Complications of auricular correction
The risk of complications of auricular correction is underestimated. There is around a 5% risk of early complications (haematoma, infection, fistulae caused by stitches and granulomae, allergic reactions, pressure ulcers, feelings of pain and asymmetry in side comparison) and a 20% risk of late complications (recurrences, telehone ear, excessive edge formation, auricle fitting too closely, narrowing of the auditory canal, keloids and complete collapse of the ear). Deformities are evaluated less critically by patients than by the surgeons, providing they do not concern how the ear is positioned. The causes of complications and deformities are, in the vast majority of cases, incorrect diagnosis and wrong choice of operating procedure. The choice of operating procedure must be adapted to suit the individual ear morphology. Bandaging technique and inspections and, if necessary, early revision are of great importance for the occurence and progress of early complications, in addition to operation techniques. In cases of late complications such as keloids and auricles that are too closely fitting, unfixed full-thickness skin flaps have proved to be the most successful. Large deformities can often only be corrected to a limited degree of satisfaction
Effects of learned helplessness on error processing investigated with event-related potentials
Das Hauptziel dieser Untersuchung war, die ZusammenhĂ€nge zwischen Hilflosigkeit und neuronalen Korrelaten der Fehlerverarbeitung (ERN und Pe) zu erfassen. 25 MĂ€nner und 25 Frauen nahmen an einem Hilflosigkeitstraining teil, basierend auf 30 lösbaren und 30 unlösbaren Zahlenreihen. AnschlieĂend baten wir unsere Probanden einen Hilflosigkeitsfragebogen (Bauer et al., 2003) auszufĂŒllen. Es erfolgte auch ein modifizierter Flanker Task (Eriksen & Eriksen, 1974), im Zuge dessen wir die ERN und Pe der Teilnehmer mittels EEG aufnahmen. Wir verglichen die ERN und Pe von 23 Probanden, die aufgrund ihrer Ratings im Hilflosigkeitsfragebogen der Extremgruppen hilflos und nicht hilflos angehörten. Wir fanden signifikant höhere ÎERN fĂŒr hilflose verglichen mit nicht hilflosen Probanden. Nachdem wir durch eine z-Transformation auch die Wirkung der konfundierenden Variable Geschlecht ausgeschlossen hatten, konnten wir auch signifikant höhere ERN Amplituden fĂŒr Hilflose feststellen verglichen mit den Nicht-Hilflosen. Hinsichtlich der Pe-Komponente konnten keine Unterschiede zwischen Hilflosen und Nicht-Hilflosen festgestellt werden. Diese Ergebnisse bestĂ€tigen unsere Hypothesen und wir interpretieren die ERN Unterschiede zwischen hilflosen und nicht hilflosen Probanden basierend auf der Wirkung des induzierten negativen Affekts.This diploma thesis aimed to investigate whether induced short time negative affect in the form of helplessness would modulate the amplitudes of the ERN and the Pe event-related potentials. Both components are established neural correlates of error processing. Thus, we exposed 25 female and 25 male participants to a helplessness training comprising 30 solvable and 30 unsolvable cognitive tasks (series of numbers). Subsequently, participants had to complete a modified version of a flanker task (Eriksen & Eriksen, 1974) while their EEG-signals were recorded. We compared ERN and Pe amplitudes of helpless (13 subjects, 9 females) vs. not helpless (10 subjects, 4 females) participants and found a significantly higher ÎERN for helpless subjects compared to their not helpless counterparts. The helplessness status of the participants was assessed using a helplessness questionnaire (Bauer et al., 2003).
After separate z-transformation of the ERN amplitudes for males and females which was conducted in order to eliminate sex confounds caused by sexual dimorphism, we also found significantly larger ERN amplitudes for helpless vs. not helpless subjects. We explain the induced modulations of the ERN in terms of negative affect.
