Facial emotion recognition and alexithymia in adults with somatoform disorders
The primary aim of this study was to investigate facial emotion recognition (FER) in patients with somatoform disorders (SFD). Also of interest was the extent to which concurrent alexithymia contributed to any changes in emotion recognition accuracy. Twenty patients with SFD and 20 healthy, age, sex and education matched, controls were assessed with the Facially Expressed Emotion Labelling Test of FER and the 26-item Toronto Alexithymia Scale. Patients withSFD exhibited elevated alexithymia symptoms relative to healthy controls.Patients with SFD also recognized significantly fewer emotional expressions than did the healthy controls. However, the group difference in emotion recognition accuracy became nonsignificant once the influence of alexithymia was controlled for statistically. This suggests that the deficit in FER observed in the patients with SFD was most likely a consequence of concurrent alexithymia. It should be noted that neither depression nor anxiety was significantly related to emotion recognition accuracy, suggesting that these variables did not contribute the emotion recognition deficit. Impaired FER observed in the patients with SFD could plausibly have a negative influence on these individuals’ social functioning
Alexithymia and interleukin variations in somatoform disorder
Objective: The aim of the present study was to investigate if somatoform disorders (SFD) are associated with changes in the normal serum levels of important interleukins, and further, to establish if these changes are related to the presence and severity of alexithymia in patients with SFD. Methods: Twenty-four unmedicated patients who met the International Classification of Diseases (ICD-10) diagnostic criteria for SFD completed the psychological questionnaire to assess alexithymia (Toronto Alexithymia Scale), symptom reporting (SCL-90-R) and diagnostic criteria for SFD (Screening for Somatoform Symptoms scale). Serum concentrations of soluble interleukin 2 receptor α (sIL-2 Rα), IL-4, IL-6, IL-10 and IL-12 were determined in patients with SFD and in 9 healthy subjects. Results: In patients with SFD, serum levels of IL-6 (p < 0.001), IL-10 (p = 0.047) and immunoglobulin E (p = 0.045) were significantly increased in comparison with healthy controls. Additionally, a negative correlation was observed between the level of alexithymia ('total' Toronto Alexithymia Scale score) and the serum levels of sIL-2 Rα (r = -0.538) in SFD. Conclusions: Taken together, these results suggest that SFD, with clinically significant alexithymia, are associated with a reduction in Th1-mediated immune function and an increase in the activation of the Th2 immune function, indicated by the augmented serum levels of IL-6 and IL-10 and elevated immunoglobulin E. ADHD symptomatology in eating disorders : a secondary psychopathological measure of severity?
Background: Attention-deficit/hyperactivity disorder (ADHD) has commonly been described in psychiatric disorders. Although several studies have found positive associations between abnormal eating patterns during childhood and ADHD, there is a lack of studies on ADHD and Eating Disorders (ED). The aims of this exploratory study were 1) to assess the ADHD symptoms level in ED and to ascertain whether there are differences among ED subtypes; 2) to analyze whether the presence of ADHD symptoms is associated with more severe eating disorder symptoms and greater general psychopathology; and 3) to assess whether the ADHD symptoms level is associated with specific temperament and character traits. Methods: 191 female ED patients were included. Assessment was carried out with the EDI-2, ASRS-v1.1, the SCL-90-R and the TCI-R. Results: The ADHD symptoms level was similar in bulimia, eating disorder not otherwise specified and binge eating subtypes, and lower in anorexic patients. Obsessiveness and Hostility were significantly positively associated with ADHD symptoms. A path model showed that ADHD was associated with high Novelty Seeking and low Self-Directedness, whereas ED severity was influenced by ADHD severity and low Self-Directedness. Conclusions: Bingeing/purging ED subtypes have a high ADHD symptoms level, also related with more severe eating, general and personality psychopathology
Quantitative aortography for assessment of aortic regurgitation in the era of percutaneous aortic valve replacement
Paravalvular leak (PVL) is a shortcoming that can erode the clinical benefits of transcatheter valve replacement (TAVR) and therefore a readily applicable method (aortography) to quantitate PVL objectively and accurately in the interventional suite is appealing to all operators. The ratio between the areas of the time-density curves in the aorta and left ventricular outflow tract (LVOT-AR) defines the regurgitation fraction (RF). This technique has been validated in a mock circulation; a single injection in diastole was further tested in porcine and ovine models. In the clinical setting, LVOT-AR was compared with trans-thoracic and trans-oesophageal echocardiography and cardiac magnetic resonance imaging. LVOT-AR > 17% discriminates mild from moderate aortic regurgitation on echocardiography and confers a poor prognosis in multiple registries, and justifies balloon post-dilatation. The LVOT-AR differentiates the individual performances of many old and novel devices and is being used in ongoing randomized trials and registries
Disfunção executiva como uma medida de funcionalidade em adultos com TDAH Executive dysfunction as a measure of functionality in adults with ADHD
