Minor injuries as a risk factor for venous thrombosis
BACKGROUND: Injuries increase the risk of venous thrombosis. So far, most research has focused on major injuries that are accompanied by other risk factors for venous thrombosis, such as plaster casts and surgery. We studied the association of venous thrombosis with common minor injuries, such as minor sural muscle ruptures and ankle sprains.\ud

METHODS: We performed a large, population-based, case-control study (the Multiple Environmental and Genetic Assessment of risk factors for venous thrombosis [MEGA] study), including consecutive patients with a first deep venous thrombosis of the leg or pulmonary embolism and control subjects. Participants with malignant neoplasms, those who underwent surgery, and those who had a plaster cast or extended bed rest were excluded.\ud

RESULTS: Of 2471 patients, 289 (11.7%), and of 3534 controls, 154 (4.4%) had a minor injury in the 3 months preceding the venous thrombosis (patients) or completion of the questionnaire (controls). Venous thrombosis was associated with previous minor injury (odds ratio adjusted for sex and age, 3.1; 95% confidence interval, 2.5-3.8). The association was strongest for injuries that occurred in the 4 weeks before thrombosis and was not apparent before 10 weeks. Thrombosis was more strongly associated with minor injuries located in the leg (odds ratio adjusted for sex and age, 5.1; 95% confidence interval, 3.9-6.7), while those located in other body parts were not associated. A 50-fold increased risk was found in factor V Leiden carriers with a leg injury compared with noncarriers without injury (odds ratio, 49.7; 95% confidence interval, 6.8-362.7).\ud

CONCLUSIONS: Minor injuries in the leg are associated with greater risk of venous thrombosis. Because minor injuries are common, they could be major contributors to the occurrence of venous thrombosis\u
Bestandsopname van mosselen op mosselkweekpercelen in de Waddenzee in december 2018
Een van de uitgangspunten bij de mosselzaadvisserij is dat de omvang van het mosselbestand in de Waddenzee en daarmee het voedselaanbod voor vogels niet minder is dan in een situatie waar niet wordt gekweekt en gevist. Dat betekent dat in het najaar een zekere hoeveelheid mosselen op de kweekpercelen in de Waddenzee aanwezig dient te zijn. Voor 2018 (peildatum 1 december) is deze hoeveelheid vastgesteld op 13,8 Miljoen kg netto versgewicht. Na de voorjaarsvisserij wordt met een bestandsopname een schatting gemaakt van het bestand op de kweekpercelen (de zogenaamde ‘starthoeveelheid’) en wordt vervolgens bijgehouden hoeveel mosselen afgevoerd worden en hoeveel mosselen erbij komen. Een tweede bestandsopname is bedoeld om te kunnen valideren of de eerder genoemde hoeveelheid mosselen aan het begin van de winter inderdaad op de percelen aanwezig is. Voorliggende rapportage betreft de bestandsopname van mosselen op percelen in de Waddenzee in december 2018, ter validatie van de voornoemde starthoeveelheid. Hiertoe zijn in de Waddenzee alle kweekpercelen of delen van kweekpercelen waar mosselen verwacht worden in december 2018 bemonsterd. Het totale bestand op de percelen in december 2018 is hiermee geschat op 91,5 miljoen kg versgewicht. Op basis van deze schatting kan geconcludeerd worden dat de benodigde minimale hoeveelheid mosselen op 1 december 2018 ruimschoots op de percelen aanwezig was

