    Localised melorheostosis

    Melorheostosis is a sclerosing bone dysplasia of unknown aetiology. Diagnosis is mainly based on a combination of clinical and imaging studies. The classical dripping candle wax picture on plain radiograph is diagnostic. We report a case of localized melorheostosis of a 14 year old boy on daily analgesics for two years for pain relief. Radiographs showed endosteal bone formation like dripping candle wax. Biopsy and decompression of the hyperostosis was done. However biopsy did not relieve his symptoms. Based on literature survey he was given a single infusion of zoledronic acid. This gave dramatic relief in pain

    : Estimation of postmortem interval plays some significant roles in medicolegal investigation. This was a descriptive experimental study using one local pig as model. Samples were taken from the right and left kidneys at several time intervals: 0 minute, 15 minutes, 30 minutes, 45 minutes, 60 minutes, 12 hours, and 24 hours postmortem. The results showed several histological changes, as follows: hydrophic degeneration in a small part of proximal tubules 30 minutes postmortem that increased after 45 minutes associated with narrowing of Bowman cavities; necrosis of glomeruli and proximal tubules associated with irregular distal tubular lumen and widening of Bowman cavities 60 minutes postmortem; necrosis of distal tubuli 12 hours postmortem; and necrosis of most kidney structures 24 hours postmortem. Conclusion: Hydrophic degeneration of proximal tubuli is the earliest histological change 30 minutes postmortem, followed by necrosis of glomeruli as well as proximal and distal tubuli that worsened after 24 hours postmortem. It is expected that this study can provide valuable contribution to medicolegal investigation, especially in early postmortem interval estimation

    PRESCRIPTION ANALYSIS, DRUG INTERACTION CHECKING AND IMPACT OF PATIENT COUNSELING BY A CLINICAL PHARMACIST IN TYPE II DIABETES MELLITUS PATIENTS IN A TERTIARY CARE HOSPITAL-A PROSPECTIVE STUDY

    Objective: To evaluate the clinical pharmacist role in the prescription analysis, drug interaction and the impact of patient counseling in type 2 diabetes mellitus patients. Methods: A prospective study was conducted in 203 type II Diabetes Mellitus patients for a period of 6 mo in the Diabetology department. Prescriptions were analyzed and self-care assessment for good health practices were collected using a questionnaire and the adherence scores were calculated. Patient counseling was provided to the patient and a follow up was done using the same self care assessment questionnaire. Results: Out of 203 patients, 86 multiple therapy, 68 dual therapy and 49 monotherapy were observed. Glimepiride+Metformin (54), a combination of short acting and intermediate-acting insulin (41) was the most commonly prescribed drugs. Out of 1102 drugs, 488 were anti diabetic drugs, 35 were antibiotics, 579 were other drug classes prescribed. The drug interactions were reported. The adherence score shows a highly significant impact after counseling. Conclusion: The results of the current study helps to understand the changes in prescription pattern, drug interactions and the impact of patient counseling by a clinical pharmacist

    Functional outcome of tendoachilles following Ponseti’s tenotomy for treatment of congenital talipes equino varus in children older than two years

    Background: Long term results of tenotomy and Ponseti technique are established worldwide. However, functions of Tendoachilles following Ponseti’s tenotomy in these cases i.e. idiopathic/neglected/operated/relapsed clubfeet (after casting or surgical correction) are not established. Tendoachilles regeneration after tenotomy has been confirmed on USG and MRI but only a few studies have done functional evaluation of tendoachilles. This study was done to evaluate the functional outcome of tendoachilles after tenotomy in patients older than two years presenting with CTEV. This study also assessed the influence of age and any previous treatment on tenotomy.Methods: In this study, 42 children (68 clubfeet) were seen in the two year study period. Children between 2-13 years coming to the outpatient department for treatment using the Ponseti’s method were followed during and after completion of treatment for 2 years. Patients were divided into two groups-first according to age and second according to previous treatment. Clinical evaluation of tendoachilles regeneration was done by evaluating the child’s ability to stand on tip of toes on single leg and walking ability.Results: The ability to stand on tip of toes after removal of the final cast was delayed maximum in the previously operated patients (9.5 weeks), lesser in patients who were previously treated by casting (7.3 weeks) and least in neglected patients (7 weeks). It also increased as the age increased (2-5 years age group required 7.4 weeks whereas 11-13 years age group required 16 weeks). Neglected patients started walking earlier (4.6 weeks) as compared to patients treated conservatively (4.8 weeks) or operatively (7.2 weeks). Younger children started walking earlier (age 2-5 years required 4.7 weeks whereas 11-13 years age group required 12 weeks). Conclusions: Functional evaluation of tendoachilles showed that all children who had tenotomy could walk and stand on tip of toes irrespective of age and previous treatment. However, older child and children having history of previous treatment, required longer time for recovery

    Loss of Gata6 causes dilation of the hair follicle canal and sebaceous duct

    The uppermost aspect of the hair follicle, known as the infundibulum or hair canal, provides a passageway for hair shaft egress and sebum secretion. Recent studies have indicated that the infundibulum and sebaceous ducts are lined by molecularly distinct differentiated cells expressing markers including Keratin 79 and Gata6. Here, we ablated Gata6 from the skin and observed dilation of both the hair canal and sebaceous ducts, independent of gender and hair cycle stage. Constitutive loss of Gata6 yielded only a mild delay in depilation‐induced entry into anagen, while unperturbed mutant mice possessed overtly normal skin and hair. Furthermore, we noted that Keratin 79 and Gata6 expression and localization did not depend upon each other.
