    Identification of Source Faults of Large Earthquakes in the Turkey‐Syria Border Region Between 1000 CE and the Present, and Their Relevance for the 2023 Mw 7.8 Pazarcık Earthquake

    The 6 February 2023, Mw 7.8 Pazarcık earthquake in the Turkey-Syria border region raises the question of whether such a large earthquake could have been foreseen, as well as what is the maximum possible magnitude (Mmax) of earthquakes on the East Anatolian Fault (EAF) system and on continental transform faults in general. To answer such questions, knowledge of past earthquakes and of their causative faults is necessary. Here, we integrate data from historical seismology, paleoseismology, archeoseismology, and remote sensing to identify the likely source faults of fourteen Mw ≥ 7 earthquakes between 1000 CE and the present in the region. We find that the 2023 Pazarcık earthquake could have been foreseen in terms of location (the EAF) and timing (an earthquake along this fault was if anything overdue), but not magnitude. We hypothesize that the maximum earthquake magnitude for the EAF is in fact 8.2, that is, a single end-to-end rupture of the entire fault, and that the 2023 Pazarcık earthquake did not reach Mmax by a fortuitous combination of circumstances. We conclude that such unusually large events are hard to model in terms of recurrence intervals, and that seismic hazard assessment along continental transforms cannot be done on individual fault systems but must include neighboring systems as well, because they are not kinematically independent at any time scale

    Acute hiatal hernia: a late complication following gastrectomy

    <p>Abstract</p> <p>Introduction</p> <p>We describe a case of acute hiatal hernia during chemotherapy, in a female patient previously treated with gastrectomy.</p> <p>Case presentation</p> <p>After gastric resection, the patient underwent chemotherapy, developing important emetic symptoms. A radiograph of the abdomen was performed because of acute epigastrial pain and it showed a marked left diaphragm elevation.</p> <p>A CT scan carried out 24 hours later identified an occlusion with herniation in the left hemi thorax. Subsequent surgical investigation resulted in a diagnosis of hiatal hernia with volvulus.</p> <p>Conclusions</p> <p>This case represents a rare, late complication occurring after gastrectomy.</p

    Association of kidney disease measures with risk of renal function worsening in patients with type 1 diabetes

    Background: Albuminuria has been classically considered a marker of kidney damage progression in diabetic patients and it is routinely assessed to monitor kidney function. However, the role of a mild GFR reduction on the development of stage 653 CKD has been less explored in type 1 diabetes mellitus (T1DM) patients. Aim of the present study was to evaluate the prognostic role of kidney disease measures, namely albuminuria and reduced GFR, on the development of stage 653 CKD in a large cohort of patients affected by T1DM. Methods: A total of 4284 patients affected by T1DM followed-up at 76 diabetes centers participating to the Italian Association of Clinical Diabetologists (Associazione Medici Diabetologi, AMD) initiative constitutes the study population. Urinary albumin excretion (ACR) and estimated GFR (eGFR) were retrieved and analyzed. The incidence of stage 653 CKD (eGFR &lt; 60 mL/min/1.73 m2) or eGFR reduction &gt; 30% from baseline was evaluated. Results: The mean estimated GFR was 98 \ub1 17 mL/min/1.73m2 and the proportion of patients with albuminuria was 15.3% (n = 654) at baseline. About 8% (n = 337) of patients developed one of the two renal endpoints during the 4-year follow-up period. Age, albuminuria (micro or macro) and baseline eGFR &lt; 90 ml/min/m2 were independent risk factors for stage 653 CKD and renal function worsening. When compared to patients with eGFR &gt; 90 ml/min/1.73m2 and normoalbuminuria, those with albuminuria at baseline had a 1.69 greater risk of reaching stage 3 CKD, while patients with mild eGFR reduction (i.e. eGFR between 90 and 60 mL/min/1.73 m2) show a 3.81 greater risk that rose to 8.24 for those patients with albuminuria and mild eGFR reduction at baseline. Conclusions: Albuminuria and eGFR reduction represent independent risk factors for incident stage 653 CKD in T1DM patients.     Oxidative stress plays a pivotal role in the pathogenesis of several chronic diseases and antioxidants may represent potential tools for the prevention of these diseases. Here, we investigated the antioxidant efficiency of different tocotrienol isoforms (alpha-, delta-, gamma-tocotrienols), and that of FeAox-6, a novel synthetic compound which combines, by a stable covalent bond, the chroman head of vitamin E and a polyisoprenyl sequence of four conjugated double bonds into a single molecule. The antioxidant efficiency was evaluated as the ability of the compounds to inhibit lipid peroxidation, reactive oxygen species (ROS) production, heat shock protein (hsp) expression in rat liver microsomal membranes as well as in RAT-1 immortalized fibroblasts challenged with different free radical sources, including 2,2'-azobis(2-amidinopropane) (AAPH), tert-butyl hydroperoxide (tert-BOOH) and H2O2. Our results show that individual tocotrienols display different antioxidant potencies. Irrespective of the prooxidant used, the order of effectiveness was: delta-tocotrienol > gamma-tocotrienol = alpha-tocotrienol in both isolated membranes and intact cells. This is presumably due to the decreased methylation of delta-tocotrienol chromane ring, which allows the molecule to be more easily incorporated into cell membranes. Moreover, we found that FeAox-6 showed an antioxidant potency greater than that of delta-tocotrienol. Such an efficiency seems to depend on the concomitant presence of a chromane ring and a phytyl chain in the molecule, which because of four conjugated double bonds, may induce a greater mobility and a more uniform distribution within cell membrane. In view of these results, FeAox-6 represents a new potential preventive agent in chronic diseases in which oxidative stress plays a pathogenic role