Er zijn in juni 2018 relatief weinig zeesterren en krabben aangetroffen op de percelen
HMG-CoA reductase inhibitors, other lipid-lowering medication, antiplatelet therapy, and the risk of venous thrombosis
Background: Statins [3-hydroxymethyl-3-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitors] and antiplatelet therapy reduce the risk of atherosclerotic disease. Besides a reduction of lipid levels, statins might also have antithrombotic and anti-inflammatory properties, and anti-platelet therapy reduces clot formation. We have studied the risk of venous thrombosis with use of statins, other lipid-lowering medication, and antiplatelet therapy. Materials and methods: Patients with a first episode of deep vein thrombosis in the leg or pulmonary embolism between March 1999 and September 2004 were included in a large population-based case–control study (MEGA study). Control subjects were partners of patients (53%) or recruited via a random-digit-dialing method (47%). Participants reported different all-medication use in a questionnaire. Results: Of 4538 patients, 154 used statins (3.3%), as did 354 of 5914 control subjects (5.7%). The use of statins [odds ratio (OR) 0.45; 95% confidence interval (CI) 0.36–0.56] but not other lipid-lowering medications (OR 1.22; 95% CI 0.62–2.43), was associated with a reduced venous thrombosis risk as compared with individuals who did not use any lipid-lowering medication, after adjustment for age, sex, body mass index, atherosclerotic disease, antiplatelet therapy and use of vitamin K antagonists. Different types and various durations of statin therapy were all associated with a decreased venous thrombosis risk. Antiplatelet therapy also reduced venous thrombosis risk (OR 0.56; 95% CI 0.42–0.74). However, sensitivity analyses suggested that this effect is most likely explained by a so-called ‘healthy user effect’. Simultaneous use of medication most strongly reduced venous thrombosis risk. Conclusion: These results suggest that the use of various types of statins is associated with a reduced risk of venous thrombosis, whereas antiplatelet therapy and other lipid-lowering medications are not.\u
Expression of rat liver cell membrane transporters for thyroid hormone in Xenopus laevis oocytes
The present study was conducted to explore the possible use of Xenopus
laevis oocytes for the expression cloning of cell membrane transporters
for iodothyronines. Injection of stage V-VI X. laevis oocytes with 23 ng
Wistar rat liver polyadenylated RNA (mRNA) resulted after 3-4 days in a
highly significant increase in [125I]T3 (5 nM) uptake from 6.4 +/- 0.8
fmol/oocyte x h in water-injected oocytes to 9.2 +/- 0.65 fmol/oocyte x h
(mean +/- SEM; n = 19). In contrast, [125I]T4 (4 nM) uptake was not
significantly stimulated by injection of total liver mRNA. T3 uptake
induced by liver mRNA was significantly inhibited by replacement of Na+ in
the incubation medium by choline+ or by simultaneous incubation with 1
microM unlabeled T3. In contrast, T3 uptake by water-injected oocytes was
not Na+ dependent. Fractionation of liver mRNA on a 6-20% sucrose gradient
showed that maximal stimulation of T3 uptake was obtained with mRNA of
0.8-2.1 kilobases (kb). In contrast to unfractionated mRNA, the 0.7- to
2.1-kb fraction also significantly stimulated transport of T4, and it was
found to induce uptake of T3 sulfate (T3S). Because T3S is a good
substrate for type I deiodinase (D1), 2.3 ng rat D1 complementary RNA
(cRNA) were injected either alone or together with 23 ng of the 0.8- to
2.1-kb fraction of rat liver mRNA. Compared with water-injected oocytes,
injection of D1 cRNA alone did not stimulate uptake of [125I]T3S (1.25
nM). T3S uptake in liver mRNA and D1 cRNA-injected oocytes was similar to
that in oocytes injected with mRNA alone, showing that transport of T3S is
independent of the metabolic capacity of the oocyte. Furthermore,
coinjection of liver mRNA and D1 cRNA strongly increased the production of
125I-, showing that the T3S taken up by the oocyte is indeed transported
to the cell interior. In conclusion, injection of rat liver mRNA into X.
laevis oocytes resulted in a stimulation of saturable, Na+-dependent T4,
T3 and T3S transport, indicating that rat liver contains mRNA(s) coding
for plasma membrane transporters for these iodothyronine derivatives
Experience with multiple control groups in a large population-based case–control study on genetic and environmental risk factors
We discuss the analytic and practical considerations in a large case–control study that had two control groups; the first control group consisting of partners of patients and the second obtained by random digit dialling (RDD). As an example of the evaluation of a general lifestyle factor, we present body mass index (BMI). Both control groups had lower BMIs than the patients. The distribution in the partner controls was closer to that of the patients, likely due to similar lifestyles. A statistical approach was used to pool the results of both analyses, wherein partners were analyzed with a matched analysis, while RDDs were analyzed without matching. Even with a matched analysis, the odds ratio with partner controls remained closer to unity than with RDD controls, which is probably due to unmeasured confounders in the comparison with the random controls as well as intermediary factors. However, when studying injuries as a risk factor, the odds ratio remained higher with partner control subjects than with RRD control subjects, even after taking the matching into account. Finally we used factor V Leiden as an example of a genetic risk factor. The frequencies of factor V Leiden were identical in both control groups, indicating that for the analyses of this genetic risk factor the two control groups could be combined in a single unmatched analysis. In conclusion, the effect measures with the two control groups were in the same direction, and of the same order of magnitude. Moreover, it was not always the same control group that produced the higher or lower estimates, and a matched analysis did not remedy the differences. Our experience with the intricacies of dealing with two control groups may be useful to others when thinking about an optimal research design or the best statistical approach
Effectiveness of YouRAction, an Intervention to Promote Adolescent Physical Activity Using Personal and Environmental Feedback: A Cluster RCT
Background: In this study the one and six months effects of the computer-tailored YouRAction (targeting individual level determinants) and YouRAction+e (targeting in addition perceived environmental determinants) on compliance with the moderate-to-vigorous physical activity (MVPA) guideline and weight status are examined. In addition the use and appreciation of both interventions are studied. Methods: A three-armed cluster randomized trial was conducted in 2009-2010 with measurements at baseline, one and six months post intervention. School classes were assigned to one of the study arms (YouRaction, YouRAction+e and Generic Information (GI) control group). MVPA was derived from self-reports at baseline, one and six months post intervention. Body Mass Index and waist circumference were measured at baseline and six months post intervention in a random sub-sample of the population. Use of the interventions was measured by webserver logs and appreciation by self-reports. Multilevel regression analyses were conducted to study the effects of the intervention against the GI control group. ANOVA's and chi-square tests were used to describe differences in use and appreciation between study arms. Results: There were no statistically significant intervention effects on compliance with the MVPA guideline, overweight or WC. Access to the full intervention was significantly lower for YouRAction (24.0%) and YouRAction+e (21.7%) compared to the GI (54.4%). Conclusion: This study could not demonstrate that the YouRAction and YouRAction+e interventions were effective in promoting MVPA or improve anthropometric outcomes among adolescents, compared to generic information. Insufficient use and exposure to the intervention content may be an explanation for the lack of effects

