    Contamination of firefighter personal protective equipment and skin and the effectiveness of decontamination procedures

    Firefighters’ skin may be exposed to chemicals via permeation/penetration of combustion byproducts through or around personal protective equipment (PPE) or from the cross-transfer of contaminants on PPE to the skin. Additionally, volatile contaminants can evaporate from PPE following a response and be inhaled by firefighters. Using polycyclic aromatic hydrocarbons (PAHs) and volatile organic compounds (VOCs) as respective markers for non-volatile and volatile substances, we investigated the contamination of firefighters’ turnout gear and skin following controlled residential fire responses. Participants were grouped into three crews of twelve firefighters. Each crew was deployed to a fire scenario (one per day, four total) and then paired up to complete six fireground job assignments.Wipe sampling of the exterior of the turnout gear was conducted pre- and post-fire.Wipe samples were also collected from a subset of the gear after field decontamination. VOCs off-gassing from gear were also measured pre-fire, post-fire, and post-decon. Wipe sampling of the firefighters’ hands and neck was conducted pre- and post-fire. Additional wipes were collected after cleaning neck skin. PAH levels on turnout gear increased after each response and were greatest for gear worn by firefighters assigned to fire attack and to search and rescue activities. Field decontamination using dish soap, water, and scrubbing was able to reduce PAH contamination on turnout jackets by a median of 85%. Off-gassing VOC levels increased post-fire and then decreased 17–36 min later regardless of whether field decontamination was performed. Median post-fire PAH levels on the neck were near or below the limit of detection (<24 micrograms per square meter [μg/m2]) for all positions. For firefighters assigned to attack, search, and outside ventilation, the 75th percentile values on the neck were 152, 71.7, and 39.3 μg/m2, respectively. Firefighters assigned to attack and search had higher post-fire median hand contamination (135 and 226 μg/m2, respectively) than other positions (<10.5 μg/m2). Cleansing wipes were able to reduce PAH contamination on neck skin by a median of 54%.This study was funded through a U.S. Department of Homeland Security, Assistance to Firefighters Grant (EMW-2013- FP-00766).     The ANTARES telescope is well-suited for detecting astrophysical transient neutrino sources as it can observe a full hemisphere of the sky at all times with a high duty cycle. The background due to atmospheric particles can be drastically reduced, and the point-source sensitivity improved, by selecting a narrow time window around possible neutrino production periods. Blazars, being radio-loud active galactic nuclei with their jets pointing almost directly towards the observer, are particularly attractive potential neutrino point sources, since they are among the most likely sources of the very high-energy cosmic rays. Neutrinos and gamma rays may be produced in hadronic interactions with the surrounding medium. Moreover, blazars generally show high time variability in their light curves at different wavelengths and on various time scales. This paper presents a time-dependent analysis applied to a selection of flaring gamma-ray blazars observed by the FERMI/LAT experiment and by TeV Cherenkov telescopes using five years of ANTARES data taken from 2008 to 2012. The results are compatible with fluctuations of the background. Upper limits on the neutrino fluence have been produced and compared to the measured gamma-ray spectral energy distribution.Comment: 27 pages, 16 figure

    Oral anticoagulation in device-detected atrial fibrillation: effects of age, sex, cardiovascular comorbidities, and kidney function on outcomes in the NOAH-AFNET 6 trial.

    Implanted pacemakers, defibrillators, and loop recorders detect short and rare episodes of device-detected atrial fibrillation [DDAF, previously also called atrial high-rate episodes or subclinical atrial fibrillation (AF)] in ∼30% of patients. Device-detected atrial fibrillation rarely has therapeutic consequences in patients with ECG-documented AF. Device-detected atrial fibrillation without ECG-documented AF can lead to consideration of oral anticoagulation in clinical practice, especially in older patients with multiple stroke risk factors and/or very long DDAF episodes, largely based on observational data. Two recent controlled trials, NOAH-AFNET 6 and ARTESiA, observed a low rate of ischaemic stroke without anticoagulation (1.1%–1.2%/patient-year) in patients with DDAF and stroke risk factors, including in patients with very long DDAF episodes in NOAH-AFNET 6. Current guidelines leave the decision to anticoagulate to clinical judgement, balancing the expected stroke risk, typically estimated by using stroke risk scores developed in patients with ECG-documented AF, and the stroke risk reduction induced by anticoagulation, with the increase in bleeding associated with anticoagulation therapy

    According to the concept 'Safety investigations on a probabilistic basis for embankment dams' which has been worked out by the DGEG in 1988, a reliability analysis for the mechanical and hydraulic behaviour of the 'structure-bedrock' system considering potential failure mechanisms is carried out for the dam 'Schmalwasser' (Thueringen). For the soil and rock mechanical parameters used for the reliability investigation, the statistic parameters are determined with the help of random samples or are assessed from comparable geotechnical problems when the amount of random samples is too little. Geometric parameters are measured deterministically. For the rock mass a structural model is worked out which is based upon statistically determined parameters and on the geological expertise. On the basis of models for the stress-strain behaviour and the permeability of jointed rock which have been developed at the Institute for Foundation Engineering, Soil Mechanics and Waterways Construction of Aachen Technical University and which are implemented in the finite-element-programs FEST03 and HYD03, two- and threedimensional stability investigations for the dam are carried out. Failure mechanisms for example slope failure are investigated with the help of conventional stability analyses. The probability of dam failure for every single failure mechanism is determined according to the first order reliability method. In the reliability analysis the dam failure mechanisms are combined to sequences of events. The overall probability of dam failure is determined by the combination of the failure spaces of all sequences of events.
