    Development of a Musculoskeletal Ultrasound Education Curriculum for Ultrasound Technologists and Musculoskeletal Fellows: An Adventure in Co-Learning

    Musculoskeletal ultrasound is highly operator dependent. Appropriate training is essential to provide high quality imaging to patients. We developed a combined musculoskeletal ultrasound curriculum for our musculoskeletal imaging fellows and our ultrasound technologists to help provide this training. This combined curriculum allows fellows to teach technologists about anatomy and pathology, and the technologists to instruct the fellows on sonographic technique. After viewing our poster exhibit, the learner should: 1.Understand the benefits of co-learning between musculoskeletal imaging fellows and ultrasound technologists 2.Be able to identify key components of a musculoskeletal ultrasound curriculum for both ultrasound technologists and musculoskeletal imaging fellows 3.Understand how we implemented this curriculum in our department through learning sessions. We will review the learning needs of ultrasound technologists as well as those of the musculoskeletal fellows. The curriculum consists of both a didactic component and a hands on component, covering several joints. The learners are given directed readings. During the lecture portion of the curriculum, anatomy, sonographic anatomy, sonographic pathology, and scan technique are reviewed. During the hands on portion of the curriculum, the faculty member first demonstrates the sonographic technique for the particular joint. Next, the faculty facilitates practice scanning between the ultrasound technologists and the musculoskeletal imaging fellows, who work together to provide education to each other based on their skill sets. Methods of assessment will be discussed. We will discuss how we implemented our multiple monthly learning sessions. We will include survey feedback from ultrasound technologists, musculoskeletal imaging fellows, and musculoskeletal imaging faculty. We will discuss implications and future proposals

    A Role for Compromise: Synaptic Inhibition and Electrical Coupling Interact to Control Phasing in the Leech Heartbeat CPG

    How can flexible phasing be generated by a central pattern generator (CPG)? To address this question, we have extended an existing model of the leech heartbeat CPG's timing network to construct a model of the CPG core and explore how appropriate phasing is set up by parameter variation. Within the CPG, the phasing among premotor interneurons switches regularly between two well defined states – synchronous and peristaltic. To reproduce experimentally observed phasing, we varied the strength of inhibitory synaptic and excitatory electrical input from the timing network to follower premotor interneurons. Neither inhibitory nor electrical input alone was sufficient to produce proper phasing on both sides, but instead a balance was required. Our model suggests that the different phasing of the two sides arises because the inhibitory synapses and electrical coupling oppose one another on one side (peristaltic) and reinforce one another on the other (synchronous). Our search of parameter space defined by the strength of inhibitory synaptic and excitatory electrical input strength led to a CPG model that well approximates the experimentally observed phase relations. The strength values derived from this analysis constitute model predictions that we tested by measurements made in the living system. Further, variation of the intrinsic properties of follower interneurons showed that they too systematically influence phasing. We conclude that a combination of inhibitory synaptic and excitatory electrical input interacting with neuronal intrinsic properties can flexibly generate a variety of phase relations so that almost any phasing is possible

    Method to provide meta-stable operation of DC microgrid comprising a pulsed load

    A Hamiltonian surface shaping power flow control (HSSPFC) method is used to analyze the meta-stability and adjust pulsed power loads on a DC electric power distribution network. Pulsed power loads are nonlinear, time-variant systems that cause nonlinear limit-cycles. During the on periods of a pulsed load, the system can be in an unstable state and is damped back to stability during the off state of the load. Therefore, over the entire period of the pulse the system may only be assessed as meta-stable. As shown through simulation, HIL and hardware results, the HSSPFC method is more accurate than the other small-signal approaches, such as Eigenvalues, Nyquist, and Floquet theory, and can reveal important details about the transient responses and performance.https://digitalcommons.mtu.edu/patents/1141/thumbnail.jp

    An integrated approach to emergency triage assessment and treatment in Uganda

    Background: Reducing mortality among patients who present to primary referral facilities in need of emergency care requires efficient triage and immediate correct management known more commonly as Emergency Triage, Assessment and Treatment (ETAT). Integrated Management of Infectious Disease (IMID) training and on-site support (OSS) have been the tools used to build capacity among mid-level practitioners who have been found to be lacking in ETAT skills in the region.Objective: To investigate the use of Integrated Management of Infectious Disease (IMID) training and on-site support (OSS) tools to improve ETAT in health care facilities in rural Uganda.Design: Randomized mixed methods evaluation.Setting: Thirty-six facilities in Uganda were randomized 1:1 to arm A (IMID and OSS) and arm B (IMID alone).Subjects: Two mid-level practitioners, (clinical officers or senior nurses), from each facility participated in offsite IMID training. Staff at 18 facilities in arm A participated in OSS (two days each month of outreach and quality improvement for nine months).Results: Time series data on facility performance of three ETAT indicators were compared over 14 months and data on mortality among pediatric inpatients were compared across arms. Improvements differed across facilities and indicators, but steady improvement in triage occurred in arm A, with convergence across arms in the management of emergency patients. Analysis of baseline indicators and improvements demonstrated that facilities could improve their performance regardless of their starting point.Conclusion: IMID and OSS both improved the management of patients with an acute illness presenting to rural health facilities
