    A Malignant Mass in the Breast Is Not Always Breast Cancer

    A 37-year-old woman presented to the Internal Medicine Clinic with complaints of abdominal pain and constipation which had begun 3 months earlier. A colonoscopy was performed, and wall thickening of the sigmoid colon was detected. A biopsy of the sigmoid colon revealed a poorly differentiated, mucin-producing adenocarcinoma with a signet-ring pattern. No distant metastasis was detected. The patient was treated with chemotherapy consisting of 5-fluorouracil, leucovorin, and oxaliplatin. One and a half years later, a painless mass, which was not fixed to the skin, measuring 1 cm in diameter, was found in the lower outer quadrant of the left breast. A core biopsy of the mass was performed, and a histopathological report confirmed metastasis to the breast from mucinous adenocarcinoma of an intestinal primary

    Study of Electron Identification in the OPERA Detector

    The OPERA experiment is designed to perform first direct observation of ╬Ż¤ä\nu_{\tau} appearance in an almost pure ╬Ż╬╝\nu_{\mu} beam. The OPERA detector is a hybrid setup which combines a lead/emulsion target with various electronic detectors. It is located in Gran Sasso Laboratory (LNGS), 730 km away from CERN where neutrino beam is produced. A good electron identification in the ECC brick would also allow OPERA to search for ╬Ż╬╝Ôćĺ╬Że\nu_{\mu} \to \nu_e oscillations. We have studied electron identification in the Emulsion Cloud Chamber (ECC) brick which was exposed to CERN SPS H4 electron beam. Emulsion scanning was performed in LNGS scanning laboratory. FEDRA framework was used for the data analysis. In total, we have found 30 electron showers in the brick. The characteristics of each shower have been studied. The background base-track contamination in the shower was estimated as 20 ┬▒\pm 4. This is mainly due to shower overlap of electrons and passing through cosmic rays

The research is designed as phenomenological qualitative research. Maximum diversity sampling of purposeful sampling methods was preferred in sample selection. The data were collected by face-to-face interview method with semi-structured interview form. Content analysis was used for data analysis. The collected data were coded to derive themes related to inductive understanding using a computer-based word processing program (NVIVO). Themes are shaped within the framework of codes. Students expressed that while the comfort and opportunities of the home environment are regarded as positive in the distance education process, the distracting stimuli in the home environment is stated to be negative as they cause motivation problems

    EVALUATION OF THE PLASMA MICRO RNA EXPRESSION LEVELS IN SECONDARY HEMOPHAGOCYTIC LYMPHOHISTIOCYTOSIS

    Background: Hemophagocytic lymphohistiocytosis (HLH) is a life threatening hyper inflammatory disease. Micro RNAs (miRNA) are about 22 nucleotide-long, small RNAs encoded with genes, and they have regulatory functions in immune response. Objective: To determine the miRNA expression levels of 11 secondary HLH patients, we evaluated the associations of miRNA levels with pathogenesis, clinical presentation, and prognosis of the disease. Patients and Methods: Patients who were diagnosed with secondary HLH from January 2011 to December 2012 were included in this study. We profiled the expressions of 379 miRNAs in plasma of both HLH patients and healthy controls. Patients were evaluated regarding with age, clinical findings, miRNA expresions, laboratory data, treatment, and prognosis, by using descriptive statistics. Results: A total of 11 secondary HLH patients and 11 healthy children were included in this study. miR-205-5p was expressed in all case and controls and expression level of miR-205-5p was found 6.21 fold higher than control group (p=0.01). We detected the second highest expression percent in miR-194-5p with 81% of cases and controls. Expression level of miR-194-5p was found to have 163 fold higher than controls (p= 0.009). miR-30c-5p showed 77% expression percent in cases and controls together. The expression level of this miRNA was detected 9 fold decreased in HLH patients compared to healthy children (p= 0.031). Conclusion: We showed that miR-205-5p, miR-194-5p and miR-30c-5p could be useful plasma biomarkers for HLH. Further research is needed in larger and homogenous study groups, especially for these miRNAs as biomarkers for HLH

    The Turkish experience with therapeutic plasma exchange: A national survey

    WOS: 000473249400014PubMed ID: 31036516Therapeutic plasma exchange (TPE) is used to treat more than 60 diseases worldwide and has drawn growing interest. Little is known about the current situation of TPE activity in Turkey, so we developed a survey to obtain information about this timely topic. We collected data on TPE from 28 apheresis units throughout Turkey. We performed a total of 24,912 TPE procedures with 3203 patients over the past decade. Twenty years ago, the majority of procedures were performed for neurological and hematological disorders, and today, most TPE procedures are done for the same reasons. The only historical change has been an increase in TPE procedures in renal conditions. Currently, renal conditions were more frequently an indication for TPE than rheumatic conditions. Fresh frozen plasma was the most frequently used replacement fluid, followed by 5% albumin, used in 57.9% and 34.6% of procedures, respectively. The most frequently used anticoagulants in TPE were ACD-A and heparin/ACD-A, used with 1671 (52.2%) and 1164 (36.4%) patients, respectively. The frequency of adverse events (AEs) was 12.6%. The most common AEs were hypocalcemia-related symptoms, hypotension, and urticaria. We encountered no severe AEs that led to severe morbidity and mortality. Overall, more than two thirds of the patients showed improvement in the underlying disease. Here, we report on a nationwide survey on TPE activity in Turkey. We conclude that there has been a great increase in apheresis science, and the number of TPE procedures conducted in Turkey has increased steadily over time. Finally, we would like to point out that our past experiences and published international guidelines were the most important tools in gaining expertise regarding TPE
