£o havendo uma varia√ß√£o marcante na contagem de c√©lulas ciliadas nasais com estrias supranucleares hipercrom√°ticas ap√≥s os tratamentos.Hydrotherapy is defined as the use of natural mineral water and other complementary means for prevention, treatment, rehabilitation or welfare. Sulfuric waters are most suitable in the treatment of the upper airway, having anti-inflammatory, antioxidant and desensitizing actions. They act favorably on mucous membranes, relieving pain, nasal obstruction and nasal congestion, decreasing the intensity and duration of acute exacerbations and preventing progression to chronicity of these otorhinolaryngological diseases. In previous studies hyperchromatic areas were found in the cytoplasm of ciliated nasal cells - hyperchromatic supranuclear stria - in greater abundance in nasal ciliated cells of healthy people and in less abundance in people with underlying otorhinolaryngological pathology. This study has as main objective to analyze the change in the structure and mucociliary clearance before and after thermal treatment. It also aimed to evaluate whether these changes alter favorably the nasal symptoms. To this end, we studied 49 patients from Unhais da Serra thermal. We conducted a clinical history oriented to the otorhinolaryngological area, we also collected nasal discharge, through nasal cytology, and applied the saccharin traffic test before and after thermal treatment. The patients, with an average of 12.4 days of thermal treatment, had an increase in the median of the nasal ciliated cells with hyperchromatic supranuclear stria , from 2 to 3 (p = 0.130). There was improvement in mucociliary clearance, resulting in a decrease in saccharin traffic test, on average, 6.81 minutes after thermal treatment (95% confidence interval: 4.88 to 8.74 min) (p <0,05). 100% of patients from our sample referred that there was symptomatic improvement. This study concluded that thermal water helps mucus cleansing and decreases inflammatory mediators, relieving pain and the sensation of nasal obstruction. Thus, there is a more effective mucociliary clearance which results in a decrease of mucociliary transport time, and no remarkable variation in the count of nasal ciliated cells with hyperchromatic supranuclear stria, after treatments

    Enquadramento: A efic√°cia das terapias assistidas por animais em algumas patologias tem vindo a ser questionada ao longo dos anos. Apesar de ser uma terapia pouco conhecida em Portugal, hoje em dia existem muitos indiv√≠duos de diferentes faixas et√°rias a recorrer a este m√©todo terap√™utico. Esta terapia √© ben√©fica na melhoria da qualidade de vida dos indiv√≠duos e intervindo ao n√≠vel f√≠sico, social e emocional. Objetivo: Verificar se as terapias assistidas por cavalos s√£o eficazes no desenvolvimento de habilidades cognitivas e comportamentais de crian√ßas diagnosticadas com perturba√ß√£o de hiperatividade/d√©fice de aten√ß√£o. M√©todo: A amostra compreende dois sujeitos do sexo feminino, com 6 e 8 anos de idade, pacientes de uma empresa de equita√ß√£o terap√™utica. Neste estudo foram implementados √†s crian√ßas os subtestes informa√ß√£o, semelhan√ßas, vocabul√°rio, compreens√£o e mem√≥ria de d√≠gitos da Escala de Intelig√™ncia de Weschler para Crian√ßas ‚Äď 3¬ļ Edi√ß√£o (WISC-III) e aos pais o teste MTA SNAP-IV-pt-PT aplicados em dois momentos, com dois meses de intervalo recorrendo a uma metodologia descritiva decorrida durante dois meses entre os dois momentos de avalia√ß√£o. Resultados: Verificou-se uma diferen√ßa significativa nos resultados do teste-reteste das crian√ßas. A pontua√ß√£o foi sempre maior ou igual no reteste, exceto no subteste mem√≥ria de d√≠gitos. Em rela√ß√£o aos problemas comportamentais, os pais referiram a exist√™ncia duma melhoria significativa. Conclus√£o: Com esta investiga√ß√£o pode concluir-se que as crian√ßas deste estudo evolu√≠ram de forma positiva, tendo diminu√≠do significativamente os sintomas ligados √† hiperatividade e aos problemas comportamentais identificados pelos pais. Apesar de ser um estudo de caso, os nossos resultados apontam para altera√ß√Ķes positivas no comportamento das crian√ßas, assim como uma evolu√ß√£o significativa nas habilidades cognitivas evidenciadas pelas crian√ßas. / Background: The effectiveness of animal-assisted therapies in some pathologies has been questioned over the years. Despite being a little-know therapy in Portugal, today there are many individuals of different age groups using this therapeutic method. This therapy is beneficial in improving the quality of life of individuals and intervening at the physical, social and emotional levels. Objective: Verifying whether horses-assisted therapies are effective in developing cognitive skills and reducing symptoms associated to behavioral disorders in children diagnosed with attention deficit/hyperactivity disorder. Method: The sample consisted of 2 girls aged 6 and 8 years, patients of an equine assisted therapy centre. In this study we implemented the subtests information, similarities, vocabulary, understanding and digit memory of the Weschler Intelligence Scale for Children ‚Äď 3¬ļ Edition (WISC-III) to the children and the MTA SNAP-IV-pt-PT test to the parents. Both children and parents were tested twice, two months apart. Were implemented using a descriptive methodology elapsed for two months between the two moments of evaluation. Results: we verified a significant difference in the children‚Äôs test-retest results. The scores obtained were always higher or equal in the retest, with the exception of the digit memory subtest. In what regards behavioral disorders, parents reported a significant improvement in their children‚Äôs behavior. Conclusion: With this investigation it can be concluded that the children‚Äôs in this study evolved positively, having significantly decreased the symptoms associated with attention deficit hyperactivity disorder and behavioral disorders identified by the parents.     First description of agonistic behaviour in Chondrostoma polylepis (Pisces: Cyprinidae) with notes on the behaviour of other Chondrostoma species

