    The contacts between the Prussian Baltic ports with Norway have a centuries old tradition. The Northern Scandinavia was a vital source of fish products, such as; salted and desiccated cod, codliver oil or hard roe. They, in turn, were considered to be the most significant ingredients of the diet of the people inhabiting the middle-east Europe. Moreover, ports such as Gda艅sk and Szczecin constituted the essential links on the trade routes between Norway and Europe. In the middle of the 19th century, due to the Industrial Revolution and, most of all, the invention of the telegraph and steam ships, the trade ties between geographically remoted contracting parties were tightened. Consequently, the trade centers such as Bergen and Copenhagen ceased to mediate. What is more, the credit system and keeping the market analyzes were modernized. The marriage of Margrete von Niemirsky from Gda艅sk with the well-off merchant from Troms酶 may be the example of such close contacts of the Prussian ports with the Far North. As another example may serve the history of the staemer which was the private initiative of the merchant Zahl, Kj忙rring酶y, aiming to bring closer the Northern Norway and the Baltic sea

    Calcific aortic valve stenosis is the most common valvular heart disease in developed countries. Without surgery, the prognosis is extremely dismal. Therefore there is general agreement that surgical aortic valve replacement should be offered to patients with symptomatic severe aortic valve stenosis. However, surgery is denied to approx. 30–40% of elderly patients with severe, symptomatic aortic stenosis due to high perioperative risk. Transcatheter aortic valve implantation (TAVI) is a novel and effective procedure which provides a promising treatment option for some of these patients. W zwi膮zku z tym w 艣wietle obowi膮zuj膮cych wytycznych u pacjent贸w z objawow膮 ci臋偶k膮 stenoz膮 aortaln膮 zaleca si臋 chirurgiczn膮 wymian臋 zastawki aortalnej. Jednak oko艂o 30–40% starszych chorych z ci臋偶k膮 objawow膮 wad膮 aortaln膮 jest dyskwalifikowanych z leczenia operacyjnego ze wzgl臋du na jego wysokie ryzyko.     Diagnosis of myelodysplastic syndromes in Poland: Polish Adult Leukemia Group (PALG) 2021 recommendations

    Myelodysplastic syndromes (MDS) are a heterogeneous group of neoplastic diseases of the hematopoietic cells manifested by ineffective hematopoiesis and a tendency to transform into acute myeloid leukemia. MDS should be considered in the differential diagnosis of cytopenia, especially in the elderly. This article presents the recommendations of MDS experts of the Polish Adult Leukemia Group (PALG) for the diagnosis of myelodysplastic syndromes. We present current classifications and prognostic indices, as well as diagnostic examinations recommended for MDS: cytological, histopathological, immunophenotypic, cytogenetic and molecular tests. W pracy przytoczono wiele bada艅, potwierdzaj膮cych, 偶e odpowiednie od偶ywianie, znacz膮co wp艂ywa na czas rekonwalescencji pacjent贸w po operacjach i jest bardzo wa偶nym elementem opieki pooperacyjnej. Celem pracy by艂o tak偶e ukazanie znaczenia dietetyka w pracy ca艂ego zespo艂u opiekuj膮cego si臋 pacjentem, a tak偶e znaczenia odpowiedniej diety w zapobieganiu i leczeniu wielu jednostek chorobowych. W pracy om贸wiono r贸偶ne rodzaje diet, a tak偶e scharakteryzowano warto艣膰 leczenia 偶ywieniowego, zar贸wno w warunkach szpitalnych, jak i domowych. Praca udowadnia, 偶e wczesne doustne 偶ywienie pooperacyjne, skraca czas rekonwalescencji oraz zwi臋ksza szanse na powr贸t pacjent贸w do zdrowia oraz pe艂ne odzyskanie sprawno艣ci.This text shows a relationship between well nutrition care for patients after surgery and time which they need for full recovery. The author presents a number of studies, which point out that proper nutrition has a significant influence on convalescence time after surgery and it is a vital part of postoperative care. The main purpose of this analysis is to show how essential dietitians are in a group which is taking care of patients. Another purpose is to draw attention to how a suitable diet can help in preventing and treating a lot of illnesses. The thesis describes various kinds of diets and nutrition support, in hospitals and in homes. In addition, it proves, that early oral nutrition after surgery, results in shorter convalescence time and increases chances for full recovery

    The Role of Light Kappa and Lambda Chains in Heart Function Assessment in Patients with AL Amyloidosis

    There are reports indicating that myocardial dysfunction in systemic immunoglobulin light chain amyloidosis (AL amyloidosis) stems not only from the amyloid deposit in the organ but also the cardiotoxicity of the amyloid precursor free light chains (FLCs) circulating in the blood. The aim of the study is to analyze the role of sFLC 魏 and 位 in the assessment of heart involvement and the degree of myocardial damage in AL amyloidosis. The study involved 71 patients diagnosed with primary AL amyloidosis. The relationship between sFLC concentrations and cardiac biochemical and echocardiographic parameters was assessed. The median concentrations of N-terminal pro b-type natriuretic peptide(NT-proBNP) and troponin I (TnI) were significantly higher in patients with amyloids formed from monoclonal 位 chains compared to patients with monoclonal 魏 proliferation. In patients with heart involvement by amyloids formed from monoclonal FLC, the study demonstrated a statistically significant positive correlation between the concentration of monoclonal antibody 位 chain and TnI (R = 0.688; p < 0.05), NT-proBNP (R = 0.449; p < 0.05), and the value of diastolic dimension of the interventricular septum (IVS; R = 0.496, p < 0.05). The above data indicate that the presence of monoclonal 位 chains in patients with AL amyloidosis may be associated with more severe damage to cardiomyocytes and dysfunction of the myocardium
