    Current Situation and Perspectives of Clinical Study in Integrative Medicine in China

    Integrative medicine is not only an innovative China model in clinical practice, but also the bridge for TCM toward the world. In the past thirty years, great achievements have been made in integrative medicine researches, especially in clinical practice. The clinical achievements mainly include the following three: innovating methodology of disease-syndrome combination, excavating the classical theory in traditional Chinese medicine (TCM), preventing and curing refractory diseases. The development ideas and strategies of integrative medicine for future mainly include (a) standing on frontier field of international medicine and improving the capability of preventing and curing refractory diseases; (b) moving prevention and control strategy forward and improving the curative effect of common and frequent disease; (c) excavating the classical theory of TCM and broadening the treatment system of modern medicine; (d) improving the innovation level of new high effective drugs on the basis of classical prescriptions and herbs in TCM; (e) rerecognizing the theory of formula corresponding to syndrome in TCM and enhancing the level of clinical research evidence based on evidence-based medicine. Integrative medicine will do obtain greater achievements in creating new medicine and pharmacology and make more tremendous contributions for the great rejuvenation of the Chinese nation and human health care

    Control Strategy on Hypertension in Chinese Medicine

    Hypertension is a clinical common disease, with high mortality and disability. Although there have also been significant advances in therapeutic concepts and measures, it has shown a certain value and significance in the treatment of Chinese medicine. The control strategy on hypertension is described from the following aspects such as differentiation of symptoms, pathogenesis, formula syndrome, and herb syndrome. As the common clinical manifestations of hypertension are dizziness, headache, fatigue, lassitude in the loins and knees, and so on, the pathogeneses of them are analysed. The author found that the main pathogenesis of the disease is heat, excessive fluid, and deficiency, which occurred incorporatively and interacted with each other in patients. Although the pathogenesis of the disease is complicated, the distribution of formula syndromes and herb syndromes is regular. The common formula syndromes include Banxia Baishu Tianma Tang (Decoction of Pinellia ternata, Atractylodes and Gastrodia elata), Da Chaihu Tang (Major Bupleurum Decoction), and Liu Wei Dihuang Wan (Pill of Rehmannia). And the common herb syndromes include Tian Ma (Gastrodia elata) syndrome, Sheng Di Huang (Radix Rehmanniae) syndrome, Niu Xi (Achyranthes Root) syndrome, and Chuan Xiong (Ligusticum wallichii) syndrome

    Chinese Patent Medicine Tongxinluo Capsule for Hypertension: A Systematic Review of Randomised Controlled Trials

    Background. This study was intended to evaluate the efficacy and safety of Tongxinluo capsule for hypertension. Search Strategy. We searched the Cochrane Central Register of Controlled Trials (CENTRAL) in the Cochrane Library, The PubMed, EMBASE, Chinese Bio-Medical Literature Database (CBM), Chinese National Knowledge Infrastructure (CNKI), Chinese Scientific Journal Database, and Wan-fang Data started from the first of database to October 28, 2013. No language restriction was applied. We included randomized clinical trials testing Tongxinluo capsule against western medicine, Tongxinluo capsule versus placebo, and Tongxinluo capsule combined with western medicine versus western medicine. Study selection, data extraction, quality assessment, and data analyses were conducted according to the Cochrane standards. Results. 25 trials with 1958 participants were included. The methodological quality of the included trials was evaluated as generally low. The blood pressure (BP) lowering effect of Tongxinluo capsule plus western medicine was significantly higher than that of western medicine (systolic blood pressure (SBP): −3.87, −5.32 to −2.41, P<0.00001; and diastolic blood pressure (DBP): −2.72, −4.19 to −1.24, P=0.0003). The BP also decreased significantly from baseline with Tongxinluo capsule than placebo (SBP: −9.40, −10.90 to −7.90, P<0.00001; and DBP: −11.80, −12.40 to −11.20, P<0.00001) or western medicine (SBP: −3.90, −4.93 to −2.87, P<0.00001; and DBP: −3.70, −3.83 to −3.57, P<0.00001). 12 trials reported adverse events without details. Conclusions. There is some but weak evidence about the effectiveness of TXL in treating patients with hypertension

