Pilot Proof of Concept Clinical Trials of Stochastic Targeted (STAR) Glycemic Control
(open access)Introduction: Tight glycemic control (TGC) has shown benefits but has been difficult to achieve consistently. STAR (Stochastic TARgeted) is a flexible, model-based TGC approach directly accounting for intra- and inter- patient variability with a stochastically derived maximum 5% risk of blood glucose (BG) < 4.0 mmol/L. This research assesses the safety, efficacy, and clinical burden of a STAR TGC controller modulating both insulin and nutrition inputs in pilot trials.
Methods: Seven patients covering 660 hours. Insulin and nutrition interventions are given 1-3 hourly as chosen by the nurse to allow them to manage workload. Interventions are calculated by using clinically validated computer models of human metabolism and its variability in critical illness to maximize the overlap of the model-predicted (5-95th percentile) range of BG outcomes with the 4.0-6.5 mmol/L band while ensuring a maximum 5% risk of BG < 4.0 mmol/L. Carbohydrate intake (all sources) was selected to maximize intake up to 100% of SCCM/ACCP goal (25 kg/kcal/h). Maximum insulin doses and dose changes were limited for safety. Measurements were made with glucometers. Results are compared to those for the SPRINT study, which reduced mortality 25-40% for length of stay ≥3 days. Written informed
consent was obtained for all patients, and approval was granted by the NZ Upper South A Regional Ethics Committee.
Results: A total of 402 measurements were taken over 660 hours (~14/day), because nurses showed a preference for 2-hourly measurements. Median [interquartile range, (IQR)] cohort BG was 5.9 mmol/L [5.2-6.8]. Overall, 63.2%, 75.9%, and 89.8% of measurements were in the 4.0-6.5, 4.0-7.0, and 4.0-8.0 mmol/L bands. There were no
hypoglycemic events (BG < 2.2 mmol/L), and the minimum BG was 3.5 mmol/L with 4.5% < 4.4 mmol/L. Per patient, the median [IQR] hours of TGC was 92 h [29-113] using 53 [19-62] measurements (median, ~13/day). Median [IQR] results: BG, 5.9 mmol/L [5.8-6.3]; carbohydrate nutrition, 6.8 g/h [5.5-8.7] (~70% goal feed median); insulin, 2.5 U/h [0.1-5.1]. All patients achieved BG < 6.1 mmol/L. These results match or exceed SPRINT and clinical workload is reduced more than 20%.
Conclusions: STAR TGC modulating insulin and nutrition inputs provided very tight control with minimal variability by managing intra- and inter- patient variability. Performance and safety exceed that of SPRINT, which reduced mortality and cost in the Christchurch ICU. The use of glucometers did not appear to impact the quality of TGC.
Finally, clinical workload was self-managed and reduced 20% compared with SPRINT
The impact of basic vs. enhanced Go NAPSACC on child care centers' healthy eating and physical activity practices: Protocol for a type 3 hybrid effectiveness-implementation cluster-randomized trial
Background: To prevent childhood obesity and promote healthy development, health authorities recommend that child care programs use the evidence-based practices that foster healthy eating and physical habits in children. Go NAPSACC is an intervention shown to improve use of these recommended practices, but it is known to encounter barriers that limit its impact and widespread use. Methods: This study will use a type 3 hybrid effectiveness-implementation cluster-randomized trial to compare effectiveness and implementation outcomes achieved from Go NAPSACC delivered with a basic or enhanced implementation approach. Participants will include approximately 25 coaches from Child Care Aware of Kentucky (serving four geographic regions), 97 child care centers with a director and teacher from each and two cross-sectional samples of 485 3-4-year-old children (one recruitment at baseline, another at follow-up). Coaches will be randomly assigned to deliver Go NAPSACC using either the basic or enhanced implementation approach. "Basic Go NAPSACC" represents the traditional way of delivering Go NAPSACC. "Enhanced Go NAPSACC" incorporates preparatory and support activities before and during their Go NAPSACC work, which are guided by the Quality Implementation Framework and the Consolidated Framework for Implementation Research. Data will be collected primarily at baseline and post-intervention, with select measures continuing through 6, 12, and 24 months post-intervention. Guided largely by RE-AIM, outcomes will assess change in centers' use of evidence-based nutrition and physical activity practices (primary, measured via observation); centers' adoption, implementation, and maintenance of the Go NAPSACC program (assessed via website use); center directors', teachers', and coaches' perceptions of contextual factors (assessed via self-report surveys); children's eating and physical activity behaviors at child care (measured via observation and accelerometers); and cost-effectiveness (assessed via logs and expense tracking). The hypotheses anticipate that "Enhanced Go NAPSACC" will have greater effects than "Basic Go NAPSACC." Discussion: This study incorporates many lessons gleaned from the growing implementation science field, but also offers opportunities to address the field's research priorities, including applying a systematic method to tailor implementation strategies, examining the processes and mechanisms through which implementation strategies produce their effects, and conducting an economic evaluation of implementation strategies. Modelling Landscape-scale Species Response to Agri-Environment Schemes
Agri-environment schemes (AES) are the most significant environmental policy delivery mechanism in England, and include the conservation of biodiversity as a key objective. Provisional results from the ongoing Landscape-scale species monitoring of AES (LandSpAES) baseline field survey have shown some positive responses of mobile taxa to AES gradients at local (1km2) or landscape (3 × 3km) scales. However, it is not known whether these provisional results might be more broadly applicable outside the regions surveyed in the LandSpAES project, i.e. in other regions, or nationally.
