Wearable devices for remote vital signs monitoring in the outpatient setting: an overview of the field
Early detection of physiological deterioration has been shown to improve patient outcomes. Due to recent improvements in technology, comprehensive outpatient vital signs monitoring is now possible. This is the first review to collate information on all wearable devices on the market for outpatient physiological monitoring.
A scoping review was undertaken. The monitors reviewed were limited to those that can function in the outpatient setting with minimal restrictions on the patient’s normal lifestyle, while measuring any or all of the vital signs: heart rate, ECG, oxygen saturation, respiration rate, blood pressure and temperature.
A total of 270 papers were included in the review. Thirty wearable monitors were examined: 6 patches, 3 clothing-based monitors, 4 chest straps, 2 upper arm bands and 15 wristbands. The monitoring of vital signs in the outpatient setting is a developing field with differing levels of evidence for each monitor. The most common clinical application was heart rate monitoring. Blood pressure and oxygen saturation measurements were the least common applications. There is a need for clinical validation studies in the outpatient setting to prove the potential of many of the monitors identified.
Research in this area is in its infancy. Future research should look at aggregating the results of validity and reliability and patient outcome studies for each monitor and between different devices. This would provide a more holistic overview of the potential for the clinical use of each device
A Review on the Scope of Photothermal Therapy–Based Nanomedicines in Preclinical Models of Colorectal Cancer
Oncologic thermal ablation involves the use of hyperthermic temperatures to damage and treat solid cancers. Thermal ablation is being investigated as a method of treatment in colorectal cancers and has the potential to complement conventional anticancer treatments in managing local recurrence and metastatic disease. Photothermal therapy utilizes photosensitive agents to generate local heat and induce thermal ablation. There is growing interest in developing nanotechnology platforms to deliver such photosensitive agents. An advantage of nanomedicines is their multifunctionality, with the capability to deliver combinations of chemotherapeutics and cancer-imaging agents. To date, there have been no clinical studies evaluating photothermal therapy–based nanomedicines in colorectal cancers. This review presents the current scope of preclinical studies, investigating nanomedicines that have been developed for delivering multimodal photothermal therapy to colorectal cancers, with an emphasis on potential clinical applications
The role of ABCG2 in modulating responses to anti-cancer photodynamic therapy
The ATP-binding cassette (ABC) superfamily G member 2 (ABCG2) transmembrane protein transporter is known for conferring resistance to treatment in cancers. Photodynamic therapy (PDT) is a promising anti-cancer method involving the use of light-activated photosensitisers to precisely induce oxidative stress and cell death in cancers. ABCG2 can efflux photosensitisers from out of cells, reducing the capacity of PDT and limiting the efficacy of treatment. Many studies have attempted to elucidate the relationship between the expression of ABCG2 in cancers, its effect on the cellular retention of photosensitisers and its impact on PDT. This review looks at the studies which investigate the effect of ABCG2 on a range of different photosensitisers in different pre-clinical models of cancer. This work also evaluates the approaches that are being investigated to address the role of ABCG2 in PDT with an outlook on potential clinical validation
Adaptation and validation of the Inventory of family protective factors for the portuguese culture
Aim: Describe the process of cultural adaptation and validation of Inventory of Family Protective Factors (IFPF) for portuguese culture. This instrument assesses the protective factors that contribute to family resilience. Studies of resilience fall the salutogenic paradigm, which focuses on protective factors of individuals or groups, without minimizing the risk factors and vulnerability. Methods: We applied this instrument to 85 families of children with special needs and, after linguistic and conceptual equivalence, used an exploratory factor analysis with principal components analysis (with varimax rotation) and calculated the Cronbach's alpha coefficient for each dimension. Results: adequate psychometric properties to be used in Portuguese population (Cronbach´s alpha =.90). Conclusion: IFPF is an useful instrument for studies which propose assess the protective factors of family resilience, however we suggest further studies