Characterising personal, household, and community PM2.5 exposure in one urban and two rural communities in China
Background
Cooking and heating in households contribute importantly to air pollution exposure worldwide. However, there is insufficient investigation of measured fine particulate matter (PM2.5) exposure levels, variability, seasonality, and inter-spatial dynamics associated with these behaviours.
Methods
We undertook parallel measurements of personal, household (kitchen and living room), and community PM2.5 in summer (May–September 2017) and winter (November 2017-Janauary 2018) in 477 participants from one urban and two rural communities in China. After stringent data cleaning, there were 67,326–80,980 person-hours (ntotal = 441; nsummer = 384; nwinter = 364; 307 had repeated PM2.5 data in both seasons) of processed data per microenvironment. Age- and sex-adjusted geometric means of PM2.5 were calculated by key participant characteristics, overall and by season. Spearman correlation coefficients between PM2.5 levels across different microenvironments were computed.
Findings
Overall, 26.4 % reported use of solid fuel for both cooking and heating. Solid fuel users had 92 % higher personal and kitchen 24-h average PM2.5 exposure than clean fuel users. Similarly, they also had a greater increase (83 % vs 26 %) in personal and household PM2.5 from summer to winter, whereas community levels of PM2.5 were 2–4 times higher in winter across different fuel categories. Compared with clean fuel users, solid fuel users had markedly higher weighted annual average PM2.5 exposure at personal (78.2 [95 % CI 71.6–85.3] μg/m3 vs 41.6 [37.3–46.5] μg/m3), kitchen (102.4 [90.4–116.0] μg/m3 vs 52.3 [44.8–61.2] μg/m3) and living room (62.1 [57.3–67.3] μg/m3 vs 41.0 [37.1–45.3] μg/m3) microenvironments. There was a remarkable diurnal variability in PM2.5 exposure among the participants, with 5-min moving average from 10 μg/m3 to 700–1200 μg/m3 across different microenvironments. Personal PM2.5 was moderately correlated with living room (Spearman r: 0.64–0.66) and kitchen (0.52–0.59) levels, but only weakly correlated with community levels, especially in summer (0.15–0.34) and among solid fuel users (0.11–0.31).
Conclusion
Solid fuel use for cooking and heating was associated with substantially higher personal and household PM2.5 exposure than clean fuel users. Household PM2.5 appeared a better proxy of personal exposure than community PM2.5

Similarly, they also had a greater increase (∼75% vs ∼20%) in personal and household PM2.5 from summer to winter, whereas community levels of PM2.5 were 2-3 times higher in winter regardless of fuel use. Compared with clean fuel users, solid fuel users had markedly higher weighted annual average PM2.5 exposure at personal (77.8 [95% CI 71.1-85.2] vs ∼40 µg/m3), kitchen (103.7 [91.5-117.6] vs ∼50 µg/m3) and living room (62.0 [57.1-67.4] vs ∼40 µg/m3) microenvironments. There was a remarkable diurnal variability in PM2.5 exposure among the participants, with 5-minute moving average 700-1,200µg/m3 in typical meal times. Personal PM2.5 was moderately correlated with living room (Spearman r: 0.64-0.66) and kitchen (0.52-0.59) levels, but only weakly correlated with community levels, especially in summer (0.15-0.34) and among solid fuel users (0.11-0.31).

