    Care staff perspectives of the role of music in the care of peopleliving with dementia

    The rationale for this study was grounded in the current emphasis on developing non-pharmacological interventions for people living with dementia in the UK. The study investigated the common practice amongst care staff caring for people living with dementia to commission music activities as a recreational pastime. It examined staff perspectives on the value of these activities and the potential to develop the scope of therapeutic benefits that might simultaneously support the well-being of both the cared-for and the care-giver. It investigated personal and professional factors that might underpin inhibitions to staff training to facilitate music activities themselves and to staff exploring music as an education tool to develop creative and reflective practice. The study sought to generate original findings and to contribute to an evidence base to inform future practice. Method triangulation involved a questionnaire survey, telephone and face-to-face interviews, focus group discussions and participant observations. One hundred and ninety two participants took part in the study, comprising 149 care staff and 13 music practitioners who collectively worked in one or more of 27 care settings for older people in east Kent, 27 older people and three informal carers. The results showed that musicians and music practitioners were typically engaged by staff to deliver a fairly similar range of activities, around ten times per year on average. Practical barriers to staff taking part in these activities centred on issues of time-tabling and the challenges of balancing routine care-giving with non-routine activities. Psychological barriers appeared to be influenced by: cultural backgrounds; staff experiences of music education at school; preconceptions about musical skill; personal levels of confidence; social pressures; inter-staff relationships; and the relationships that staff have with visiting musicians. Barriers to formal training for staff to explore music as a catalyst for creative and reflective practice also included low or non-prioritisation of music in relation to their care-giving role and a belief that only musical staff would benefit

    The therapeutic properties of music have been recognised since antiquity. There is now a growing evidence-base to support claims to its benefit for individuals with certain health conditions, including dementia. It has been reported that music interventions can lead to improvements in cognition, behaviour and psychosocial well-being in people with the condition, as well as offering support for carers. There are a variety of types of music interventions that can be used, and it is suggested that nurses consider harnessing music’s potential as part of the care they provide. This article explores the evidence-based use of music in dementia care and outlines its potential benefits

    Introduction Relationships between people with dementia and their carers can prove challenging over the trajectory of the disease. Interventions with a potential to address this include arts and music-based activities. This research project aimed to evaluate a community engagement programme (Carers Create) where both people with dementia and their carers participate together in singing and other activities. A specific focus was on the impact of the sessions on the dyadic relationship. Methods A grounded theory approach involved conducting three focus group interviews with carers of people with dementia (n=16) facilitated by members of a local U3A (University of the Third Age) who were trained and supported by university researchers. Recorded conversations were transcribed and analysed using a three-stage coding and thematic development technique. Results Four overarching themes were identified from the discourse: remembering the positive qualities of the cared-for; the physical and emotional demands of caring; Carers Create as a shared, beneficial activity; the enduring value of Carers Create. Conclusion Carers found the sessions to positively influence the relationship with the person they cared for through offering some relief from the day-to-day pressures of caring and in some cases restoring elements of a previously strong relationship. Crucial to the experience was the fact that the sessions included both carer and cared-for, offering activities to do together, and that they took place within a group, thereby offering a degree of mutual support. In addition, some carers were able to build on learning which had taken place and use certain techniques, such as singing, to help manage care, thus extending the improved relationship.

    Effects of a museum-based social-prescription intervention on quantitative measures of psychological wellbeing in older adults

    Aims: To assess psychological wellbeing in a novel social prescription intervention for older adults called Museums on Prescription, and to explore the extent of change over time in six self-rated emotions (‘absorbed, ‘active’, ‘cheerful’, ‘encouraged’, ‘enlightened’ and ‘inspired’). Methods: Participants (n=115) aged 65-94 were referred to museum-based programmes comprising 10, weekly sessions, by healthcare and third sector organisations using inclusion criteria (e.g. socially isolated; able to give informed consent; not in employment; not regularly attending social or cultural activities) and exclusion criteria (e.g. unable to travel to the museum; unable to function in a group situation; unlikely to be able to attend all sessions; unable to take part in interviews and complete questionnaires). In a within-participants design, the Museum Wellbeing Measure for Older Adults (MWM-OA) was administered pre-post session at start- mid- and end-programme. Twelve programmes, facilitated by museum staff and volunteers, were conducted in seven museums in central London and across Kent. In addition to the quantitative measures, participants, carers where present, museum staff and researchers kept weekly diaries following guideline questions, and took part in end programme in-depth interviews. Results: Multivariate analyses of variance showed significant participant improvements in all six MWM-OA emotions, pre-post session at start- mid- and end-programme. Two emotions, ‘absorbed’ and ‘enlightened’, increased pre-post session disproportionately to the others; ‘cheerful’ attained the highest pre-post session scores whereas ‘active’ was consistently lowest. Conclusions: Museums can be instrumental in offering museum-based programmes for older adults to improve psychological wellbeing over time. Participants in the study experienced a sense of privilege, valued the opportunity to liaise with curators, visit parts of the museum closed to the public, and handle objects normally behind glass. Participants appreciated opportunities afforded by creative and co-productive activities to acquire learning and skills, and get to know new people in a different context

    Community singing, wellbeing and older people: implementing and evaluating an English singing tool for health intervention in Rome

