The Opportunity of Point-of-Care Diagnostics in General Practice:Modelling the Effects on Antimicrobial Resistance
OBJECTIVES: Antimicrobial resistance (AMR) is a public health threat associated with antibiotic consumption. Community-acquired acute respiratory tract infections (CA-ARTIs) are a major driver of antibiotic consumption in primary care. We aimed to quantify the investments required for a large-scale rollout of point-of care (POC) diagnostic testing in Dutch primary care, and the impact on AMR due to reduced use of antibiotics. METHODS: We developed an individual-based model that simulates consultations for CA-ARTI at GP practices in the Netherlands and compared a scenario where GPs test all CA-ARTI patients with a hypothetical diagnostic strategy to continuing the current standard-of-care for the years 2020-2030. We estimated differences in costs and future AMR rates caused by testing all patients consulting for CA-ARTI with a hypothetical diagnostic strategy, compared to the current standard-of-care in GP practices. RESULTS: Compared to the current standard-of-care, the diagnostic algorithm increases the total costs of GP consultations for CA-ARTI by 9% and 19%, when priced at âŹ5 and âŹ10, respectively. The forecast increase in Streptococcus pneumoniae resistance against penicillins can be partly restrained by the hypothetical diagnostic strategy from 3.8 to 3.5% in 2030, albeit with considerable uncertainty. CONCLUSIONS: Our results show that implementing a hypothetical diagnostic strategy for all CA-ARTI patients in primary care raises the costs of consultations, while lowering antibiotic consumption and AMR. Novel health-economic methods to assess and communicate the potential benefits related to AMR may be required for interventions with limited gains for individual patients, but considerable potential related to antibiotic consumption and AMR
Distinct RNA profiles in subpopulations of extracellular vesicles: apoptotic bodies, microvesicles and exosomes
Introduction: In recent years, there has been an exponential increase in the number of studies aiming to understand the biology of exosomes, as well as other extracellular vesicles. However, classification of membrane vesicles and the appropriate protocols for their isolation are still under intense discussion and investigation. When isolating vesicles, it is crucial to use systems that are able to separate them, to avoid cross-contamination. Method: EVs released from three different kinds of cell lines: HMC-1, TF-1 and BV-2 were isolated using two centrifugation-based protocols. In protocol 1, apoptotic bodies were collected at 2,000×g, followed by filtering the supernatant through 0.8 µm pores and pelleting of microvesicles at 12,200×g. In protocol 2, apoptotic bodies and microvesicles were collected together at 16,500×g, followed by filtering of the supernatant through 0.2 µm pores and pelleting of exosomes at 120,000×g. Extracellular vesicles were analyzed by transmission electron microscopy, flow cytometry and the RNA profiles were investigated using a Bioanalyzer®. Results: RNA profiles showed that ribosomal RNA was primary detectable in apoptotic bodies and smaller RNAs without prominent ribosomal RNA peaks in exosomes. In contrast, microvesicles contained little or no RNA except for microvesicles collected from TF-1 cell cultures. The different vesicle pellets showed highly different distribution of size, shape and electron density with typical apoptotic body, microvesicle and exosome characteristics when analyzed by transmission electron microscopy. Flow cytometry revealed the presence of CD63 and CD81 in all vesicles investigated, as well as CD9 except in the TF-1-derived vesicles, as these cells do not express CD9. Conclusions: Our results demonstrate that centrifugation-based protocols are simple and fast systems to distinguish subpopulations of extracellular vesicles. Different vesicles show different RNA profiles and morphological characteristics, but they are indistinguishable using CD63-coated beads for flow cytometry analysis
Uniform tiling with electrical resistors
The electric resistance between two arbitrary nodes on any infinite lattice
structure of resistors that is a periodic tiling of space is obtained. Our
general approach is based on the lattice Green's function of the Laplacian
matrix associated with the network. We present several non-trivial examples to
show how efficient our method is. Deriving explicit resistance formulas it is
shown that the Kagom\'e, the diced and the decorated lattice can be mapped to
the triangular and square lattice of resistors. Our work can be extended to the
random walk problem or to electron dynamics in condensed matter physics.Comment: 22 pages, 14 figure
Lagrangian Reachabililty
We introduce LRT, a new Lagrangian-based ReachTube computation algorithm that
conservatively approximates the set of reachable states of a nonlinear
dynamical system. LRT makes use of the Cauchy-Green stretching factor (SF),
which is derived from an over-approximation of the gradient of the solution
flows. The SF measures the discrepancy between two states propagated by the
system solution from two initial states lying in a well-defined region, thereby
allowing LRT to compute a reachtube with a ball-overestimate in a metric where
the computed enclosure is as tight as possible. To evaluate its performance, we
implemented a prototype of LRT in C++/Matlab, and ran it on a set of
well-established benchmarks. Our results show that LRT compares very favorably

Exploring functional strength changes during nusinersen treatment in symptomatic children with SMA types 2 and 3
The Hammersmith Functional Motor Scale-Expanded (HFMSE) is a validated outcome measure for monitoring changes in functional strength in patients with spinal muscular atrophy (SMA). The objective of this study was to explore changes in HFMSE item-scores in children with SMA types 2 and 3a treated with nusinersen over a period of six to twenty months. We stratified patients according to motor ability (sitting and walking), and calculated numbers and percentages for each specific improvement (positive score change) or decrease (negative score change) for the total group and each subgroup and calculated frequency distributions of specific score changes. Ninety-one percent of the children showed improvement in at least 1 item, twenty-eight percent showed a score decrease in 1 or more items. In the first six to twenty months of nusinersen treatment motor function change was characterized by the acquisition of the ability to perform specific tasks with compensation strategies (score changes from 0 to 1). Children with the ability to sit were most likely to improve in items that assess rolling, whilst children with the ability to walk most likely improved in items that assess half-kneeling. The ability most frequently lost was hip flexion in supine position