of revalidation.Objetivo: Describir el proceso de adaptaciĂłn cultural y validaciĂłn para la cultura portuguesa de Inventory of Family Protective Factors (IFPF). Este instrumento evalĂşa los factores de protecciĂłn que contribuyan a la resiliencia familiar. Estudios de resiliĂŞncia familiar se apoyan en el paradigma salutogĂ©nico, que se centra en los factores de protecciĂłn de individuos o grupos, sin subestimar los factores de riesgo y vulnerabilidad. Metodologia: Aplicamos este instrumento a 85 familias de niños con necesidades especiales y, despuĂ©s de la equivalencia lingĂĽĂstica y conceptual, hemos llevado a cabo un análisis factorial exploratorio de componentes principales con rotaciĂłn varimax y calculamos el coeficiente alfa de Cronbach. Resultados: la IFPF tiene adecuadas propiedades psicomĂ©tricas para la poblaciĂłn portuguesa (alfa de Cronbach = .90). Conclusion: Esta es una herramienta Ăştil para evaluar los factores protectores de la resiliencia familiar, sin embargo sugerimos estudios futuros de revalidaciĂłn.Objetivos: adaptar e validar o Inventory of Family Protective Factors (IFPF) para a cultura portuguesa. Este instrumento avalia os fatores protetores que contribuem para a resiliĂŞncia familiar. Realizamos uma análise fatorial de componentes principais com rotação varimax dos itens da escala e calculamos o coeficiente Alpha de Cronbach para cada dimensĂŁo. AtravĂ©s de uma amostragem aleatĂłria simples, aplicamos este instrumento a 85 famĂlias de crianças com necessidades especiais que o auto-preencheram. Resultados: o IFPF apresenta caracterĂsticas psicomĂ©tricas adequadas para a população portuguesa (alfa de Cronbach de .90). ConclusĂŁo: o IFPF foi adaptado e validado para a cultura portuguesa. Gender differences in presentation and diagnosis of chest pain in primary care
<p>Abstract</p> <p>Background</p> <p>Chest pain is a common complaint and reason for consultation in primary care. Research related to gender differences in regard to Coronary Heart Disease (CHD) has been mainly conducted in hospital but not in primary care settings. We aimed to analyse gender differences in aetiology and clinical characteristics of chest pain and to provide gender related symptoms and signs associated with CHD.</p> <p>Methods</p> <p>We included 1212 consecutive patients with chest pain aged 35 years and older attending 74 general practitioners (GPs). GPs recorded symptoms and findings of each patient and provided follow up information. An independent interdisciplinary reference panel reviewed clinical data of every patient and decided about the aetiology of chest pain at the time of patient recruitment. Multivariable regression analysis was performed to identify clinical predictors that help to rule in or out CHD in women and men.</p> <p>Results</p> <p>Women showed more psychogenic disorders (women 11,2%, men 7.3%, p = 0.02), men suffered more from CHD (women 13.0%, men 17.2%, p = 0.04), trauma (women 1.8%, men 5.1%, p < 0.001) and pneumonia/pleurisy (women 1.3%, men 3.0%, p = 0.04) Men showed significantly more often chest pain localised on the right side of the chest (women 9.1%, men 25.0%, p = 0.01). For both genders known clinical vascular disease, pain worse with exercise and age were associated positively with CHD. In women pain duration above one hour was associated positively with CHD, while shorter pain durations showed an association with CHD in men. In women negative associations were found for stinging pain and in men for pain depending on inspiration and localised muscle tension.</p> <p>Conclusions</p> <p>We found gender differences in regard to aetiology, selected clinical characteristics and association of symptoms and signs with CHD in patients presenting with chest pain in a primary care setting. Further research is necessary to elucidate whether these differences would support recommendations for different diagnostic approaches for CHD according to a patient's gender.</p
Profile of subjective quality of life and its correlates in a nation-wide sample of high school students in an Arab setting using the WHOQOL-Bref
<p>Abstract</p> <p>Background</p> <p>The upsurge of interest in the quality of life (QOL) of children is in line with the 1989 Convention on the Rights of the Child, which stressed the child's right to adequate circumstances for physical, mental, and social development. The study's objectives were to: (i) highlight how satisfied Kuwaiti high school students were with life circumstances as in the WHOQOL-Bref; (ii) assess the prevalence of at risk status for impaired QOL and establish the QOL domain normative values; and (iii) examine the relationship of QOL with personal, parental, and socio-environmental factors.</p> <p>Method</p> <p>A nation-wide sample of students in senior classes in government high schools (N = 4467, 48.6% boys; aged 14-23 years) completed questionnaires that included the WHOQOL-Bref.