Practice of sedation and analgesia in German intensive care units: results of a national survey
INTRODUCTION: Sedation and analgesia are provided by using different agents and techniques in different countries. The goal is to achieve early spontaneous breathing and to obtain an awake and cooperative pain-free patient. It was the aim of this study to conduct a survey of the agents and techniques used for analgesia and sedation in intensive care units in Germany. METHODS: A survey was sent by mail to 261 hospitals in Germany. The anesthesiologists running the intensive care unit were asked to fill in the structured questionnaire about their use of sedation and analgesia. RESULTS: A total of 220 (84%) questionnaires were completed and returned. The RAMSAY sedation scale was used in 8% of the hospitals. A written policy was available in 21% of hospitals. For short-term sedation in most hospitals, propofol was used in combination with sufentanil or fentanyl. For long-term sedation, midazolam/fentanyl was preferred. Clonidine was a common part of up to two-thirds of the regimens. Epidural analgesia was used in up to 68%. Neuromuscular blocking agents were no longer used. CONCLUSION: In contrast to the US 'Clinical practice guidelines for the sustained use of sedatives and analgesics in the critically ill adult', our survey showed that in Germany different agents, and frequently neuroaxial techniques, were used
Ethanol-Induced Alterations of T Cells and Cytokines after Surgery in a Murine Infection Model
Background. Interactions between alcohol, infection, and surgery and their effect on differentiation and functionality of T helper cells are not yet completely understood. We hypothesized that alcohol and surgery disturb differentiation of T helper cells and contribute to an impaired immune response. Methods. Mice were treated with alcohol for two weeks. Saline treatment served as control. Clinical performance and weight were assessed. On day 14, a median laparotomy was performed and animals were challenged with Klebsiella pneumoniae intranasally. Bacterial load was determined in lungs and blood. T helper cell subpopulations and the released cytokines were assessed in lungs, spleens, and plasma. Key transcription factors of T cell differentiation were evaluated. Results. Alcohol significantly impaired clinical appearance and body weight of animals with postsurgical infection (p<0.05). Bacterial load was significantly higher after alcohol treatment (p<0.05). T helper cell subsets and released cytokine levels were significantly altered in lung, but not in spleen. Expression of transcription factors of T helper cell lineage commitment did not translate into different counts of T helper cells. Conclusions. Alcohol and surgery lead to significant cellular and functional modulations of T helper cells during postsurgical infection. These effects may contribute to an impaired immune response after surgery
Predictive Ability of the Stability and Workload Index for Transfer Score to Predict Unplanned Readmissions After ICU Discharge
Objective: Unplanned readmission of hospitalized patients to an ICU is associated with an increased mortality and hospital length of stay. The ability to identify patients at risk, who would benefit from prolonged ICU treatment, is limited. The aim of this study is to validate a previously published numerical index named the Stability and Workload Index for Transfer in a heterogeneous group of ICU patients. Design: In this retrospective data analysis, the Stability and Workload Index for Transfer score was calculated for all patients, and the ability of the score to predict readmission was compared with the original publication. Setting: Four ICUs, one intermediate care unit, and one postanesthesia care unit of the department of anesthesia and intensive care of a university hospital. Patients: All consecutive patients treated in one of the units. Interventions: None. Measurements and Main Results: Unplanned ICU readmissions or unexpected death within 7 days of ICU discharge. The data of 7,175 patients were included in the analysis. Five hundred ninety-six patients were readmitted or died within 7 days of discharge. The patients who are readmitted to the ICU are significantly older and have significantly higher scores that define the severity of disease at the time of admission and discharge of their first ICU stay. The source of admission for the initial ICU stay did not differ (p = 0.055), and the last Glasgow Coma Scale and the last Pao(2)/Fio(2) ratio before discharge from the ICU were higher in patients who did not need a readmission to the ICU. The performance of the Stability and Workload Index for Transfer score is poor with an area under the receiver operator curve of 0.581 (95% CI, 0.556-0.605; p < 0.001). Conclusions: Based on the data from our patients, the proposed Stability and Workload Index for Transfer score by Gajic et al is not ideal in aiding the clinician in the decision, if a patient can be discharged safely from the ICU and further research is necessary to define the patients at risk for readmission
Risk factors in critical illness myopathy during the early course of critical illness: a prospective observational study
INTRODUCTION: Non-excitable muscle membrane indicates critical illness myopathy (CIM) during early critical illness. We investigated predisposing risk factors for non-excitable muscle membrane at onset of critical illness. METHODS: We performed sequential measurements of muscle membrane excitability after direct muscle stimulation (dmCMAP) in 40 intensive care unit (ICU) patients selected upon a simplified acute physiology (SAPS-II) score >OR= 20 on 3 successive days within 1 week after ICU admission. We then investigated predisposing risk factors, including the insulin-like growth factor (IGF)-system, inflammatory, metabolic and hemodynamic parameters, as well as suspected medical treatment prior to first occurrence of abnormal dmCMAP. Nonparametric analysis of two-factorial longitudinal data and multivariate analysis were used for statistical analysis. RESULTS: 22 patients showed abnormal muscle membrane excitability during direct muscle stimulation within 7 (5 to 9.25) days after ICU admission. Significant risk factors for the development of impaired muscle membrane excitability in univariate analysis included inflammation, disease severity, catecholamine and sedation requirements, as well as IGF binding protein-1 (IGFBP-I), but did not include either adjunctive hydrocortisone treatment in septic shock, nor administration of neuromuscular blocking agents or aminoglycosides. In multivariate Cox regression analysis, interleukin-6 remained the significant risk factor for the development of impaired muscle membrane excitability (HR 1.006, 95%-CI (1.002 to 1.011), P = 0.002). CONCLUSIONS: Systemic inflammation during early critical illness was found to be the main risk factor for development of CIM during early critical illness. Inflammation-induced impairment of growth-factor mediated insulin sensitivity may be involved in the development of CIM
A Randomised Controlled Trial
Liver surgery is still associated with a high rate of morbidity and mortality.