Here, we present the findings of an analytical project to explore the use of national Citizen Science (CitSci) scheme data, to investigate whether similar relationships with AES gradients would be found at a national scale in CitSci data to those shown with LandSpAES data, and whether integrated modelling was possible with combined CitSci and LandSpAES datasets. The design of LandSpAES has high power to detect AES effects, including the independent testing of the local and landscape AES gradients, but is restricted to six regions. The national CitSci are more representative of England as a whole, but have not been designed to detect AES effects.
The aim of this project was to determine whether the provisional taxon responses to the AES gradients found in the LandSpAES project could be detected at a national scale using CitSci scheme data.
To achieve this aim, three key questions were addressed through the analytical work:
1) Can addition of covariates account for environmental variation between survey squares in each dataset, to improve the comparability of AES gradient effects between LandSpAES and CitSci schemes?
2) Do the CitSci scheme datasets show similar relationships between taxa responses and the AES gradients, to those found with the LandSpAES data?
3) Can integrated approaches to combining datasets be used to jointly model CitSci and LandSpAES data, and does integrated modelling reduce uncertainty in quantifying the effects of AES gradients on taxa responses at a national scale across England
Erasmus Language students in a British University – a case study
Students’ assessment of their academic experience is actively sought by Higher Education institutions, as evidenced in the National Student Survey introduced in 2005. Erasmus students, despite their growing numbers, tend to be excluded from these satisfaction surveys, even though they, too, are primary customers of a University. This study aims to present results from bespoke questionnaires and semi-structured interviews with a sample of Erasmus students studying languages in a British University. These methods allow us insight into the experience of these students and their assessment as a primary customer, with a focus on language learning and teaching, university facilities and student support. It investigates to what extent these factors influence their levels of satisfaction and what costs of adaptation if any, they encounter. Although excellent levels of satisfaction were found, some costs affect their experience. They relate to difficulties in adapting to a learning methodology based on a low number of hours and independent learning and to a guidance and support system seen as too stifling. The results portray this cohort’s British University as a well-equipped and well-meaning but ultimately overbearing institution, which may indicate that minimising costs can eliminate some sources of dissatisfaction
Electrically Charged Topological Solitons
Two new families of T-Dual integrable models of dyonic type are constructed.
They represent specific singular Non-Abelian Affine Toda models
having U(1) global symmetry. Their 1-soliton spectrum contains both neutral and
U(1) charged topological solitons sharing the main properties of 4-dimensional
Yang-Mills-Higgs monopoles and dyons. The semiclassical quantization of these
solutions as well as the exact counterterms and the coupling constant
renormalization are studied.Comment: 40 pages, latex, typos corrected in eq. (2.3), new section on spin of
solitons added, extended discussion on T-duality relation between axial and
vector topological theta terms in new subsection 2.
Simulations of the Static Friction Due to Adsorbed Molecules
The static friction between crystalline surfaces separated by a molecularly
thin layer of adsorbed molecules is calculated using molecular dynamics
simulations. These molecules naturally lead to a finite static friction that is
consistent with macroscopic friction laws. Crystalline alignment, sliding
direction, and the number of adsorbed molecules are not controlled in most
experiments and are shown to have little effect on the friction. Temperature,
molecular geometry and interaction potentials can have larger effects on
friction. The observed trends in friction can be understood in terms of a
simple hard sphere model.Comment: 13 pages, 13 figure
The Intentional Use of Service Recovery Strategies to Influence Consumer Emotion, Cognition and Behaviour
Service recovery strategies have been identified as a critical factor in the success of. service organizations. This study develops a conceptual frame work to investigate how specific service recovery strategies influence the emotional, cognitive and negative behavioural responses of . consumers., as well as how emotion and cognition influence negative behavior. Understanding the impact of specific service recovery strategies will allow service providers' to more deliberately and intentionally engage in strategies that result in positive organizational outcomes. This study was conducted using a 2 x 2 between-subjects quasi-experimental design. The results suggest that service recovery has a significant impact on emotion, cognition and negative behavior. Similarly, satisfaction, negative emotion and positive emotion all influence negative behavior but distributive justice has no effect
Monte Carlo Methods for Estimating Interfacial Free Energies and Line Tensions
Excess contributions to the free energy due to interfaces occur for many
problems encountered in the statistical physics of condensed matter when
coexistence between different phases is possible (e.g. wetting phenomena,
nucleation, crystal growth, etc.). This article reviews two methods to estimate
both interfacial free energies and line tensions by Monte Carlo simulations of
simple models, (e.g. the Ising model, a symmetrical binary Lennard-Jones fluid
exhibiting a miscibility gap, and a simple Lennard-Jones fluid). One method is
based on thermodynamic integration. This method is useful to study flat and
inclined interfaces for Ising lattices, allowing also the estimation of line
tensions of three-phase contact lines, when the interfaces meet walls (where
"surface fields" may act). A generalization to off-lattice systems is described
as well.
The second method is based on the sampling of the order parameter
distribution of the system throughout the two-phase coexistence region of the
model. Both the interface free energies of flat interfaces and of (spherical or
cylindrical) droplets (or bubbles) can be estimated, including also systems
with walls, where sphere-cap shaped wall-attached droplets occur. The
curvature-dependence of the interfacial free energy is discussed, and estimates
for the line tensions are compared to results from the thermodynamic
integration method. Basic limitations of all these methods are critically
discussed, and an outlook on other approaches is given
A measurement of the tau mass and the first CPT test with tau leptons
We measure the mass of the tau lepton to be 1775.1+-1.6(stat)+-1.0(syst.) MeV
using tau pairs from Z0 decays. To test CPT invariance we compare the masses of
the positively and negatively charged tau leptons. The relative mass difference
is found to be smaller than 3.0 10^-3 at the 90% confidence level.Comment: 10 pages, 4 figures, Submitted to Phys. Letts.
