Organisationsstrukturen zur Verbesserung der Wirtschaftlichkeit medizinischer Laboratorien in Krankenhäusern. Organizational structures improving the economic efficiency of medical hospital laboratories
Abstract The common trend to form larger organisations has nowadays also reached the health system and the diagnostic laboratories. This development is dominated by economic motivations, notably by the wish to reduce costs. This tendency bears risks, specifically for the quality of medical services. Too much centralisation, which is frequently accompanied by all-inclusive prices and downgrading, puts quality at risk. Quality management does not necessarily increase costs but can also help to reduce costs. For example changes in the organisation, cooperation and management of laboratories may help to reduce costs but yet maintain or even improve quality. The achievement of this goal needs great efforts for defining visions and strategies and developing concepts. Expertise from economic and organisational sciences rather than mere medical knowledge are necessary to reach these objectives. Know-how on methods and problem solution must be gained or obtained from external sources. All involved individuals must be willing to change and to overcome antagonistic attitudes. Legal measures including the Health Care Modernisation Act (Gesundheitssystem-Modernisierungsgesetz =GMG) are aimed to reach integrated medical care, for example in out-patient and in-patient settings. It is important that the involved parties cooperate to overcome the disadvantages and to combine the advantages of the present situation. Thereby the health system will develop innovative solutions, which are efficient and cost-saving but also guarantee the maintenance or even improvement of current quality standard
Survey of Period Variations of Superhumps in SU UMa-Type Dwarf Novae. VIII: The Eighth Year (2015-2016)
Continuing the project described by Kato et al. (2009, arXiv:0905.1757), we
collected times of superhump maxima for 128 SU UMa-type dwarf novae observed
mainly during the 2015-2016 season and characterized these objects. The data
have improved the distribution of orbital periods, the relation between the
orbital period and the variation of superhumps, the relation between period
variations and the rebrightening type in WZ Sge-type objects. Coupled with new
measurements of mass ratios using growing stages of superhumps, we now have a
clearer and statistically greatly improved evolutionary path near the terminal
stage of evolution of cataclysmic variables. Three objects (V452 Cas, KK Tel,
ASASSN-15cl) appear to have slowly growing superhumps, which is proposed to
reflect the slow growth of the 3:1 resonance near the stability border.
ASASSN-15sl, ASASSN-15ux, SDSS J074859.55+312512.6 and CRTS J200331.3-284941
are newly identified eclipsing SU UMa-type (or WZ Sge-type) dwarf novae.
ASASSN-15cy has a short (~0.050 d) superhump period and appears to belong to EI
Psc-type objects with compact secondaries having an evolved core. ASASSN-15gn,
ASASSN-15hn, ASASSN-15kh and ASASSN-16bu are candidate period bouncers with
superhump periods longer than 0.06 d. We have newly obtained superhump periods
for 79 objects and 13 orbital periods, including periods from early superhumps.
In order that the future observations will be more astrophysically beneficial
and rewarding to observers, we propose guidelines how to organize observations

Frequency and gender differences in the use of professional home care in late life: Findings from three German old-age cohorts
Aim: The aim of this study was to investigate the frequency of and the gender
differences in the use of professional home care in Germany.
Methods: We used harmonized data from three large cohort studies from
Germany (“Healthy Aging: Gender-specific trajectories into the latest life”;
AgeDifferent.de Platform). Data were available for 5,393 older individuals
(75 years and older). Mean age was 80.2 years (SD: 4.1 years), 66.6%
were female. Professional homecare outcome variables were use of
outpatient nursing care, paid household assistance, and meals on wheels’
services. Logistic regression models were used, adjusting for important
sociodemographic variables.
Results: Altogether 5.2% of older individuals used outpatient nursing care (6.2%
women and 3.2% men; p < 0.001), 24.2% used paid household assistance
(26.1% women and 20.5% men; p < 0.001) and 4.4% used meals on wheels’
services (4.5% women and 4.0% men; p = 0.49). Regression analysis revealed
that women had higher odds of using paid household assistance than men
(OR = 1.48, 95% CI: [1.24–1.76]; p < 0.001), whereas they had lower odds of
using meals on wheels’ services (OR = 0.64, 95% CI: [0.42–0.97]; p < 0.05). No statistically significant differences in using outpatient nursing care between
women and men were found (OR = 1.26, 95% CI: [0.87–1.81]; p = 0.225).
Further, the use of home care was mainly associated with health-related
variables (e.g., stroke, Parkinson’s disease) and walking impairments.
Conclusions: Our study showed that gender differences exist in using paid
household assistance and in culinary dependency. For example, meals on
wheels’ services are of great importance (e.g., for individuals living alone or
for individuals with low social support). Gender differences were not identified
regarding outpatient nursing care. Use of professional home care servicesmay
contribute to maintaining autonomy and independence in old age
Rewriting DNA Methylation Signatures at Will:The Curable Genome Within Reach?
