Urban agriculture (UA) is steadily gaining supporters worldwide, and this is partly because constantly growing urban populations recognize the need to increase food production and promote a healthy diet. UA can also generate extra income, promote socialization leading to valorisation of urban areas. Nevertheless, UA faces several challenges, including limited available space, stressed natural resources, pollution in a context characterized by climate change and new consumption patterns. At the same time, if UA is practiced in the same way as other forms of agriculture, it may have some undesirable environmental impacts on urban areas. The use of indicators can provide decision makers with a tool to evaluate the sustainable insertion of agriculture in cities. This work aims to propose a minimum set of indicators as the basis for an index to assess progress of urban vegetable production towards sustainability. Nineteen urban sustainability indexes, composed of several indicators founded in the literature and already used by several institutions, were analysed. These indexes were broken down into a set of 1579 indicators. Analysis of the relevancy to issues such as food, energy, water, land and pollution, and the selection based on the criteria of being measurable, sensitive to stress, predictable, anticipatory, controllable, integrative, responsive and stable, led to a 26 basic indicators selection. These are proposed as the basis for an innovative UA sustainability index. This will be built based on the Delphi method and is intended to support communities in establishing sustainable and resilient cities

PURPOSE: To evaluate the pediatric prehospital care in São Paulo, the databases from basic life support units (BLSU) and ALSU, and to propose a simple and effective method for evaluating trauma severity in children at the prehospital phase. METHODS: A single firemen headquarter coordinates all prehospital trauma care in São Paulo city. Two databases were analyzed for children from 0 to 18 years old between 1998 and 2001: one from the Basic Life Support Units (BLSU - firemen) and one from the Advanced Life Support Units (ALSU - doctor and firemen). During this period, advanced life support units provided medical reports from 604 victims, while firemen provided 12.761 reports (BLSU+ALSU). Pre-Hospital Pediatric Trauma Classification is based on physiological status, trauma mechanism and anatomic injuries suggesting high energy transfer. In order to evaluate the proposed classification, it was compared to the Glasgow Coma Score and to the Revised Trauma Score. RESULTS: There was a male predominance in both databases and the most common trauma mechanism was transport related, followed by falls. Mortality was 1.6% in basic life support units and 9.6% in ALSU. There was association among the proposed score, the Glasgow Coma Score and to the Revised Trauma Score (p<0.0001). CONCLUSION: Pre-Hospital Pediatric Trauma Classification is a simple and reliable method for assessment, triage and recruitment of pediatric trauma resources.OBJETIVO: Avaliar o atendimento pré-hospitalar de crianças e adolescentes em São Paulo, avaliar o banco de dados das Unidades de Suporte Básico (UR) e Avançado (USA) e propor um método simples e eficaz para a avaliação da gravidade do trauma pediátrico na fase pré-hospitalar. Universidade Federal de São Paulo (UNIFESP) Department of SurgeryUNIFESP, Department of SurgerySciEL

Payment for Environmental Services (PES) is an important tool for environmental conservation and is a relevant practice in many countries for the maintenance of forests. Thus, the objective of the present study was to evaluate challenges and opportunities of implementing a Payment for Environmental Services program in the municipality of Senhora de Oliveira, Minas Gerais. Socioeconomic and environmental diagnosis was performed using the Rapid Participatory Diagnosis technique. The total of 20 socioeconomic and environmental questionnaires were applied in the municipality, aiming to know in detail the local reality, raising potentialities and demands. The number of members of the 20 families that participated in the rural diagnosis was 72, of which 36 were men and 36 were women, with an average of 3.6 persons per family. Of the 20 rural properties that participated in the rural diagnosis, 35% have delimited RL areas and none of them has a management plan. The exploration of Legal Reserve occurs in 15% of the evaluated properties, being the production of firewood and cuttings the predominant activity. The average value of disposition receivable by rural producers was R$ 220.00 ha-1year-1, in which the producers who did not respond to this question were not considered. The municipality of Senhora de Oliveira, Minas Gerais has the necessary conditions to implement the PES, however the producers still do not receive any incentive to provide environmental services. If this process of PES is implemented, there will certainly be advances in environmental conservation and will bring social benefits throughout the region

    Medical treatment of prolactinomas.

Prolactinomas, the most prevalent type of neuroendocrine disease, account for approximately 40% of all pituitary adenomas. The most important clinical problems associated with prolactinomas are hypogonadism, infertility and hyposexuality. In patients with macroprolactinomas, mass effects, including visual field defects, headaches and neurological disturbances, can also occur. The objectives of therapy are normalization of prolactin levels, to restore eugonadism, and reduction of tumor mass, both of which can be achieved in the majority of patients by treatment with dopamine agonists. Given their association with minimal morbidity, these drugs currently represent the mainstay of treatment for prolactinomas. Novel data indicate that these agents can be successfully withdrawn in a subset of patients after normalization of prolactin levels and tumor disappearance, which suggests the possibility that medical therapy may not be required throughout life. Nevertheless, multimodal therapy that involves surgery, radiotherapy or both may be necessary in some cases, such as patients who are resistant to the effects of dopamine agonists or for those with atypical prolactinomas. This Review reports on efficacy and safety of pharmacotherapy in patients with prolactinomas

The Montenegro skin test is widely used as a diagnostic method for American cutaneous leishmaniasis (ACL) but little is known about the histological changes that occur in the skin after administration of the antigen. This report is based on histological studies of biopsied material obtained, from inoculation sites, 48 hours after individuals had been given intradermal injections with a standardized Montenegro antigen. The material examined was obtained from four distinctly different test groups: naturally infected patients with parasitologically proved ACL and with positive Montenegro's reaction; individuals without previous history of ACL and not previously tested with Montenegro antigen; participants in anti-ACL vaccine trials who developed positive reactions to Montenegro antigen after vaccination; other participants in vaccine trials who had negative Montenegro responses after vaccination or had served as controls in the trials. The histological pictures of each group are described and discussed. Histologically, the reactions of vaccinated individuals were indistinguishable from those with naturally acquired infections.

Primary productivity and phytoplankton dynamics in a subtropical estuary: a multiple timescale approach

Temporal variability of physicochemical parameters and phytoplankton primary productivity, abundance and composition were investigated at Babitonga Bay, southern Brazil, using a hierarchical sampling design on the scales of months, weeks and days during spring 2012 and summer 2013. Only temperature, respiration rates and concentrations of dissolved oxygen and silicate exhibited significant differences in the greatest timescale (seasons: spring; summer). In contrast, most physicochemical parameters, such as salinity and the concentration of nitrogen compounds, varied mainly among weeks and days. This short-scale variability was similarly observed for the microphytoplankton abundance, ranging from 0.04 to 1.7.106 cells L–1 during a bloom of the diatom Skeletonema costatum. Two major phytoplankton assemblages were associated with high primary production rates: > 30 μm centric diatoms such as Cyclotella spp. and Cymatodiscus sp., in spring; and a summer assemblage dominated by smaller, bloom-forming diatoms (S. costatum and Eucampia cornuta). Primary production ranged from 10.5 to 1793 mg C m–2 d–1 and varied significantly between days and months, being associated with the photosynthetic active radiation level and weather conditions on the sampling day. Abundance of specific plankton taxa appeared controlled by trophic interactions, as revealed for the mixotrophic, toxic dinoflagellate Dinophysis acuminata complex and its prey, the ciliate Mesodinium rubrum