Este artigo aborda a presença e a apresentação clÃnica da disfunção executiva em pacientes com TDAH. Na medida em que sintomas de disfunção executiva determinam as alterações de funcionamento desses pacientes nas atividades do dia-a-dia, torna-se relevante o estudo das relações entre alterações em testes neuropsicológicos formais de funções executivas e a ocorrência desses sintomas. A constatação relativamente recente do papel da disfunção executiva no TDAH tem impulsionado a pesquisa neuropsicológica, mas os dados até agora disponÃveis são ainda pouco consistentes. Uma visão atualizada dessas questões sugere que a semiologia neuropsicológica disponÃvel ainda não alcança plenamente a constelação de sintomas práticos do TDAH no dia-a-dia, mostrando a necessidade de avanços na pesquisa dessas relações.<br>This article addresses the presence and the clinical presentation of executive dysfunction in patients with ADHD. Because symptoms of executive dysfunction apparently determine functional abnormalities in daily activities, the study of the correlations between such manifestations and abnormalities in formal neuropsychological tests becomes clinically relevant. The relatively recent realization of the pivotal role of executive dysfunction in ADHD has kindled interest in neuropsychological research, but the data generated thus far are still not fully consistent. An updated view of these issues suggests that the available neuropsychological semiology is not sufficient to identify or predict the full constellation of practical symptoms of ADHD in daily life. Thus, more research is needed to establish secure links between neuropsychological testing and symptoms of executive dysfunction in the practical functioning of patients with ADHD
The quick delay questionnaire: a measure of delay aversion and discounting in adults
Individuals with ADHD often display an altered response to delay. To date, assessment of this has typically involved neuropsychological testing—however, such tests are designed specifically for children and may not be suitable for adults. They are also relatively time-consuming and expensive. In the current paper, we describe the initial validation of a short questionnaire to assess delay-related behaviour in adults. The Quick Delay Questionnaire (QDQ) is a 10-item scale. The questionnaire was administered to 575 participants from the normal population (ranging in age from 18 to 77 years). Forty of the original sample were selected at random and tested 1 week later. Data on ADHD, anxiety and depression were also collected. There were two five-item scales—(1) delay aversion; and (2) delay discounting. These had internal consistency and had good reliability. Subscales were differentially associated with ADHD, anxiety and depression. The QDQ is a potentially valuable way of assessing response to delay in adults. Further work is required to validate the scale against direct observation and neuropsychological assessment in clinically ascertained samples
Dysfunction in the fronto-parietal network in attention deficit hyperactivity disorder (ADHD: An fMRI Study
ADHD is associated with spatial working memory deficits, thought to be subserved by dysfunction of neural circuits. In this study we aimed to further examine fronto-parietal dysfunction in ADHD by examining brain activation associated with the Raven’s Progressive Matrices task, a visuo-spatial pattern sequencing task involving relational reasoning and thereby placing high-demand loading on the prefrontal cortex. Functional MRI was conducted on twelve right-handed 8–12 year old boys with ADHD-combined type and 12 right-handed, age and performance IQ-matched, healthy boys as they completed items from the Raven’s Progressive Matrices. Our findings further confirm striatum and parietal lobe dysfunction in ADHD. Furthermore, dysfunction in lateral prefrontal regions was found. In addition to lateral prefrontal, striatum and posterior parietal regions, the temporal lobe was also less active. These findings suggest deficits in a widespread ‘functional network’ in ADHD that may be fundamental for visuo-spatial information processing and relational reasoning
Source Discrimination in Adults with Attention Deficit Hyperactivity Disorder
<p>Objectives: The context of memory experiences is referred to as source memory and can be distinguished from the content of episodic item memory. Source memory represents a crucial part of biographic events and elaborate memory experiences. Whereas individuals with attention deficit hyperactivity disorder (ADHD) were shown to have inefficient item memory, little is known about the context of memory experiences.</p><p>Methods: The present study compared 37 adult patients with a diagnosed ADHD with 40 matched healthy participants on a word list paradigm. Memory functions of encoding, retention and source discrimination were assessed. Furthermore, standardized measures of memory and executive control were applied in order to explore a qualitative differentiation of memory components.</p><p>Results: Adult patients with ADHD showed impaired performance in encoding of new information whereas the retention of encoded items was found to be preserved. The most pronounced impairment of patients with ADHD was observed in source discrimination. Regression models of cognitive functions on memory components supported some qualitative differentiation.</p><p>Conclusions: Data analysis suggests a differential pattern of memory impairment in adults suffering from ADHD with a particular deficit in source discrimination. Inefficient source discrimination in adults with ADHD can affect daily functioning by limiting biographic awareness and disturbing general cognitive processes.</p>
