Accidental swallowing of partial denture: a case report
We describe a 42-year-old age woman who accidentally swallowed her lower denture, which was composed of eleven teeth. The daily descent of the denture was followed by plain abdominal radiography and physical examination. The image was localized at the left upper quadrant on admission day, but it stopped on its way at the right lower quadrant on day two and three. Since the patient's complaints increased we planned surgical removal of the denture. The effect of diosmin on pancreatic injury induced by hepatic ischemia reperfusion in rats
Abstract: Background: Hepatic ischemia-reperfusion causes histological injury to the pancreatic cells during
transplantation, trauma and emergency surgery. We investigated the effects of diosmin, a phlebotrophic drug with
antioxidant and antiinfl ammatory effects, on pancreatic injury in the experimental liver ischemia-reperfusion model.
Materials and methods: Forty rats were divided into the four groups: sham (Group 1), control (Group 2), peroperative
diosmin (50 mg/kg) treatment (Group 3) and preoperative 10-day diosmin (50 mg/kg) treatment group
(Group 4). Ischemia-reperfusion model was carried out by clamping the hepatic pedicle for 60 min and then
reperfusing the liver for 90 min. At the end of the procedures, blood and pancreas tissue samples were obtained
for biochemical and histopathological assessment.
Results: According to the results of liver function tests, amylase and the plasma and pancreatic tissue oxidative
stress parameters, there was a signifi cant difference between the control and other groups. Histopathologically;
the specimens of the Group 2 showed specifi c morphological abnormalities. The groups 3 and 4 showed the
pancreas histomorphology similar to the sham group. Pathological scores were signifi cantly different between
the Group 2 and other groups.
Conclusions: Diosmin can be administered for a protection of destructive effects of hepatic ischemia–reperfusion
injury on pancreas both emergent and elective hepatic surgical operations in which possible ischemic periods
were expecte
Elderly and aged asthma have different characteristics: results of a multicenter study
Background/aim: Characteristics of asthma in the elderly population is not well-known. The aim of the present study was to evaluate asthma in the elderly population, to compare disease characteristics between patients diagnosed <60 (aged asthma) and ≥60 (elderly asthma) years of age.
Materials and methods: The study was a prospective, multicenter, cross-sectional type. A questionnaire was filled out to patients 60 years of age and over, that have been followed for asthma for at least 3 months. Asthma Control Test (ACT), eight-item Morisky Medication Adherence Scale (MMAS-8) was filled out, inhaler device technique was assessed.
Results: A total of 399 patients were included from 17 tertiary care centers across the country. Mean age was 67.11 years and 331 (83%) were female. The age at asthma diagnosis was ≥60 in 146 (36.6%) patients. Patients diagnosed ≥60 years were older (p < 0.001), had higher education level (p < 0.001), more commonly had first-degree relative with asthma (p = 0.038), asthma related comorbidities (p = 0.009) and accompanying rhinitis/rhinosinusitis (p = 0.005), had better asthma control (p = 0.001), were using less controller medications (p = 0.014). Inhaler technique was correct in 37% of the patients with no difference in between the groups. Treatment compliance was better in elderly asthma patients (p < 0.001). In the multivariate logistic regression analysis, having well-controlled asthma (odds ratio = 1.61, CI = 1.04-2.51), and high medication adherence rate (odds ratio = 2.43, CI = 1.48-4.0) were associated with being in the elderly asthma group.
Conclusion: The characteristics of asthma are different among patients aged 60 years and over which seems to be related to onset age of asthma. In our cohort, the elderly asthma patients had higher education level, and treatment adherence and asthma control was better. Patients diagnosed ≥60 years of age did not have more severe disease
Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries
Abstract
Background
Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres.
Methods
This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries.
Results
In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia.
Conclusion
This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
Benign Proliferative Breast Lesions and Risk of Cancer
Benign breast lesions (BBL) includes a wide variety of histologic entities, which have been broadly classified into non-proliferative lesions, proliferative lesions without atypia, and hyperplasia with atypia. With the increased use of mammography, more benign lesions are being detected, and in order to estimate the risk of breast cancer for specific histologic categories is of great importance to guide clinical management. Women with proliferative lesions without atypia are at slightly increased risk of subsequent breast cancer, whereas women with proliferative lesions with atypia have a higher risk. The risk is 1.5- 2-fold in women with proliferative lesions without atypia, 4-5-fold in women with proliferative lesions with atypia, and 8-10 fold in women with ductal carcinoma in situ. Age at diagnosis of BBL, menopausal status, family history of breast cancer in a first-degree relative, and time since BBL diagnosis on risk of breast cancer are important for risk evaluation. AIM: Spinal Cord Injury (SCI) is routinely treated with standardized methyl prednisolone sodium succinate (MPSS) dose, so it is reassuring to find its effects on liver. We also evaluated the effects of albumin and immunoglobulin G (Ig G) therapies on liver if they are used in case of experimental SCI. MATERIAL and METHODS: The rats were allocated into six groups as control, trauma, vehicle, MPSS, Ig G and albumin consisting 8 rats for each. The rats with SCI were assigned to 30mg/kg MPSS, 5 mg/kg albumin and 400 mg/kg Ig G treatments. Tissue samples from liver were obtained for light and electron microscopy examinations and determination of myeloperoxidase (MPO) activity RESULTS: Trauma increased M PO activity and caused cellular changes of liver tissue. Both albumin and Ig G treatments decreased MPO activity significantly The light and electron microscopic evaluations showed remarkable preservation of liver ultra-structure with all treatments including MPSS. CONCLUSIONS: SCI resulted in neutrophil infiltration and changes in ultrastructure of liver. It was revealed that MPSS has no detrimental effects on liver. Although all treatments preserved liver tissue structure, Although all treatments preserved liver tissue structure, Ig G and albumin treatments also prevented neutrophil infiltration. To provide protection from secondary liver injury after SCI, use of albumin and Ig G treatments may be beneficial.Wo
Effect of propolis on oxidative stress and histomorphology of liver tissue in experimental obstructive jaundice
Background: Propolis is a natural product collected by honey bees from various plant sources. We aimed to determine the possible effects of propolis on oxidative stress and hepatocyte apoptosis in experimental obstructive jaundice. Methods: Thirty rats were divided into three groups: group I, sham-operated; group II, ligation and division of the common bile duct (BDL); group III, BDL followed by oral supplementation of propolis in a daily dose of 100 mg/kg. Liver samples were examined under the light microscope and transmission electron microscope. Hepatocyte apoptosis was quantitated using the transferase-mediated uridine nick end labeling (TUNEL) assay. Plasma and liver malondialdehyde (MDA) levels and glutathione peroxidase (GSH-Px) activities were measured. Results: The plasma and liver levels of MDA were significantly lower in the propolis group than in the BDL group (p 0.05). In the propolis group, the enlargement of hepatocytes, dilatation of canaliculi and the edema regressed. The regenerating and normal hepatocytes were demonstrated. In the TUNEL assay, propolis administration reduced hepatocyte apoptosis. Conclusion: Propolis showed a significant hepatoprotective effect in this experimental obstructive jaundice model.
