    A correction factor for, and its application to, visual censuses of littoral fish

    Visual estimation techniques were used between October 1976 and May 1977 on the fish fauna of an enclosed mid-intertidal pool in the eastern Cape, South Africa, to test the usefulness and accuracy of this non-consumptive census method. In January, 1977, two pools were censused for fish, then poisoned using the ichthyocide ‘pronoxfish’ and all fish collected. Forty species in 21 families were obtained from the two stations. The accuracy of visual estimates varied with the species of fish. Secretive and cryptically coloured types, as well as species which Inhabit crevices, were underestimated, between 0 and 86% being counted. Other fish with protective colours but that occur more openly and do not react adversely to divers, as well as schooling species, were also underestimated, but up to 100% of the total were observed. Those which occur singly or in small groups in the water column were counted more accurately, with 57 to 100% of the actual number being seen. Correction factors were calculated from these data and applied to a census taken of one pool in May. Comparisons of corrected and actual numbers Indicate that these factors were relatively accurate for non-secretive species. The factors will, however, vary for each species, from observer to observer, depending on their personal experience, and from area to area

    Critical review of leaflets about conservative management used in UK renal services

    Background: Written information supplements nurse‐led education about treatment options. It is unclear if this information enhances patients’ reasoning about conservative management (CM) and renal replacement therapy decisions. Aim: This study describes a critical review of resources U.K. renal staff use when providing CM options to people with Established Kidney Disease (EKD) during usual pre‐dialysis education. Design: A survey using mixed methods identified and critically analysed leaflets about CM. Participants & measurements: All 72 renal units in the United Kingdom received an 11‐item questionnaire to elicit how CM education is delivered, satisfaction and/or needs with patient resources and staff training. Copies of leaflets were requested. A coding frame was utilised to produce a quality score for each leaflet. Results: Fifty‐four (75%) units participated. Patients discuss CM with a nephrologist (98%) or nurse (100%). Eighteen leaflets were reviewed, mean scores were 8.44 out of 12 (range 5–12, SD = 2.49) for information presentation; 3.50 out of 6 (range 0–6, SD = 1.58) for inclusion of information known to support shared decision‐making and 2.28 out of 6 (range 1–4, SD = 0.96) for presenting non‐biased information. Conclusions: Nurses preferred communicating via face‐to‐face contact with patients and/or families because of the emotional consequences and complexity of planning treatment for the next stage of a person's worsening kidney disease. Conversations were supplemented with written information; 66% of which were produced locally. Staff perceived a need for using leaflets, and spend time and resources developing them to support their services. However, no leaflets included the components needed to help people reason about conservative care and renal replacement therapy options during EKD education consultations

    A lattice model for the line tension of a sessile drop

    Within a semi--infinite thre--dimensional lattice gas model describing the coexistence of two phases on a substrate, we study, by cluster expansion techniques, the free energy (line tension) associated with the contact line between the two phases and the substrate. We show that this line tension, is given at low temperature by a convergent series whose leading term is negative, and equals 0 at zero temperature

    Rigorous Probabilistic Analysis of Equilibrium Crystal Shapes

    The rigorous microscopic theory of equilibrium crystal shapes has made enormous progress during the last decade. We review here the main results which have been obtained, both in two and higher dimensions. In particular, we describe how the phenomenological Wulff and Winterbottom constructions can be derived from the microscopic description provided by the equilibrium statistical mechanics of lattice gases.     Dryland restoration successes in the Sahel and Greater Horn of Africa show how to increase scale and impact

    Drylands occupy more than 40% of the world’s land area and are home to some two billion people. This includes a disproportionate number of the world’s poorest people, who live in degraded and severely degraded landscapes. The United Nations Convention to Combat Desertification states on its website that 12 million hectares are lost annually to desertification and drought, and that more than 1.5 billion people are directly dependent on land that is being degraded, leading to US$42 billion in lost earnings each year. In Africa, three million hectares of forest are lost annually, along with an estimated 3% of GDP, through depleted soils. The result is that two-thirds of Africa’s forests, farmlands and pastures are now degraded. This means that millions of Africans have to live with malnutrition and poverty, and in the absence of options this further forces the poor to overexploit their natural resources to survive. This in turn intensifies the effects of climate change and hinders economic development, threatening ecological functions that are vital to national economies

    Au/TiO2(110) interfacial reconstruction stability from ab initio

    We determine the stability and properties of interfaces of low-index Au surfaces adhered to TiO2(110), using density functional theory energy density calculations. We consider Au(100) and Au(111) epitaxies on rutile TiO2(110) surface, as observed in experiments. For each epitaxy, we consider several different interfaces: Au(111)//TiO2(110) and Au(100)//TiO2(110), with and without bridging oxygen, Au(111) on 1x2 added-row TiO2(110) reconstruction, and Au(111) on a proposed 1x2 TiO reconstruction. The density functional theory energy density method computes the energy changes on each of the atoms while forming the interface, and evaluates the work of adhesion to determine the equilibrium interfacial structure.Comment: 20 pages, 11 figure

