    Brza progresija hronične limfocitne leukemije u Rihterov sindrom kod bolesnika sa kariotipom blizu triploidnog broja hromozom

    Introduction. The presence of aneuploidy in patients diagnosed with chronic lymphocytic leukemia (CLL), except trisomy 12, is considered quite uncommon. Hyperdiploidy or near-tetraploidy (occurring in 1–3% of all CLL patients) usually confer a poor prognosis. Case report. We report a patient in a progressive phase of CLL with near–triploid karyotype. The prognosis of the disease was more precisely determined by applying the cytogenetic analysis of the karyotype and was complemented with molecular methods and pathohistological examination. The complex karyotype was accompanied by the TP53, C-MYC, and IGH gene disruptions, the most probable cause of rapid evolution into Richter’s syndrome. Conclusion. The use of comprehensive contemporary diagnostic techniques is highly recommended in patients who are in the progressive phase of CLL, primarily for the adequate choice of management strategy. The presented case confirms that aneuploidy in CLL patients indicates poor prognosis, which is in accordance with previous publications reporting on cases of CLL patients with aneuploidy.Uvod. Prisustvo aneuploidije kod bolesnika sa dijagnozom hronične limfocitne leukemije (HLL), sa izuzetkom trizomije 12, smatra se retkom pojavom. Pojava hiperdiploidnog ili kariotipa blizu tetraploidnog broja hromozoma (koji se javlja kod 1–3% svih bolesnika sa HLL) smatra se lošim prognostičkim parametrom. Prikaz bolesnika. Prikazan je bolesnik u uznapredovaloj fazi HLL sa kariotipom blizu triploidnog broja hromozoma. Prognoza bolesti je preciznije određena citogenetičkom analizom kariotipa bolesnika, i dopunjena molekularnim metodama i patohistološkom analizom. Otkriveno je prisustvo kompleksnog kariotipa udruženog sa poremećajima u genima TP53, C-MYC i IGH, što je najverovatnije bio uzrok brze progresije u Rihterov sindrom. Zaključak. Primena savremenih dijagnostičkih metoda veoma je značajna kod bolesnika u uznapredovaloj fazi HLL, prvenstveno zbog adekvatnog terapijskog pristupa. Prikazani slučaj ukazuje da je prisustvo aneuploidije kod bolesnika sa HLL loš prognostički znak, što je u saglasnosti sa prethodno publikovanim prikazima bolesnika sa HLL i sa aneuploidijom u kariotipu

    Opstacles in neuropathic pain treatment in the developing country: Neuropathic pain treatment

    Introduction: Neuropathic pain is initiated or caused by a primary lesion or dysfunction in the peripheral or central nervous system. The central neuropathic pain includes thalamic pain syndrome, pain related to multiple sclerosis, poststroke pain, post spinal cord injury pain. Almost 60% of patients with spinal cord injuries have neuropathic pain whose treatment requires great socioeconomic support for a long time. Case Report: Male, 56 years old, previously healthy, was examined at the Outpatient pain Clinic, Clinical Centre of Serbia. He complained about a strong pain in both legs and a sense of tightening around the trunk. The pain was present on rest and getting worse on mobilization, intensity 7-10/10 by NRS. In 2008. after falling into the shallow water, he sustained comminuted C7 fracture resulting in quadriplegia. He was operated and regained motor skills in both arms with some residual sensory deficits. Detailed neurological examination diagnosed the presence of neuropathic pain in the trunk (T4-L1 dermatome being the worst pain with hyperalgesia, dysesthesia, hyperpathia), with sensitivity fairly preserved. Regarding therapy, only fentanyl patches helped with the dose increase but tightening sense persisted. Combined lidocaine and ketamine infusion was applied - the response was temporarily good. The patient still feels pain and uses fentanyl patches, baclofen and physical therapy. Discussion: Treatment of neuropathic pain represents a great socioeconomic problem even for developed countries because there is no benefit of conventional pharmacologic therapy in long-term use. Also, neuromodulatory therapy methods provide the most benefits for this group of patients but demand trained personnel and use of expensive equipment. Conclusion: This case is a good example of difficulties regarding treatment of this type of chronic pain in a developing country: limits of pharmacotherapy, no trained doctors for implantation of neuromodulatory devices, high price of neuromodulatory devices

    Uticaj položaja grana u cvastima na formiranje prinosa i kvalitet semena livadskog vijuka (Festuca pratensis Huds.) i visokog vijuka (Festuca arundinacea Schreb.)

    During two investigation years two trials with meadow fescue (Festuca pratensis Huds.), cultivar K21 and tall fescue (Festuca arundinacea Schreb.), cultivar K20 have been performed. Analysis of upper, intermediate and lower panicle branches from generative shots has showed their influence on determining of yield and quality of seed. Lower and intermediate panicle branches have showed higher importance in seed yield determining than upper branches in both species. Influence of panicle branches position on 1000 seed weight has been more variable in meadow fescue (CV(%)=8,41), while in tall fescue variability for germination energy and total germination have been larger (CV(%)=11,05 and 2,26 respectively).Tokom dvogodišnjeg istraživanja (2006-2007) izvedeni su ogledi sa livadskim vijukom (Festuca pratensis Huds.), sorta K21, i visokim vijukom (Festuca arundinacea Schreb.), sorta K20. Uzimanjem gornjih, srednjih i donjih grana iz cvasti (metlice) generativnih stabljika utvrđen je njihov uticaj na formiranje prinosa i kvalitet semena. Kod obe ispitivane vrste donje i srednje grane iz cvasti su ispoljile daleko veći značaj u formiranju prinosa semena u odnosu na gornje grane. U pogledu kvaliteta semena, uticaj položaja grana na masu 1000 semena pokazao je veću varijabilnost kod livadskog vijuka, (CV(%) = 8,41), dok je visoki vijuk ispoljio veću varijabilnost za energiju klijanja semena i ukupnu klijavost (CV(%) = 11,05 i CV(%) = 2,26)

    Iron salt-promoted oxidation of steroidal phenols by m -chloroperbenzoic acid: a route to possible antitumor agents

    Iron salt-promoted reaction of estrone and its derivatives with meta -chloroperoxybenzoic acid was developed and epoxyquinols were further transformed. Most compounds showed in vitro antiproliferative activity. , A new oxidant, containing m -chloroperoxybenzoic acid (MCPBA) and an iron salt, was developed and used for oxidation of steroidal phenols to a quinol/epoxyquinol mixture. Reaction was optimized for estrone, by varying initiators (Fe-salts), reaction temperature, time and mode of MCPBA application. A series of five more substrates (17β-estradiol and its hydrophobized derivatives) was subjected to the optimized oxidation, providing corresponding p -quinols and 4β,5β-epoxyquinols in good to moderate yields. The obtained epoxyquinols were additionally transformed by oxidation, as well as the acid-catalyzed oxirane opening.