DNA methyltransferases are important enzymes in a broad range of organisms. Dysfunction of DNA methyltransferases in humans leads to many severe diseases, including cancer. This book focuses on the biochemical properties of these enzymes, describing their structures and mechanisms in bacteria, humans and other species, including plants, and also explains the biological processes of reading of DNA methylation and DNA demethylation. It covers many emerging aspects of the biological roles of DNA methylation functioning as an essential epigenetic mark and describes the role of DNA methylation in diseases. Moreover, the book explains modern technologies, like targeted rewriting of DNA methylation by designed DNA methyltransferases, as well as technological applications of DNA methyltransferases in DNA labelling. Prevalence of Anxiety Symptoms and Their Association With Loss Experience in a Large Cohort Sample of the Oldest-Old. Results of the AgeCoDe/AgeQualiDe Study
Background: Anxiety in adults is a common mental health problem. However, studies on anxiety in the oldest-old are lacking. We sought to identify the age- and gender-specific prevalence of anxiety symptoms in a large sample of general practice patients. Furthermore, we investigated relevant associations of anxiety specifically with respect to recent experience of loss.Methods: Based on the German Study on Ageing, Cognition and Dementia in general practice patients, a sample of 897 patients aged 82 years and older was assessed. Anxiety was assessed using the short form of the Geriatric Anxiety Inventory (GAI-SF). For the assessment of loss, patients were asked whether there were cases of death in their closer social environment since the last assessment. Descriptive and logistic regression analyses were run.Results: Of the oldest-old individuals (aged 82+ years, mean age: 86.8), 14.5% (95% CI 12.4–16.8) suffered from anxiety symptoms. Highest prevalence rates were found for 82- to 85-year-old women (17.2%, 95% CI 12.6–22.1) and for 86- to 90-year-old patients (both sexes) in general (15.9%, 95% CI 12.6–19.2). Older individuals who experienced cases of death in their close social environment within the last 18 months had almost twice the odds [odds ratio (OR) 1.91, 95% confidence interval (CI) 1.15–3.17] of reporting anxiety compared to those without a recent loss. As expected, depression and impaired cognitive status were associated with the presence of anxiety symptoms. No relation was found between social network, gender, age, frailty, or physical illness and anxiety in regression analysis.Conclusions: This study provides for the first time age- and gender-specific prevalence rates of anxiety symptoms and associated risk factors among a large population-based sample of oldest-old primary care attenders. Anxiety is highly prevalent in individuals aged 82 years and older. Depression, impaired cognitive status, and recent experience of loss are associated with late-life anxiety. Our findings support the idea that recent experience of loss should be taken seriously in the context of clinical practice with respect to diagnosing and treating anxiety in old age
Genomic and transcriptomic changes complement each other in the pathogenesis of sporadic Burkitt lymphoma
Burkitt lymphoma (BL) is the most common B-cell lymphoma in children. Within the International Cancer Genome Consortium (ICGC), we performed whole genome and transcriptome sequencing of 39 sporadic BL. Here, we unravel interaction of structural, mutational, and transcriptional changes, which contribute to MYC oncogene dysregulation together with the pathognomonic IG-MYC translocation. Moreover, by mapping IGH translocation breakpoints, we provide evidence that the precursor of at least a subset of BL is a B-cell poised to express IGHA. We describe the landscape of mutations, structural variants, and mutational processes, and identified a series of driver genes in the pathogenesis of BL, which can be targeted by various mechanisms, including IG-non MYC translocations, germline and somatic mutations, fusion transcripts, and alternative splicing
Mathematics and biology: a Kantian view on the history of pattern formation theory
Driesch’s statement, made around 1900, that the physics and chemistry of his day were unable to explain self-regulation during embryogenesis was correct and could be extended until the year 1972. The emergence of theories of self-organisation required progress in several areas including chemistry, physics, computing and cybernetics. Two parallel lines of development can be distinguished which both culminated in the early 1970s. Firstly, physicochemical theories of self-organisation arose from theoretical (Lotka 1910–1920) and experimental work (Bray 1920; Belousov 1951) on chemical oscillations. However, this research area gained broader acceptance only after thermodynamics was extended to systems far from equilibrium (1922–1967) and the mechanism of the prime example for a chemical oscillator, the Belousov–Zhabotinski reaction, was deciphered in the early 1970s. Secondly, biological theories of self-organisation were rooted in the intellectual environment of artificial intelligence and cybernetics. Turing wrote his The chemical basis of morphogenesis (1952) after working on the construction of one of the first electronic computers. Likewise, Gierer and Meinhardt’s theory of local activation and lateral inhibition (1972) was influenced by ideas from cybernetics. The Gierer–Meinhardt theory provided an explanation for the first time of both spontaneous formation of spatial order and of self-regulation that proved to be extremely successful in elucidating a wide range of patterning processes. With the advent of developmental genetics in the 1980s, detailed molecular and functional data became available for complex developmental processes, allowing a new generation of data-driven theoretical approaches. Three examples of such approaches will be discussed. The successes and limitations of mathematical pattern formation theory throughout its history suggest a picture of the organism, which has structural similarity to views of the organic world held by the philosopher Immanuel Kant at the end of the eighteenth century