    In this paper we describe for the first time the occurrence of agonistic behaviour in Chondrostoma polylepis (Pisces: Cyprinidae) in captivity. In this species agonistic behaviour was present in adults of both sexes and juveniles, during spring and summer, well after the end of the breeding season. Qualitative observations revealed the presence of a similar pattern of agonistic behaviour in Chondrostoma duriensis. Despite intense observation effort, no signs of agonistic behaviour were detected in three other Chondrostoma species of the same geographical area, C. macrolepidotus, C. lemmingii and C. lusitanicum. It is hypothesized that agonistic behaviour in C. polylepis and C. duriensis may be functionally linked to their feeding ecology

    Prognostic factors for early relapse in non-metastatic triple negative breast cancer ‚ÄĒ real world data

    BACKGROUND: Triple negative breast cancer (TNBC) has the worst prognosis amongst all subtypes. Studies have shown that the achievement of pathologic complete response in the breast and axilla correlates with improved survival. The aim of this study was to identify clinical or pathological features of real-life TNBC patients with a higher risk of early relapse. MATERIALS AND METHODS: Single-centre retrospective analysis of 127 women with TNBC, stage II-III, submitted to neoadjuvant treatment and surgery between January 2016 and 2020. Multivariate Cox regression analysis for disease free survival (DFS) at 2 years was performed and statistically significant variables were computed into a prognostic model for early relapse. RESULTS: After 29 months of median follow-up, 105 patients (82.7%) were alive and, in total, 38 patients (29.9%) experienced recurrence. The 2-year DFS was 73% (95% CI: 21.3‚Äď22.7). In multivariate analysis, being submitted to neoadjuvant radiotherapy [HR 2.8 (95% CI: 1.2‚Äď6.4), p = 0.017] and not achieving pathologic complete response [HR 0.3 (95% CI: 0.1‚Äď1.7), p = 0.011] were associated with higher risk of recurrence. In our prognostic model, the presence of at least one of these variables defined a subgroup of patients with a worse 2-year DFS than those without these features (59% vs. 90%, p &lt; 0.001, respectively).¬† CONCLUSIONS: In this real-life non-metastatic TNBC cohort, neoadjuvant radiotherapy (performed due to insufficient clinical response to neoadjuvant chemotherapy or significant toxicity) impacted as an independent prognostic factor for relapse along with the absence of pathologic complete response identifying a subgroup of higher risk patients for early relapse that might merit a closer follow-up

    SARS-CoV-2 introductions and early dynamics of the epidemic in Portugal

    Genomic surveillance of SARS-CoV-2 in Portugal was rapidly implemented by the National Institute of Health in the early stages of the COVID-19 epidemic, in collaboration with more than 50 laboratories distributed nationwide. Methods By applying recent phylodynamic models that allow integration of individual-based travel history, we reconstructed and characterized the spatio-temporal dynamics of SARSCoV-2 introductions and early dissemination in Portugal. Results We detected at least 277 independent SARS-CoV-2 introductions, mostly from European countries (namely the United Kingdom, Spain, France, Italy, and Switzerland), which were consistent with the countries with the highest connectivity with Portugal. Although most introductions were estimated to have occurred during early March 2020, it is likely that SARS-CoV-2 was silently circulating in Portugal throughout February, before the first cases were confirmed. Conclusions Here we conclude that the earlier implementation of measures could have minimized the number of introductions and subsequent virus expansion in Portugal.