    Sanqi Panax Notoginseng Injection for Angina Pectoris

    Objective. To evaluate the clinical curative effects of SPN injection on AP. Methods. Six databases were systematically searched including Cochrane Central, PubMed, EMBASE, CBM, Chinese National Knowledge Infrastructure (CNKI), and VIP to identify randomized controlled trials (RCTs). We assessed the quality of included studies, extracted valid data, and undertook meta-analysis following the steps of systematic review recommended by the Cochrane group. Results. Ten moderate-to-low quality randomized controlled trials involving 969 patients were included. There was no evidence that SPN alone had better or worse effects than conventional drugs on improving clinical symptoms (RR 1.09, 95% CI 0.85 to 1.39) and ECG (RR 1.17, 95% CI 0.84 to 1.63). However, there was an evidence that SPN combined with western medications was a better treatment option than conventional drugs alone in improving clinical symptoms (RR 1.28, 95% CI 1.19 to 1.33) and ECG (RR 1.27, 95% CI 1.12 to 1.45). No serious adverse effects were reported. Conclusion. Compared with conventional treatment, SPN may show the potential of optimizing symptomatic outcomes. As a kind of alternative and complementary medicine, SPN may provide another choice for AP patients and further large-scale high-quality trials are needed to confirm this efficacy

    Traditional Chinese Medicine Syndromes for Essential Hypertension: A Literature Analysis of 13,272 Patients

    Background. To simplify traditional Chinese medicine syndrome differentiation and allow researchers to master syndrome differentiation for hypertension, this paper retrospectively studied the literature and analyzed syndrome elements corresponding to hypertension syndromes. Methods. Six databases including PubMed, EMBASE, Chinese Bio-Medical Literature Database, Chinese National Knowledge Infrastructure, Chinese Scientific Journal Database, and Wan-fang Data were searched from 1/January/2003 to 30/October/2013. We included all clinical literature testing hypertension syndromes and retrospectively studied the hypertension literature published from 2003 to 2013. Descriptive statistics calculated frequencies and percentages. Results. 13,272 patients with essential hypertension were included. Clinical features of hypertension could be attributed to 11 kinds of syndrome factors. Among them, seven syndrome factors were excess, while four syndrome factors were deficient. Syndrome targets were mainly in the liver and related to the kidney and spleen. There were 33 combination syndromes. Frequency of single-factor syndromes was 31.77% and frequency of two-factor syndromes was 62.26%. Conclusions. Excess syndrome factors of hypertension patients include yang hyperactivity, blood stasis, phlegm turbidity, internal dampness, and internal fire. Deficient syndrome factors of hypertension patients are yin deficiency and yang deficiency. Yin deficiency with yang hyperactivity, phlegm-dampness retention, and deficiency of both yin and yang were the three most common syndromes in clinical combination

    Detection and localization of continuous gravitational waves with pulsar timing arrays: the role of pulsar terms

    A pulsar timing array is a Galactic-scale detector of nanohertz gravitational waves (GWs). Its target signals contain two components: the `Earth term' and the `pulsar term' corresponding to GWs incident on the Earth and pulsar respectively. In this work we present a Frequentist method for the detection and localization of continuous waves that takes into account the pulsar term and is significantly faster than existing methods. We investigate the role of pulsar terms by comparing a full-signal search with an Earth-term-only search for non-evolving black hole binaries. By applying the method to synthetic data sets, we find that (i) a full-signal search can slightly improve the detection probability (by about five percent); (ii) sky localization is biased if only Earth terms are searched for and the inclusion of pulsar terms is critical to remove such a bias; (iii) in the case of strong detections (with signal-to-noise ratio \gtrsim 30), it may be possible to improve pulsar distance estimation through GW measurements.Comment: 12 pages, 9 figures, typos corrected.     Effectiveness of Stimulation of Acupoint KI 1 by Artemisia vulgaris

    Objective. A systematic review of randomized controlled trials has been performed to assess the effectiveness of stimulation of acupoint KI 1 by Artemisia vulgaris (the Japanese name is moxa) to lower blood pressure compared to antihypertensive drugs. Methods and Findings. Articles published from 1980 to August 2013 in databases of CENTRAL, Pubmed, CBM, CNKI, VIP, and online clinical trial registry websites were searched. Studies included were randomized controlled trials (RCTs); moxibustion-type intervention on KI 1 compared with antihypertensive drugs; meta-analysis showed superior effects of moxibustion plus antihypertensive drugs on systolic blood pressure (WMD: −4.91 [−7.54, −2.28]; P=0.0003) but no superior effects on diastolic blood pressure (WMD: −6.38 [−17.17, 4.41]; P=0.25). Conclusions. Our systematic review of the current literature shows a beneficial effect of using moxibustion interventions on KI 1 to lower blood pressure compared to antihypertensive drugs. However, the results are influenced by the existing differences in design of the current trials

    Panax notoginseng saponins (PNS) are one of the most important compounds derived from roots of the herb Panax notoginseng which are traditionally used as a hemostatic medicine to control internal and external bleeding in China for thousands of years. To date, at least twenty saponins were identified and some of them including notoginsenoside R1, ginsenoside Rb1, and ginsenoside Rg1 were researched frequently in the area of cardiovascular protection. However, the protective effects of PNS on cardiovascular diseases based on experimental studies and its underlying mechanisms have not been reviewed systematically.
