    Relation between torsion and cross-sectional area change in the human left ventricle

    During the ejection phase, motion of the left ventricular (LV) wall is such that all myocardial fibers shorten to the same extent. In a mathematical model of LV mechanisms it was found that this condition could be satisfied only if torsion around the long axis followed a unique function of the ratio of cavity volume to wall volume. When fiber shortening becomes non-uniform due to cardiac pathology, this pathology may be reflected in aberration of the torsional motion pattern. In the present study we investigated whether the predicted regular motion pattern could be found in nine healthy volunteers, using Magnetic Resonance Tagging. In two parallel short-axis cross-sections, displacement, rotation, and area ejection were derived from the motion of tags, attached non-invasively to the myocardium. Information from both sections was combined to determine area ejection, quantified as the change in the logarithm of the ratio of cavity area to wall area, and torsion, represented by the shear angle on the epicardium. Linear regression was applied to torsion as a function of area ejection. The slope thus found (-0.173 +/- 0.024 rad, mean +/- S.D.) was similar to the slope as predicted by the model of LV mechanics (-0.194 +/- 0.026 rad). In conclusion, the relation between area ejection and torsion could be assessed noninvasively in humans. In healthy volunteers, the relation was close to what was predicted by a mathematical model of LV mechanics, and also close to what was found earlier in experiments on animals.     A controlled evaluation of an enhanced self-directed behavioural family intervention for parents of children with conduct problems in rural and remote areas.

    Few studies have examined the impact of parenting interventions for families in rural and isolated areas who have children with conduct problems, where-access to professional services can be difficult. The present investigation compared the effects of three conditions, two levels of self-directed behavioral family intervention: an enhanced self-directed program that combined a self-help program using written materials and a weekly telephone consultation (ESD), a self-help program (SD) and a waitlist control group (WL). At postintervention the ESD group reported significantly lower levels of disruptive behaviour, and lower levels of dysfunctional parenting than the SD and WL controls, and higher levels of consumer satisfaction. At 6 months follow-up the main effects for the ESD group had been maintained. The SD group continued to evidence improvement from postintervention to follow-up such that 65% of children in the ESD condition and 57% of children in the SD condition showed clinical reliable change on measures of disruptive behaviour. Implications of findings and directions for future research are discussed

    Law, politics and the governance of English and Scottish joint-stock companies 1600-1850

    This article examines the impact of law on corporate governance by means of a case study of joint-stock enterprise in England and Scotland before 1850. Based on a dataset of over 450 company constitutions together with qualitative information on governance practice, it finds little evidence to support the hypothesis that common-law regimes such as England were more supportive of economic growth than civil-law jurisdictions such as Scotland: indeed, levels of shareholder protection were slightly stronger in the civil-law zone. Other factors, such as local political institutions, played a bigger role in shaping organisational forms and business practice

    Heavy-ion resonance and statistical fission competition in the Mg24+24Mg system at Ec.m.=44.4 MeV

    The fully energy-damped cross sections of the Mg24+24Mg reaction at Ec.m.=44.4 MeV have been measured for all of the major fission channels. High-resolution Q-value spectra have been obtained for the large-angle yields in the Mg24+24Mg and Ne20+28Si channels. Calculations based on the transition-state model are found to reproduce the fully damped cross sections in all of the observed mass channels. The pronounced structure that is observed in the excitation-energy spectra for the more symmetric mass channels, even for the strongly damped yields, is shown to be qualitatively reproduced by assuming a spin-weighted population of the fragment states. There is no evidence, however, that the structure of the nascent fission fragments at scission may influence the population of states in the fragments. These results, taken together with earlier measurements of the resonance behavior of this system, suggest the coexistence of fission from the normal, compact compound nucleus with that from the deformed configurations believed to be responsible for the resonance behavior

    Fusion of O16 + Ca40 at Elab(16O)=13.4 MeV/nucleon

    Mass and velocity distributions have been measured for the evaporation residue and fusion-fission products from the O16+40Ca reaction at 214 MeV. Comparisons of Monte Carlo statistical evaporation simulations to the observed angle and mass dependences of the evaporation-residue velocity distributions were used to set limits on the maximum complete-fusion cross section and to extract information about the magnitude and character of incomplete-fusion processes. The extracted value of the complete fusion evaporation-residue cross section is discussed in the framework of previous results and existing models

    Selective population of states in fission fragments from the S32+24Mg reaction

    The symmetric and near-symmetric mass fission yields from the S32+24Mg reaction have been studied in a particle-particle- coincidence measurement. Evidence is presented for a selective population of states in Si28 fragments arising from the symmetric fission of the Ni56 compound nucleus. A statistical-model calculation of the expected strength to specific mutual excitations of the fission fragments is presented and compared to the experimental results. This calculation is found to describe the structures observed at high excitation energy in the fission Q-value spectra quite well. Analysis of the -ray spectra indicates, however, that a specific set of states in Si28, corresponding to a highly deformed prolate band, is populated more strongly than expected based on a purely spin-weighted, statistical decay of the compound nucleus. It is suggested that the population pattern of states in the fission fragments may reflect nuclear structure effects at the point of scission

    Routine versus on demand removal of the syndesmotic screw; A protocol for an international randomised controlled trial (RODEO-trial)

    Background: Syndesmotic injuries are common and their incidence is rising. In case of surgical fixation of the syndesmosis a metal syndesmotic screw is used most often. It is however unclear whether this screw needs to be removed routinely after the syndesmosis has healed. Traditionally the screw is removed after six to 12 weeks as it is thought to hamper ankle functional and to be a source of pain. Some studies however suggest this is only the case in a minority of patients. We therefore aim to investigate the effect of retaining the syndesmotic screw on functional outcome. Design: This is a pragmatic international multicentre randomised controlled trial in patients with an acute syndesmotic injury for which a metallic syndesmotic screw was placed. Patients will be randomised to either routine removal of the syndesmotic screw or removal on demand. Primary outcome is functional recovery at 12 months measured with the Olerud-Molander Score. Secondary outcomes are quality of life, pain and costs. In total 194 patients will be needed to demonstrate non-inferiority between the two interventions at 80% power and a significance level of 0.025 including 15% loss to follow-up. Discussion: If removal on demand of the syndesmotic screw is non-inferior to routine removal in terms of functional outcome, this will offer a strong argument to adopt this as standard practice of care. This means that patients will not have to undergo a secondary procedure, leading to less complications and subsequent lower costs.