Additionally, we did not asses any differences regarding the Pe of helpless vs. not helpless subjects and no ERN related differences between helpless males and females were found. The hypothesis about the moderator role of the variable sex was neither confirmed nor rejected due to the lack of conclusive results
Effekte von genetischen Risikofaktoren fĂŒr Schizophrenie und Bipolare Störung auf die kortikale Dicke in gesunden Probanden
Das noch junge Feld des Genomic Imaging hilft bei dem VerstĂ€ndnis der Pathophysiologie neuropsychiatrischer und neurodegenerativer Erkrankungen durch die Untersuchung von Risikogenen mit bildgebenden Verfahren. Dabei können mögliche strukturelle und/oder funktionelle VerĂ€nderungen des Gehirns, ausgelöst durch genetische Varianten, helfen, die Entwicklung solcher Erkrankungen aufzuschlĂŒsseln. Eine der untersuchten Merkmale des Hirnparenchyms ist die kortikale Dicke, welche als reproduzierbarer Biomarker mit bekanntermaĂen hoher HeritibilitĂ€t gilt. FĂŒr sowohl die Schizophrenie als auch die Bipolare Störung, beides Krankheiten mit groĂem genetischen Einfluss, wurden bereits kortikale DickenĂ€nderungen in AbhĂ€ngigkeit des genetischen Hintergrundes der Probanden in vorangegangenen Studien beschrieben. In unserer Studie untersuchten wir 98 gesunde Probanden mit Hilfe einer Software namens Freesurfer auf mögliche kortikale DickenĂ€nderungen und verglichen dabei RisikoalleltrĂ€ger mit Nicht-RisikoalleltrĂ€gern fĂŒr mit Schizophrenie und/oder Bipolarer Störung assoziierten SNPs. Hierbei konzentrierten wir uns besonders auf die Auswahl der SNPs und schlossen nur genomweit signifikante Varianten nach strengen Kriterien in die Studie ein. Bei 7 von 17 untersuchten SNPs fanden wir signifikante neuroanatomische VerĂ€nderungen, nĂ€mlich kortikale Dickenabnahmen, zwischen den beiden gebildeten Probandengruppen in AbhĂ€ngigkeit ihres Risikostatus. Dabei entsprachen die gefundenen Hirnregionen mit signifikant verminderter Kortexdicke zum groĂen Teil den bekanntermaĂen funktionell mit Schizophrenie und/oder Bipolarer Störung assoziierten Arealen, z. B. den superioren Temporallappen (rs1344706 - ZNF804A). Unsere Studie steht somit in Einklang mit der bereits vorhandenen Literatur. Hierbei stellt sie, nach unserem Wissen, die erste Publikation mit solch systematischer Auswahl der zu untersuchenden SNPs und ihre Korrelation mit der kortikalen Dicke dar. Inwiefern diese morphologischen AuffĂ€lligkeiten bereits prĂ€natal, in der Kindesentwicklung oder im Erwachsenenalter entstehen und wie sehr Umweltfaktoren diese noch beeinflussen, ist noch nicht vollstĂ€ndig geklĂ€rt. Es gibt jedoch Hinweise darauf, dass bereits in der fötalen Entwicklung, in AbhĂ€ngigkeit des Risikostatus fĂŒr mit SCZ/BD assoziierten SNPs, VerĂ€nderungen der Genexpression transkriptions-regulatorische Auswirkungen haben, die letztlich zu einer verĂ€nderten Morphologie und dementsprechend möglicherweise erhöhtem Erkrankungsrisiko fĂŒhren. Es ist wahrscheinlich, dass dieser Prozess auch postnatal wĂ€hrend der Hirnentwicklung fortbesteht. Obwohl bekannt ist, dass Umweltfaktoren weniger als der genetische Hintergrund in der Entstehung dieser psychiatrischen Erkrankungen eine Rolle spielen, muss insbesondere der soziale und kognitive Einfluss der Umgebung auf die Hirnentwicklung und Altersprozesse, wie die altersbedingte oder dementielle Atrophie, zukĂŒnftig genauer analysiert und in folgenden Studien mehr berĂŒcksichtigt werden. Eine klinische Relevanz der Korrelation von genetischem Hintergrund mit der Neuroanatomie bestĂŒnde dann, wenn durch die Kombination aus genetischem und neuroanatomischem Befund die Wahrscheinlichkeit von psychiatrischen Erkrankungen, wie Schizophrenie oder Bipolare Störung, vorhergesagt werden könnte. Dies wĂŒrde helfen, durch neue medikamentöse, psychotherapeutische oder interventionelle Therapien, das Lebenszeitrisiko von Gesunden mit erhöhter Erkrankungswahrscheinlichkeit zu senken oder sogar die Entstehung einer entsprechenden Pathologie vollstĂ€ndig zu verhindern. Ob die Untersuchung eines erhöhten Erkrankungsrisikos und die Information des Patienten darĂŒber ethisch vertretbar ist, mĂŒsste durch die daran beteiligten Fachgesellschaften genau erörtert und, falls zugelassen, unter strenger Reglementierung angewandt