    Climate Change and the New Normal for Cardiorespiratory Disease

    Climate change is already affecting the cardiorespiratory health of populations around the world, and these impacts are expected to increase. The present overview serves as a primer for respirologists who are concerned about how these profound environmental changes may affect their patients. The authors consider recent peer-reviewed literature with a focus on climate interactions with air pollution. They do not discuss in detail cardiorespiratory health effects for which the potential link to climate change is poorly understood. For example, pneumonia and influenza, which affect >500 million people per year, are not addressed, although clear seasonal variation suggests climate-related effects. Additionally, large global health impacts in low-resource countries, including migration precipitated by environmental change, are omitted. The major cardiorespiratory health impacts addressed are due to heat, air pollution and wildfires, shifts in allergens and infectious diseases along with respiratory impacts from flooding. Personal and societal choices about carbon use and fossil energy infrastructure should be informed by their impacts on health, and respirologists can play an important role in this discussion

    This is a case study describing how climate change may affect the health of British Columbians and to suggest a way forward to promote health and policy research, and adaptation to these changes. After reviewing the limited evidence of the impacts of climate change on human health we have developed five principles to guide the development of research and policy to better predict future impacts of climate change on health and to enhance adaptation to these change in BC. We suggest that, with some modification, these principles will be useful to policy makers in other jurisdictions

    Background Mortality estimates can measure and monitor the impacts of conflict on a population, guide humanitarian efforts, and help to better understand the public health impacts of conflict. Vital statistics registration and surveillance systems are rarely functional in conflict settings, posing a challenge of estimating mortality using retrospective population-based surveys. Results We present a two-stage cluster sampling method for application in population-based mortality surveys. The sampling method utilizes gridded population data and a geographic information system (GIS) to select clusters in the first sampling stage and Google Earth TM imagery and sampling grids to select households in the second sampling stage. The sampling method is implemented in a household mortality study in Iraq in 2011. Factors affecting feasibility and methodological quality are described. Conclusion Sampling is a challenge in retrospective population-based mortality studies and alternatives that improve on the conventional approaches are needed. The sampling strategy presented here was designed to generate a representative sample of the Iraqi population while reducing the potential for bias and considering the context specific challenges of the study setting. This sampling strategy, or variations on it, are adaptable and should be considered and tested in other conflict settings

    Aeroallergens in Canada: Distribution, Public Health Impacts, and Opportunities for Prevention

    Aeroallergens occur naturally in the environment and are widely dispersed across Canada, yet their public health implications are not well-understood. This review intends to provide a scientific and public health-oriented perspective on aeroallergens in Canada: their distribution, health impacts, and new developments including the effects of climate change and the potential role of aeroallergens in the development of allergies and asthma. The review also describes anthropogenic effects on plant distribution and diversity, and how aeroallergens interact with other environmental elements, such as air pollution and weather events. Increased understanding of the relationships between aeroallergens and health will enhance our ability to provide accurate information, improve preventive measures and provide timely treatments for affected populations

    The effect of portable HEPA filter air cleaners on indoor PM2.5 concentrations and second hand tobacco smoke exposure among pregnant women in Ulaanbaatar, Mongolia: The UGAAR randomized controlled trial

    Background Portable HEPA filter air cleaners can reduce indoor fine particulate matter (PM2.5), but their use has not been adequately evaluated in high pollution settings. We assessed air cleaner effectiveness in reducing indoor residential PM2.5 and second hand smoke (SHS) exposures among non-smoking pregnant women in Ulaanbaatar, Mongolia. Methods We randomized 540 participants to an intervention group receiving 1 or 2 HEPA filter air cleaners or a control group receiving no air cleaners. We followed 259 intervention and 253 control participants to the end of pregnancy. We measured one-week indoor residential PM2.5 concentrations in early (~11weeks gestation) and late (~31weeks gestation) pregnancy and collected outdoor PM2.5 data from centrally-located government monitors. We assessed blood cadmium in late pregnancy. Hair nicotine was quantified in a subset (n=125) to evaluate blood cadmium as a biomarker of SHS exposure. We evaluated air cleaner effectiveness using mixed effects and multiple linear regression models and used stratified models and interaction terms to evaluate potential modifiers of effectiveness. Results The overall geometric mean (GM) one-week outdoor PM2.5 concentration was 47.9?g/m3 (95% CI: 44.6, 51.6?g/m3), with highest concentrations in winter (118.0?g/m3; 110.4, 126.2?g/m3). One-week indoor and outdoor PM2.5 concentrations were correlated (r=0.69). Indoor PM2.5 concentrations were 29% (21, 37%) lower in intervention versus control apartments, with GMs of 17.3?g/m3 (15.8, 18.8?g/m3) and 24.5?g/m3 (22.2, 27.0?g/m3), respectively. Air cleaner effectiveness was greater when air cleaners were first deployed (40%; 31, 48%) than after approximately five months of use (15%; 0, 27%). Blood cadmium concentrations were 14% (4, 23%) lower among intervention participants, likely due to reduced SHS exposure.     Comparison of Questionnaire Responses with Biomarkers of Tobacco Smoke Exposure in A Canadian Birth Cohort at Three Months of Age

    Background Exposure to tobacco smoke increases the risk for several adverse health effects in children including wheeze, asthma, and asthma exacerbation. Accurately assessing tobacco smoke exposure is important for understanding and preventing these health effects. Questionnaires are a flexible and relatively inexpensive method of assessing exposure, but biomarkers of tobacco smoke exposure are considered more accurate. We developed questionnaire-based exposure models predicting urinary levels of biomarkers cotinine and trans-3’-hydroxycotinine (3HC) (metabolites of nicotine) in 3-month old infants using parent-reported questionnaire responses about tobacco smoke exposure from the Canadian Healthy Infant Longitudinal Development (CHILD) Study. Methods We used a manual model building process to build multiple linear regression models predicting urinary concentrations of cotinine, 3HC, and the sum of cotinine and 3HC on a molar basis (Cot+3HC) for 987, 1003, and 983 infants, respectively. Questions were included on the infant’s exposure assessed at 3 months of age and tobacco smoke odour in the home. We also included questions on maternal smoking status and history, passive exposure, and family socio-economic status assessed during pregnancy, as potential indirect measures of the infant’s exposure at 3 months. Adjusted R2 values were maximized in the final models. Results During pregnancy, the prevalence of maternal smoking was 2.4 %, and 115 (11.4 %) mothers reported smoking by at least 1 person at home. Of the 144 (14.3 %) infants whose mothers reported that smoking occurred at home when their child was 3 months, 129 (89.6%) and 136 (94.4%) had cotinine and 3HC levels above the detection limit (0.03 ng/mL), respectively. Of the 811 infants who had no parent-reported exposure at 3 months, 538 (66.3%) and 715 (88.2%) had detectable cotinine and 3HC levels, respectively. After correcting for urine dilution, the geometric mean levels were 0.085 ng/mL for cotinine, 0.20 ng/mL for 3HC, and 1.62 picomole/mL for Cot+3HC. The final questionnaire models explained 43.4%, 41.0%, and 42.9% of the variance in cotinine, 3HC, and Cot+3HC levels, respectively. Conclusions Our results indicate that exposure of these infants to tobacco smoke is not completely captured by questionnaires, suggesting that exposure assessment could be improved by using a combination of biomarker and questionnaire methods. Though more detectable, the inclusion of 3HC did not increase the ability of the questionnaires to explain variance in metabolite levels, but 3HC may be important since the ratio of 3HC to cotinine can be used to quantify the rate of nicotine metabolism and variation within population