Whole Body CT Imaging in Deceased Donor Screening for Malignancies
Background. In most western countries, the median donor age is increasing. The incidence of malignancies in older
populations is increasing as well. To prevent donor-derived malignancies we evaluated radiologic donor screening in a retrospective donor cohort. Methods. This study analyzes the efficacy of a preoperative computed tomography (CT) scan
on detecting malignancies. All deceased organ donors in the Netherlands between January 2013 and December 2017
were included. Donor reports were analyzed to identify malignancies detected before or during organ procurement. Findings
between donor screening with or without CT-scan were compared. Results. Chest or abdominal CT-scans were performed in 17% and 18% of the 1644 reported donors respectively. Screening by chest CT-scan versus radiograph resulted
in 1.5% and 0.0% detected thoracic malignancies respectively. During procurement no thoracic malignancies were found in
patients screened by chest CT compared with 0.2% malignancies in the radiograph group. Screening by abdominal CT-scan
resulted in 0.0% malignancies, compared with 0.2% in the abdominal ultrasound group. During procurement 1.0% and 1.3%
malignancies were found in the abdominal CT-scan and ultrasound groups, respectively. Conclusions. Screening by
CT-scan decreased the perioperative detection of tumors by 30%. A preoperative CT-scan may be helpful by providing additional information on (aberrant) anatomy to the procuring or transplanting surgeon. In conclusion, donor screening by CT-scan
could decrease the risk of donor-derived malignancies and prevents unnecessary procurements per year in the Netherlands
Phase-Induced (In)-Stability in Coupled Parametric Oscillators
We report results on a model of two coupled oscillators that undergo periodic
parametric modulations with a phase difference . Being to a large
extent analytically solvable, the model reveals a rich dependence of
the regions of parametric resonance. In particular, the intuitive notion that
anti-phase modulations are less prone to parametric resonance is confirmed for
sufficiently large coupling and damping. We also compare our results to a
recently reported mean field model of collective parametric instability,
showing that the two-oscillator model can capture much of the qualitative

âThis battle, between your gut feeling and your mind. Try to find the right balanceâ: Parental experiences of children with spinal muscular atrophy during COVID-19 pandemic
Aims: Parents of children with spinal muscular atrophy (SMA) often struggle with the all-consuming nature of the demands of caring for a child with substantial physical needs. Our aim was to explore experiences, challenges and needs of parents of a child with SMA in a COVID-19 pandemic situation. Method: Nineteen parents of 21 children (15 months to 13 years of age) with SMA types 1â3 participated in semi-structured interviews in June to July 2020. The interviews were analysed using inductive thematic analysis. Results: Parents mentioned the protection of the health and well-being of the child as the central perspective and driving force during the COVID-19 pandemic. Three subthemes were identified: (1) responsibility, (2) balancing vulnerability and resilience and (3) (in)security. Some parents focused on the positive aspects during the lockdown, such as continuation of nusinersen treatment and family life. Some parents described helpful and positive cognitions to cope with the situation. In general, parents described a need for information with regard to COVID-19 and their child with SMA and a need for discussing their dilemmas and insecurities with a healthcare professional. Interpretation: Parents put the health and well-being of their children first during the pandemic. From this study, we learned that parents of children with SMA need information and value direct contact with a healthcare professional to share their dilemmas and insecurities. The dialogue can help to empower parents in the conflicts and decisions they have to make during a pandemic
Wartenbergâs migrant sensory neuritis: a prospective follow-up study
Migrant sensory neuropathy (Wartenbergâs migrant sensory neuritis) is characterized by sudden numbness in the distribution of one or multiple cutaneous nerves. To study disease course and outcome, we prospectively followed 12 patients who presented to our tertiary referral neuromuscular outpatient clinic between January 2003 and January 2004. Medical history, neurological, laboratory and electrophysiological examinations were obtained from all patients. All patients were reviewed a second time in 2007, and five had a follow-up electrophysiological examination. At the first visit, 50% described an episode of stretching preceding the sensory complaints. All but three described pain in the affected area before or concomitant with sensory loss. At clinical examination a median of six skin areas were affected, and in 75% this could be confirmed by nerve conduction studies in at least one nerve. Forty-two percent had involvement of the trigeminal nerve. After a mean disease duration of 7.5Â years, three patients reported a complete disappearance of sensory complaints and five that the pain had disappeared, but numbness remained. Three patients still had both painful and numb sensory deficits. One patient developed a distal symmetric sensory polyneuropathy. In conclusion, Wartenbergâs sensory neuritis is a distinct, exclusively sensory, neuropathy, marked by pain preceding numbness in affected nerves. An episode of stretching preceding pain is not necessary for the diagnosis. Wartenbergâs sensory neuritis often retains its spotty, exclusively sensory characteristics after long term follow-up