werden. Unsere Studie stellt einen Beitrag zur neurogenetischen und neuroanatomischen Grundlagenforschung dar. Unsere Ergebnisse und deren Interpretationen mĂŒssen durch weitere Studien, mit möglichst groĂer Probandenzahl, Einschluss von schizophrenen und/oder bipolaren Patienten und der Erweiterung auf weitere genetische Marker, verifiziert und konkretisiert werden. Insbesondere longitudinale Studien mit Vergleich kortikaler DickenĂ€nderungen zwischen Schizophrenen, Bipolaren und Gesunden ĂŒber groĂe ZeitrĂ€ume wĂ€ren hochinteressant, um die Auswirkungen kortikaler DickenĂ€nderungen besser zu verstehen. Möglicherweise könnten kortikale Grenzwerte gefunden werden, die zwischen Pathologie und Zufallsbefund differenzieren lassen

Results. Selected and substantiated the main spatial planning and design solutions.
Completed layout of the structural scheme, collection and calculation of loads. The
calculation of the pile-strip foundation was carried out. The rational scheme of
combination of technological processes was chosen. The project of performance of works
and the corresponding technological maps was developed. The analysis of prospects of
arrangement of piles on pulse technology at construction of multistory apartment building
was executed.

Common variation in NCAN, a risk factor for bipolar disorder and schizophrenia, influences local cortical folding in schizophrenia
Background Recent studies have provided strong evidence that variation in the gene neurocan (NCAN, rs1064395) is a common risk factor for bipolar disorder (BD) and schizophrenia. However, the possible relevance of NCAN variation to disease mechanisms in the human brain has not yet been explored. Thus, to identify a putative pathomechanism, we tested whether the risk allele has an influence on cortical thickness and folding in a well-characterized sample of patients with schizophrenia and healthy controls. Method Sixty-three patients and 65 controls underwent T1-weighted magnetic resonance imaging (MRI) and were genotyped for the single nucleotide polymorphism (SNP) rs1064395. Folding and thickness were analysed on a node-by-node basis using a surface-based approach (FreeSurfer). Results In patients, NCAN risk status (defined by AA and AG carriers) was found to be associated with higher folding in the right lateral occipital region and at a trend level for the left dorsolateral prefrontal cortex. Controls did not show any association (p>0.05). For cortical thickness, there was no significant effect in either patients or controls. Conclusions This study is the first to describe an effect of the NCAN risk variant on brain structure. Our data show that the NCAN risk allele influences cortical folding in the occipital and prefrontal cortex, which may establish disease susceptibility during neurodevelopment. The findings suggest that NCAN is involved in visual processing and top-down cognitive functioning. Both major cognitive processes are known to be disturbed in schizophrenia. International Consensus Statement on Rhinology and Allergy: Rhinosinusitis
Background: The 5 years since the publication of the first International Consensus Statement on Allergy and Rhinology: Rhinosinusitis (ICARâRS) has witnessed foundational progress in our understanding and treatment of rhinologic disease. These advances are reflected within the more than 40 new topics covered within the ICARâRSâ2021 as well as updates to the original 140 topics. This executive summary consolidates the evidenceâbased findings of the document. Methods: ICARâRS presents over 180 topics in the forms of evidenceâbased reviews with recommendations (EBRRs), evidenceâbased reviews, and literature reviews. The highest grade structured recommendations of the EBRR sections are summarized in this executive summary. Results: ICARâRSâ2021 covers 22 topics regarding the medical management of RS, which are grade A/B and are presented in the executive summary. Additionally, 4 topics regarding the surgical management of RS are grade A/B and are presented in the executive summary. Finally, a comprehensive evidenceâbased management algorithm is provided. Conclusion: This ICARâRSâ2021 executive summary provides a compilation of the evidenceâbased recommendations for medical and surgical treatment of the most common forms of RS
