Once positioned within the MRI the subject cannot be displaced and therefore subject centering within the scanner (A-P and L-R directions) is not ensured. Thus, centering spinal cord images in the A-P and L-R directions is necessary to make group analysis and accurately quantifying cord atrophy in SCI patients. A Symmetry-based reorientation algorithm for the spinal cord 3T R images was proposed and evaluated on 32 data using both an imposed rotation angle and visual strategies. The proposed reorientation algorithm was proven to be efficient at C2 vertebral level

    Parkinson's disease patients show reduced cortical-subcortical sensorimotor connectivity

    International audienceReduced dopamine input to cortical and subcortical brain structures, particularly those in the sensorimotor network, is a hallmark of Parkinson's disease (PD). The extent to which dopamine dysfunction affects connectivity within this and other brain networks remains to be investigated. The purpose of this study was to measure anatomical and functional connectivity in groups of PD patients and controls to determine whether connectivity deficits within the cortico-basal ganglia thalamocortical system could be attributed to PD, particularly in sensorimotor connections. A neuroimaging paradigm involving diffusion-weighted magnetic resonance imaging (MRI) and resting-state functional MRI was implemented in a large cohort of PD patients and control subjects. Probabilistic tractography and functional correlation analyses were performed to map connections between brain structures and to derive indices of connectivity that were then used to compare groups. Anatomical connectivity deficits were demonstrated in PD patients, specifically for sensorimotor connections. Functional deficits were also found in some of the same connections. In addition, functional connectivity was found to increase in associative and limbic connections in PD patients compared with controls. This study lends support to findings regarding the dysfunction of the sensorimotor circuit in PD. As deficits in anatomical and functional connectivity within this circuit were in some cases concordant in PD patients, a possible link between brain structure and function is suggested. Increases in functional connectivity in other cortico-basal ganglia thalamocortical circuits may be indicative of compensatory effects in response to system deficits elsewhere. © 2012 Movement Disorder Society

    Motor and language deficits before and after surgical resection of mesial frontal tumour.

    International audienceOBJECTIVE: The goal of the present study was to better characterize pre- and immediately post-surgical motor and language deficits resulting from the surgery of tumours located in the medial part of the frontal lobe. PATIENTS AND METHODS: Seven patients treated by surgical resection of low-grade gliomas affecting the medial part of the frontal lobe were studied with neuropsychological tasks investigating motor and language abilities before surgery and at three time points after surgery (first, third and seventh day after surgery). The tasks were constructed in a way that allowed the structured comparison between language and motor functions, and controlled the level of external constraint of the production. RESULTS: The main results of this study are: (1) globally the patients were impaired in both language and motor production the day after surgery; (2) the performance improved faster for tasks with strongly constrained production; (3) the verbal and semantic fluency were very sensitive and appropriate tasks for examination of the deficits resulting from the resection; and (4) performances were back to normal seven days after the surgery for most of the tasks. CONCLUSION: These results confirm that surgery of low-grade gliomas affecting the prefrontal midline areas affects only transiently motor and language functions as tested in this study. They also suggest that verbal and semantic fluency were the most severely affected tests postoperatively. On the basis of these results, the surgical resection of the low-grade gliomas of the prefrontal midline seems a valuable treatment alternative

    Reduced basal forebrain atrophy progression in a randomized Donepezil trial in prodromal Alzheimer's disease

    Hippocampus Study GroupInternational audienceAcetylcholinesterase inhibitors are approved drugs currently used for the treatment of Alzheimer's disease (AD) dementia. Basal forebrain cholinergic system (BFCS) atrophy is reported to precede both entorhinal cortex atrophy and memory impairment in AD, challenging the traditional model of the temporal sequence of topographical pathology associated with AD. We studied the effect of one-year Donepezil treatment on the rate of BFCS atrophy in prodromal AD patients using a double-blind, randomized, placebo-controlled trial of Donepezil (10 mg/day). Reduced annual BFCS rates of atrophy were found in the Donepezil group compared to the Placebo treated arm. Secondary analyses on BFCS subregions demonstrated the largest treatment effects in the Nucleus Basalis of Meynert (NbM) and the medial septum/diagonal band (Ch1/2). Donepezil administered at a prodromal stage of AD seems to substantially reduce the rate of atrophy of the BFCS nuclei with highest concentration of cholinergic neurons projecting to the cortex (NbM), hippocampus and entorhinal cortex (Ch1/2)

    Distinct patterns of antiamyloid-β antibodies in typical and atypical Alzheimer disease

    International audienceOBJECTIVE: To compare serum antiamyloid-β (Aβ) antibodies in typical and atypical Alzheimer disease (AD). DESIGN: Preliminary observations. SUBJECTS: Thirteen patients with AD, 8 patients with posterior cortical atrophy with evidence of AD (PCA-AD) pathophysiological process by both cerebrospinal fluid (CSF) biomarkers and amyloid imaging, and 12 age-matched control individuals. INTERVENTIONS: The class and subclass levels of serum anti-Aβ antibodies were measured using an oligomer-based enzyme-linked immunosorbent assay. This method allowed measuring both free antibodies and, after acidic treatment, the total fraction that includes all antibodies complexed with circulating Aβ40/42 and any cross-reacting antigen. RESULTS: Anti-Aβ IgG were restricted to the IgG1 and IgG3 subclasses. Their total levels were strikingly lower and more homogeneous in patients with PCA compared with both typical AD and controls, while biomarkers of amyloid deposition (CSF Aβ42 and positron emission tomography amyloid imaging) were similar in patients with AD and patients with PCA. CONCLUSIONS: Serum anti-Aβ IgG1 and IgG3 antibodies differ between distinct forms of AD. Its significance is discussed for possible implications as immune effectors in the specific pathophysiology of AD variants

    The EADC-ADNI Harmonized Protocol for manual hippocampal segmentation on magnetic resonance:Evidence of validity

    BACKGROUND: An international Delphi panel has defined a harmonized protocol (HarP) for the manual segmentation of the hippocampus on MR. The aim of this study is to study the concurrent validity of the HarP toward local protocols, and its major sources of variance. METHODS: Fourteen tracers segmented 10 Alzheimer's Disease Neuroimaging Initiative (ADNI) cases scanned at 1.5 T and 3T following local protocols, qualified for segmentation based on the HarP through a standard web-platform and resegmented following the HarP. The five most accurate tracers followed the HarP to segment 15 ADNI cases acquired at three time points on both 1.5 T and 3T. RESULTS: The agreement among tracers was relatively low with the local protocols (absolute left/right ICC 0.44/0.43) and much higher with the HarP (absolute left/right ICC 0.88/0.89). On the larger set of 15 cases, the HarP agreement within (left/right ICC range: 0.94/0.95 to 0.99/0.99) and among tracers (left/right ICC range: 0.89/0.90) was very high. The volume variance due to different tracers was 0.9% of the total, comparing favorably to variance due to scanner manufacturer (1.2), atrophy rates (3.5), hemispheric asymmetry (3.7), field strength (4.4), and significantly smaller than the variance due to atrophy (33.5%, P < .001), and physiological variability (49.2%, P < .001). CONCLUSIONS: The HarP has high measurement stability compared with local segmentation protocols, and good reproducibility within and among human tracers. Hippocampi segmented with the HarP can be used as a reference for the qualification of human tracers and automated segmentation algorithms