    Enhancing Agricultural Productivity of CLMV Countries: Challenges and Agenda for Reforms

    Responding positively to economic reforms, the economies of Cambodia, the Lao People's Democratic Republic, Myanmar, and Viet Nam (CLMV) have shown tremendous growth since the mid-1980s, including in their respective agriculture sectors. Recent developments, however, have brought into question the CLMV countries' ability to sustain further increases in agricultural productivity given the slow pace of reforms and emerging challenges. Going forward, the reform agenda must go beyond the traditional view of expanding yields and supply of agricultural products for development gains in the sector to contribute to inclusive growth, poverty alleviation, and food security. This will require changing the market structures and regulatory policies that govern the sector

    Assessment of information resources for people with hypodontia

    Aim: To assess the adequacy of patient information to support understanding and decision-making for people affected by hypodontia. Methods: 1) Questionnaire to understand the provision of patient information by dentists; 2) Systematic search to identify online open-access patient information; 3) Quality assessment of written patient information. Results: Questionnaire response rate was 49% (319/649); 91% examined and/or treated people with hypodontia. Most general dentists referred patients to specialist services without providing written hypodontia information. The majority of dental specialists provide patient leaflets but less than a third used web-resources. Only 19% of respondents felt current resources were fit-for-purpose. Thirty-one patient resources (18 leaflets and 13 online) were assessed against quality criteria. The aim of the resource was seldom explicit, the content was often incomplete and variation in readability scores indicated high levels of literacy were required. Discussion: Access to, and quality of, patient information for hypodontia is inadequate. Current resources are not sufficiently comprehensive to prepare young patients to engage in shared dental care decisions with their parents and/or dental professionals. Conclusion: There is a need for improved access to, and provision of, information about hypodontia if dental professionals want to meet best practice guidance and involve patients in shared decision-making

    Adjuvant chemotherapy in upper tract urothelial carcinoma (the POUT trial): a phase 3, open-label, randomised controlled trial

    Background: Urothelial carcinomas of the upper urinary tract (UTUCs) are rare, with poorer stage-for-stage prognosis than urothelial carcinomas of the urinary bladder. No international consensus exists on the benefit of adjuvant chemotherapy for patients with UTUCs after nephroureterectomy with curative intent. The POUT (Peri-Operative chemotherapy versus sUrveillance in upper Tract urothelial cancer) trial aimed to assess the efficacy of systemic platinum-based chemotherapy in patients with UTUCs. Methods: We did a phase 3, open-label, randomised controlled trial at 71 hospitals in the UK. We recruited patients with UTUC after nephroureterectomy staged as either pT2–T4 pN0–N3 M0 or pTany N1–3 M0. We randomly allocated participants centrally (1:1) to either surveillance or four 21-day cycles of chemotherapy, using a minimisation algorithm with a random element. Chemotherapy was either cisplatin (70 mg/mÂČ) or carboplatin (area under the curve [AUC]4·5/AUC5, for glomerular filtration rate <50 mL/min only) administered intravenously on day 1 and gemcitabine (1000 mg/mÂČ) administered intravenously on days 1 and 8; chemotherapy was initiated within 90 days of surgery. Follow-up included standard cystoscopic, radiological, and clinical assessments. The primary endpoint was disease-free survival analysed by intention to treat with a Peto-Haybittle stopping rule for (in)efficacy. The trial is registered with ClinicalTrials.gov, NCT01993979. A preplanned interim analysis met the efficacy criterion for early closure after recruitment of 261 participants. Findings: Between June 19, 2012, and Nov 8, 2017, we enrolled 261 participants from 57 of 71 open study sites. 132 patients were assigned chemotherapy and 129 surveillance. One participant allocated chemotherapy withdrew consent for data use after randomisation and was excluded from analyses. Adjuvant chemotherapy significantly improved disease-free survival (hazard ratio 0·45, 95% CI 0·30–0·68; p=0·0001) at a median follow-up of 30·3 months (IQR 18·0–47·5). 3-year event-free estimates were 71% (95% CI 61–78) and 46% (36–56) for chemotherapy and surveillance, respectively. 55 (44%) of 126 participants who started chemotherapy had acute grade 3 or worse treatment-emergent adverse events, which accorded with frequently reported events for the chemotherapy regimen. Five (4%) of 129 patients managed by surveillance had acute grade 3 or worse emergent adverse events. No treatment-related deaths were reported. Interpretation: Gemcitabine–platinum combination chemotherapy initiated within 90 days after nephroureterectomy significantly improved disease-free survival in patients with locally advanced UTUC. Adjuvant platinum-based chemotherapy should be considered a new standard of care after nephroureterectomy for this patient population. Funding: Cancer Research UK