Background: Nicotine dependence during adolescence increases the risk of continuing smoking into adulthood. The magnitude of nicotine dependence among adolescents in the European Union (EU) has not been established. We aimed to estimate the number of nicotine dependent 15-year-old adolescents in the EU, and identify high-risk groups. Methods: The number of nicotine dependent 15-year-olds in the EU was derived combining: (i) total number of 15-year-olds in the EU (2013 Eurostat), (ii) smoking prevalence among 15-year-olds (2013/2014 HBSC survey) and (iii) proportion of nicotine dependent 15-year-olds in six EU countries (2013 SILNE survey). Logistic regression analyses identified high-risk groups in the SILNE dataset. Results: We estimated 172 636 15-year-olds were moderately to highly nicotine dependent (3.2% of all 15 years old; 35.3% of daily smokers). In the total population, risk of nicotine dependence was higher in males, adolescents with poor academic achievement, and those with smoking parents or friends. Among daily smokers, only lower academic achievement and younger age of smoking onset were associated with nicotine dependence. Conclusion: According to our conservative estimates, more than 172 000 15-year-old EU adolescents were nicotine dependent in 2013. Prevention of smoking initiation, especially among adolescents with poor academic performance, is necessary to prevent a similar number of adolescents getting addicted to nicotine each consecutive year.publishersversionpublishe