</p> <p>Results</p> <p>Using Cummins' norm of 70% - 80%, we found that, as a group, they barely achieved the well-being threshold score for physical health (70%), social relations (72.8%), environment (70.8%) and general facet (70.2%), but not for psychological health (61.9%). These scores were lower than those reported from other countries. Using the recommended cut-off of <1<it>SD </it>of population mean, the prevalence of at risk status for impaired QOL was 12.9% - 18.8% (population age-adjusted: 15.9% - 21.1%). In all domains, boys had significantly higher QOL than girls, mediated by anxiety/depression; while the younger ones had significantly higher QOL (<it>p </it>< 0.001), mediated by difficulty with studies and social relations. Although poorer QOL was significantly associated with parental divorce and father's low socio-economic status, the most important predictors of poorer QOL were perception of poor emotional relationship between the parents, poor self-esteem and difficulty with studies.</p> <p>Conclusion</p> <p>Poorer QOL seemed to reflect a circumstance of social disadvantage and poor psychosocial well-being in which girls fared worse than boys. The findings indicate that programs that address parental harmony and school programs that promote study-friendly atmospheres could help to improve psychosocial well-being. The application of QOL as a school population health measure may facilitate risk assessment and the tracking of health status.</p
Chest pain in primary care: is the localization of pain diagnostically helpful in the critical evaluation of patients? - A cross sectional study
BACKGROUND: Chest pain is a common complaint and reason for consultation in primary care. Traditional textbooks still assign pain localization a certain discriminative role in the differential diagnosis of chest pain. The aim of our study was to synthesize pain drawings from a large sample of chest pain patients and to examine whether pain localizations differ for different underlying etiologies. METHODS: We conducted a cross-sectional study including 1212 consecutive patients with chest pain recruited in 74 primary care offices in Germany. Primary care providers (PCPs) marked pain localization and radiation of each patient on a pictogram. After 6Â months, an independent interdisciplinary reference panel reviewed clinical data of every patient, deciding on the etiology of chest pain at the time of patient recruitment. PCP drawings were entered in a specially designed computer program to produce merged pain charts for different etiologies. Dissimilarities between individual pain localizations and differences on the level of diagnostic groups were analyzed using the Hausdorff distance and the C-index. RESULTS: Pain location in patients with coronary heart disease (CHD) did not differ from the combined group of all other patients, including patients with chest wall syndrome (CWS), gastro-esophageal reflux disease (GERD) or psychogenic chest pain. There was also no difference in chest pain location between male and female CHD patients. CONCLUSIONS: Pain localization is not helpful in discriminating CHD from other common chest pain etiologies
H-1 NMR-metabolomics: Can they be a useful tool in our understanding of cardiac arrest?
Objective: This review focuses on the presentation ofthe emerging technology of metabolomics, a promising
tool for the detection of identifying the unrevealed biological pathways that lead to cardiac arrest.
Data sources: The electronic bases of PubMed, Scopus, and EMBASE were searched. Research terms
were identified using the MESH database and were combined thereafter. Initial search terms were
“cardiac arrest”, “cardiopulmonary resuscitation”, “post-cardiac arrest syndrome” combined with
“metabolomics”.
Results: Metabolomics allow the monitoring of hundreds of metabolites from tissues or body fluids
and already influence research in the field of cardiac metabolism. This approach has elucidated several
pathophysiological mechanisms and identified profiles of metabolic changes that can be used to follow
the disease processes occurring in the peri-arrest period. This can be achieved through leveraging the
strengths of unbiased metabolome-wide scans, which include thousands of final downstream products
of gene transcription, enzyme activity and metabolic products of extraneously administered substances,
in order to identify a metabolomic fingerprint associated with an increased risk of cardiac arrest.
Conclusion: Although this technology is still under development, metabolomics is a promising tool for elucidating
biological pathways and discovering clinical biomarkers, strengthening the efforts for optimizing
both the prevention and treatment of cardiac arrest