Identification of piRNA Targets in Urinary Extracellular Vesicles for the Diagnosis of Prostate Cancer
Emerging studies demonstrate that PIWI-interacting RNAs (piRNAs) are associated with various human cancers. This study aimed to evaluate the urinary extracellular vesicles (EVs) piRNAs as non-invasive biomarkers for prostate cancer (PCa) diagnosis. RNA was extracted from urinary EVs from five PCa patients and five healthy controls (HC), and the piRNAs were analyzed by small RNA sequencing. Dysregulated piRNAs were identified and then validated in another 30 PCa patients and 10 HC by reverse-transcription polymerase chain reaction (RT-qPCR). The expressions of novel_pir349843, novel_pir382289, novel_pir158533, and hsa_piR_002468 in urinary EVs were significantly increased in the PCa group compared with the HC group. The area under the curve (AUC) of novel_pir158533, novel_pir349843, novel_pir382289, hsa_piR_002468, and the combination of the four piRNA in PCa diagnosis was 0.723, 0.757, 0.777, 0.783, and 0.853, respectively. After the RNAhybrid program analysis, all four piRNAs had multiple potential binding sites with key mRNAs in PTEN/PI3K/Akt, Wnt/beta-catenin, or androgen receptor pathway, which are critical in PCa development and progression. In conclusion, our findings indicate that specific piRNAs in urinary EVs may serve as non-invasive diagnostic biomarkers for PCa
Bleeding post-transplantation intrarenal pseudoaneurysms
We herein present a case of an intrarenal pseudoaneurysm developing 2 weeks after renal transplantation. The patient presented with hemorrhagic shock. Computed tomography confirmed an intrarenal pseudoaneurysm with extravasation of contrast and a large surrounding hematoma. Angiography confirmed the pseudoaneurysm, and embolization of the segmental graft renal artery was performed. In view of the uncertain etiology and the possibility of a mycotic pseudoaneurysm, we administered empiric antimicrobial therapy. The clinical course, investigations, and management are described. Finally, a review of the literature regarding post-transplantation pseudoaneurysms is presented.
以下是一宗於移植後 2 週出現的腎內僞動脈瘤個案,患者以出血性休克表現。當時為其安排緊急電腦斷層攝影 (CT),發現有一個腎內僞動脈瘤,並呈現顯影劑溢出現象,且被一大型血腫圍繞。緊急血管攝影證實為植入腎之僞動脈瘤,同時我們對涉及的節段性動脈予以栓塞處置。基於病因並不明確,而且不能排除黴菌性僞動脈瘤的可能性,因此我們實施了經驗性抗微生物學療法。在本文中,我們描述了本個案的臨床歷程、相關檢查及後續處置,並指出我們在治療上所遭遇到的困難。最後,我們對移植後僞動脈瘤的相關文獻作出了回顧
Conversion from Recombinant Human Erythropoietin to Once Every 4 Weeks Darbepoetin Alfa for Treatment of Renal Anemia in CAPD Patients
BackgroundDarbepoetin alfa is a new erythropoietic protein with a three-fold longer half-life than recombinant human erythropoietin (rHuEPO), allowing for an extended dosing interval of once every 2 weeks in patients with chronic renal failure. The objective of this study was to investigate the possibility of further extending the dose interval of this erythropoietic agent to once every 4 weeks in the treatment of renal anemia in dialysis patients.MethodsA prospective study was carried out in 14 continuous ambulatory peritoneal dialysis (CAPD) patients stably maintained on subcutaneous rHuEPO with hemoglobin level of 10–13 g/dL. They were switched to subcutaneous darbepoetin alfa administered once every 4 weeks for a period of 24 weeks. The starting dose was 40 [.proportional]g. The dose of darbepoetin alfa was then adjusted to maintain a target hemoglobin level between 10 and 13 g/dL. When darbepoetin alfa was increased by 100%, the dosing interval was shortened to maintain the target hemoglobin. Evaluation was done during the last 4 weeks.ResultsOf the 14 patients recruited, 11 patients completed the study. Of these 11 patients, 9 (82%) successfully maintained the target hemoglobin with once every 4 weeks darbepoetin alfa. For those successful patients, the mean hemoglobin level during the evaluation period was 11.13 ± 2.04 g/dL (mean ± standard deviation), and the mean change in hemoglobin level from baseline was −1.03 g/dL (95% CI: −2.34, 0.27). The mean weekly darbepoetin alfa dose requirement during the evaluation period was 12.33 ± 4.80 [.proportional]g/week, and the mean change in weekly dose from baseline was +2.33 [.proportional]g/week (95% CI: −1.35, 6.02). No serious adverse event related to darbepoetin alfa occurred during the study.ConclusionDarbepoetin alfa administered once every 4 weeks effectively maintained hemoglobin level in most CAPD patients after conversion from previously stabilized rHuEPO treatment. Darbepoetin alfa is safe and well tolerated, allowing for less frequent dosing. [Hong Kong J Nephrol 2007;9(2):77–81
Pneumocystis carinii Pneumonia Among Renal Transplant Recipients Despite Antibiotic Prophylaxis
Pneumocystis carinii pneumonia (PCP) is a well-known opportunistic infection in renal transplant recipients; it is associated with high mortality, mostly within the first 6 months post-transplantation. The disease has been effectively prevented by routine antibiotic prophylaxis. Recently, however, we encountered three consecutive cases of PCP; one developed the disease at 8 months and another at 11 months post-transplantation. An overall assessment of a patient's degree of immunosuppression is essential when considering the duration of PCP prophylaxis. Instead of the routine regimen of 6 months, 1-year PCP prophylaxis may be required for those who are on both tacrolimus and mycophenolate mofetil

The establishment of kidney cancer organoid line in drug testing
Abstract Introduction Kidney cancer is a common urological malignancy worldwide with an increasing incidence in recent years. Among all subtypes, renal cell carcinoma (RCC) represents the most predominant malignancy in kidney. Clinicians faced a major challenge to select the most effective and suitable treatment regime for patients from a wide range of modalities, despite improved understanding and diagnosis of RCC. Objective Recently, organoid culture gained more interest as the 3D model is shown to be highly patient specific which is hypothetically beneficial to the investigation of precision medicine. Nonetheless, the development and application of organotypic culture in RCC is still immature, therefore, the primary objective of this study was to establish an organoid model for RCC. Materials and Methods Patients diagnosed with renal tumor and underwent surgical intervention were recruited. RCC specimen was collected and derived into organoids. Derived organoids were validated by histological examminations, sequencing and xenograft. Drug response of organoids were compared with resistance cell line and patients' clinical outcomes. Results Our results demonstrated that organoids could be successfully derived from renal tumor and they exhibited high concordance in terms of immunoexpressional patterns. Sequencing results also depicted concordant mutations of driver genes in both organoids and parental tumor tissues. Critical and novel growth factors were discovered during the establishment of organoid model. Besides, organoids derived from renal tumor exhibited tumorigenic properties in vivo. In addition, organoids recapitulated patient's in vivo drug resistance and served as a platform to predict responsiveness of other therapeutic agents. Conclusion Our RCC organoid model recaptiluated histological and genetic features observed in primary tumors. It also served as a potential platform in drug screening for RCC patients, though future studies are necessary before translating the outcomes into clinical practices
Urinary Cell-Free DNA in Bladder Cancer Detection
Urinary bladder cancer is a common urological cancer. Although flexible cystoscopy is widely employed in bladder cancer detection, it is expensive, invasive, and uncomfortable to the patients. Recently, urinary cell-free DNA (ucfDNA) isolated from urine supernatant has been shown to have great potential in bladder cancer detection and surveillance. Molecular features, such as integrity and concentration of ucfDNA, have been shown to be useful for differentiating bladder cancer patients from healthy controls. Besides, bladder cancer also exhibits unique genetic features that can be identified from sequencing and expression of ucfDNA. Apart from bladder cancer detection, ucfDNA is also useful for molecular classification. For example, ucfDNA exhibits significant differences, both molecularly and genetically, in non-muscle-invasive and muscle-invasive bladder cancers. There is no doubt that ucfDNA is a very promising tool for future applications in the field of bladder cancer