We aimed to compare different haemodynamic treatments in liver surgery. In a
prospective, blinded, randomised, controlled pilot trial patients undergoing
liver resection were randomised to receive haemodynamic management guided by
conventional haemodynamic parameters or by oesophageal Doppler monitor (ODM,
CardioQ-ODM) or by pulse power wave analysis (PPA, LiDCOrapid) within a goal-
directed algorithm adapted for liver surgery. The primary endpoint was stroke
volume index before intra-operative start of liver resection. Secondary
endpoints were the haemodynamic course during surgery and postoperative pain
levels. Due to an unbalance in the extension of the surgical procedures with a
high rate of only minor procedures the conventional group was dropped from the
analysis. Eleven patients in the ODM group and 10 patients in the PPA group
were eligible for statistical analysis. Stroke volume index before start of
liver resection was 49 (37; 53) ml/m2 and 48 (41; 56) ml/m2 in the ODM and PPA
group, respectively (p=0.397). The ODM guided group was haemodynamically
stable as shown by ODM and PPA measurements. However, the PPA guided group
showed a significant increase of pulse-pressure-variability (p=0.002) that was
not accompanied by a decline of stroke volume index displayed by the PPA
(p=0.556) but indicated by a decline of stroke volume index by the ODM
(p<0.001). The PPA group had significantly higher postoperative pain levels
than the ODM group (p=0.036). In conclusion, goal-directed optimization by ODM
and PPA showed differences in intraoperative cardiovascular parameters
indicating that haemodynamic optimization is not consistent between the two
monitors

Background Intensive care unit (ICU)-acquired weakness in critically ill
patients is a common and significant complication affecting the course of
critical illness. Whole-body vibration is known to be effective muscle
training and may be an option in diminishing weakness and muscle wasting.
Especially, patients who are immobilized and not available for active
physiotherapy may benefit. Until now whole-body vibration was not investigated
in mechanically ventilated ICU patients. We investigated the safety,
feasibility, and metabolic response of whole-body vibration in critically ill
patients. Methods We investigated 19 mechanically ventilated, immobilized ICU
patients. Passive range of motion was performed prior to whole-body vibration
therapy held in the supine position for 15 minutes. Continuous monitoring of
vital signs, hemodynamics, and energy metabolism, as well as intermittent
blood sampling, took place from the start of baseline measurements up to 1
hour post intervention. We performed comparative longitudinal analysis of the
phases before, during, and after intervention. Results Vital signs and
hemodynamic parameters remained stable with only minor changes resulting from
the intervention. No application had to be interrupted. We did not observe any
adverse event. Whole-body vibration did not significantly and/or clinically
change vital signs and hemodynamics. A significant increase in energy
expenditure during whole-body vibration could be observed. Conclusions In our
study the application of whole-body vibration was safe and feasible. The
technique leads to increased energy expenditure. This may offer the chance to
treat patients in the ICU with whole-body vibration. Further investigations
should focus on the efficacy of whole-body vibration in the prevention of ICU-
acquired weakness. Trial registration Applicability and Safety of Vibration

Influence of Sedation Level and Ventilation Status on the Diagnostic Validity of Delirium Screening Tools in the ICU—An International, Prospective, Bi-Center Observational Study (IDeAS)
Background and objectives:
The use of delirium screening instruments (DSIs) is recommended in critical care practice for a timely detection of delirium. We hypothesize that the patient-related factors "level of sedation" and "mechanical ventilation" impact test validity of DSIs.
Materials and Methods:
This is a prospective, bi-center observational study (clinicaltrials.gov: NCT01720914). Critically ill patients were screened for delirium daily for up to seven days after enrollment using the Nursing Delirium Screening Scale (Nu-DESC), Intensive Care Delirium Screening Checklist (ICDSC), and Confusion Assessment Method for the Intensive Care Unit (CAM-ICU). Reference standard for delirium diagnosis was the neuropsychiatric examination using the criteria of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR). Immediately before delirium assessment, ventilation status and sedation levels were documented.
Results:
160 patients were enrolled and 151 patients went into final analysis. Delirium incidence was 23.2%. Nu-DESC showed a sensitivity and specificity of 88.5%, a positive predictive value (PPV) of 71.9%, and a negative predictive value (NPV) of 95.8%. ICDSC had a sensitivity of 62.5%, a specificity of 92.4%, a PPV of 71.4%, and a NPV of 89.0%. CAM-ICU showed a sensitivity of 75.0%, a specificity of 94.7%, a PPV of 85.7%, and a NPV of 90.0%. For Nu-DESC and ICDSC, test validity was significantly better for non-sedated patients (Richmond Agitation Sedation Scale (RASS) 0/-1), whereas test validity for CAM-ICU in a severity scale version showed no significant differences for different sedation levels. No DSI showed a significant difference in test validity between noninvasively and invasively ventilated patients.
Conclusions:
Test validities of DSIs were comparable to previous studies. The observational scores ICDSC and Nu-DESC showed a significantly better performance in awake and drowsy patients (RASS 0/-1) when compared with other sedation levels. Physicians should refrain from sedation whenever possible to avoid suboptimal performance of DSIs