    Aim: The aim of this research was to explore the transferability and effectiveness of the English Silver Song Clubs model for older people in a different social and cultural context, i.e. in the capital city of Italy, Rome. Methods: A single condition, pre-test, post-test design was implemented. Participants completed two questionnaires: EQ-5D and York SF-12. Results: After the singing experience, participants showed a decrease in their levels of anxiety and depression. An improvement was also found from baseline to follow up in reported performance of usual activities. The English study showed a difference between the singing and non-singing groups at three and six months on mental health, and after three months on specific anxiety and depression measures. The current (Rome) study shows similar findings with an improvement on specific anxiety and depression items. Conclusions: Policy makers in different national contexts should consider social singing activities to promote the health and wellbeing of older adults as they are inexpensive to run and have been shown to be enjoyable and effective

    The role of social capital in participatory arts for wellbeing: findings from a qualitative systematic review

    BACKGROUND:Social capital is often cited as shaping impacts of participatory arts, although the concept has not been systematically mapped in arts, health and wellbeing contexts. In wider health inequalities research, complex, differential, and sometimes negative impacts of social capital have been recognised. METHODS:This paper maps of social capital concepts in qualitative research as part of the UK What Works for Wellbeing evidence review programme on culture, sport and wellbeing. RESULTS:Studies often cite positive impacts of bonding and, to a lesser extent, bridging social capital. However, reported challenges suggest the need for a critical approach. Forms of linking social capital, such as reframing and political engagement to address social divisions, are less often cited but may be important in participatory arts and wellbeing. CONCLUSIONS:Future research should further specify dimensions of social capital as well as their nuanced effects in arts, and wellbeing contexts

    A Search for Jet Handedness in Hadronic Z0Z^0 Decays

    We have searched for signatures of polarization in hadronic jets from Z0→qqˉZ^0 \to q \bar{q} decays using the ``jet handedness'' method. The polar angle asymmetry induced by the high SLC electron-beam polarization was used to separate quark jets from antiquark jets, expected to be left- and right-polarized, respectively. We find no evidence for jet handedness in our global sample or in a sample of light quark jets and we set upper limits at the 95% C.L. of 0.063 and 0.099 respectively on the magnitude of the analyzing power of the method proposed by Efremov {\it et al.}Comment: Revtex, 8 pages, 2 figure

    Measurement of the Charged Multiplicities in b, c and Light Quark Events from Z0 Decays

    Average charged multiplicities have been measured separately in bb, cc and light quark (u,d,su,d,s) events from Z0Z^0 decays measured in the SLD experiment. Impact parameters of charged tracks were used to select enriched samples of bb and light quark events, and reconstructed charmed mesons were used to select cc quark events. We measured the charged multiplicities: nˉuds=20.21±0.10(stat.)±0.22(syst.)\bar{n}_{uds} = 20.21 \pm 0.10 (\rm{stat.})\pm 0.22(\rm{syst.}), nˉc=21.28±0.46(stat.)−0.36+0.41(syst.)\bar{n}_{c} = 21.28 \pm 0.46(\rm{stat.}) ^{+0.41}_{-0.36}(\rm{syst.}) nˉb=23.14±0.10(stat.)−0.37+0.38(syst.)\bar{n}_{b} = 23.14 \pm 0.10(\rm{stat.}) ^{+0.38}_{-0.37}(\rm{syst.}), from which we derived the differences between the total average charged multiplicities of cc or bb quark events and light quark events: Δnˉc=1.07±0.47(stat.)−0.30+0.36(syst.)\Delta \bar{n}_c = 1.07 \pm 0.47(\rm{stat.})^{+0.36}_{-0.30}(\rm{syst.}) and Δnˉb=2.93±0.14(stat.)−0.29+0.30(syst.)\Delta \bar{n}_b = 2.93 \pm 0.14(\rm{stat.})^{+0.30}_{-0.29}(\rm{syst.}). We compared these measurements with those at lower center-of-mass energies and with perturbative QCD predictions.     What works for wellbeing in culture and sport? Report of a DELPHI process to support coproduction and establish principles and parameters of an evidence review

    Aims: There is a growing recognition of the ways in which culture and sport can contribute to wellbeing. A strong evidence base is needed to support innovative service development and a 3-year research programme is being undertaken to capture best evidence of wellbeing impacts and outcomes of cultural and sporting activities in order to inform UK policy and practice. This article provides an overview of methods and findings from an initial coproduction process with key stakeholders that sought to explore and agree principles and parameters of the evidence review for culture, sport and wellbeing (CSW). Methods: A two-stage DELPHI process was conducted with a purposeful sample of 57 stakeholders between August and December 2015. Participants were drawn from a range of culture and sport organisations and included commissioners and managers, policy makers, representatives of service delivery organisations (SDOs) and scholars. The DELPHI 1 questionnaire was developed from extensive consultation in July and August 2015. It explored definitions of wellbeing, the role of evidence, quality assessment, and the culture and sport populations, settings and interventions that are most likely to deliver wellbeing outcomes. Following further consultation, the results, presented as a series of ranked statements, were sent back to participants (DELPHI 2), which allowed them to reflect on and, if they wished, express agreement or disagreement with the emerging consensus. Results: A total of 40 stakeholders (70.02%) responded to the DELPHI questionnaires. DELPHI 1 mapped areas of agreement and disagreement, confirmed in DELPHI 2. The exercise drew together the key priorities for the CSW evidence review. Conclusion: The DELPHI process, in combination with face-to-face deliberation, enabled stakeholders to engage in complex discussion and express nuanced priorities while also allowing the group to come to an overall consensus and agree outcomes. The results will inform the CSW evidence review programme until its completion in March 2018
